The Emerging Role of Immunothrombosis in the Control and Pathogenesis of Mycobacterium tuberculosis.

Greater understanding of the immunopathogenesis of tuberculosis is critical for developing novel therapies. Here, we propose that immunothrombosis plays an important role in the immune response to Mycobacterium tuberculosis. This interplay among macrophages, neutrophils, and platelets leads to microthrombosis at the site of infection, trapping the mycobacterium to prevent dissemination. We explore how dysregulated immunothrombosis might contribute to tuberculosis pathogenesis, with excessive microthrombosis driving drug resistance, leading to lung damage and venous thromboembolism. Further res…

Efficacy of regimens targeting Mycobacterium abscessus in vitro and in vivo.

BACKGROUND: M. abscessus (Mabs) is one of the principal pathogenic strains among nontuberculous mycobacterial. Mabs infections pose a significant global public health challenge, leading to substantial morbidity and mortality. However, a standard treatment regimen has not yet been established. The goal of this study was to provide clear insights into constructing regimens. METHODS: We evaluated the efficacy of 7 clinically available drugs against Mabs under various environments through microplate alamar blue assay (MABA), biofilm assays, Wayne model and nutrient-starvation model. The checkerbo

A Case of Rare Facial Infection Caused by Mycobacterium scrofulaceum.

BACKGROUND: In July 2024, our hospital confirmed a rare case of facial infection with Mycobacterium scrofulaceum. The patient visited our hospital due to pain and pus discharge from the right orbital incision for one month. The patient suffered multiple facial fractures due to trauma three months ago. He underwent systemic anti infection treatment and open reduction and internal fixation surgery at an external hospital. After the surgery, there was repeated swelling around the orbit, and the patient did not fully recover. One month ago, the infraorbital area was swollen again, locally ruptured…

Interaction of Mycobacterium tuberculosis with the Host from the Pathogen's Perspective.

Since the Mycobacterium tuberculosis genome was decoded in 1998, our understanding of this dangerous pathogen - which has become the leading cause of bacterial infection-related deaths in modern human history - has expanded significantly. This review examines genetic, physiological, and metabolic factors believed to play a key role in the adaptation strategies of M. tuberculosis to survival within the human host. These strategies underpin critical aspects of the bacterium life cycle, including infection, persistence, dissemination, and transmission to the new hosts. We also discuss how M. tube…

In vitro and in vivo activity of eravacycline against Mycobacterium abscessus.

Eravacycline is a fluorocycline antibiotic with activity against multidrug-resistant Gram-negative bacteria. However, its activity against Mycobacterium abscessus (MAB) has not been fully evaluated. This study aimed to evaluate the in vitro and in vivo activity of eravacycline against MAB. The in vitro activity of eravacycline against 73 MAB isolates was evaluated by broth microdilution to determine minimal inhibition concentrations (MICs). For in vivo assessment, a BALB/c nude mouse model was established via intravenous infection with MAB, resulting in systemic dissemination of bacteria. Outc…

Mycobacterium marinum as a model for understanding principles of mycobacterial pathogenesis.

Mycobacterium marinum is a fish pathogen that has become a powerful and well-established model that has accelerated our understanding of the mechanisms of mycobacterial disease. M. marinum is a versatile surrogate for understanding the closely related human pathogen M. tuberculosis, which causes tuberculosis in humans. M. marinum has defined key mechanisms of pathogenesis, both shared with M. tuberculosis and unique to this species. In this review, we discuss the discovery of M. marinum as an occasional human pathogen, the shared aspects of pathogenesis with M. tuberculosis, and how M. marinum…

Mycobacterium abscessus persistence in the face of Pseudomonas aeruginosa antagonism.

INTRODUCTION: Chronic bacterial infections are responsible for significant morbidity and mortality in cystic fibrosis (CF) patients. Pseudomonas aeruginosa (Pa), the dominant CF pathogen, and Mycobacterium abscessus (Mab) can individually cause persistent, difficult to treat pulmonary infections. Co-infection by both pathogens leads to severe disease and poor clinical outcomes. Although interactions between Pa and other co-infecting pathogens in CF patients have been the focus of numerous studies, the dynamics of Pa-Mab interactions remain poorly understood. METHODS: To address this knowledge…

Application of antimicrobial drugs in Mycobacterium tuberculosis and research progress.

Treatment regimens for Tuberculosis (TB) primarily rely on first-line drugs such as isoniazid, rifampicin, ethambutol, and pyrazinamide. Although these drugs have been pivotal in the treatment of drug-resistant tuberculosis, their high costs limit widespread use in low- and middle-income countries. However, the emergence of multidrug-resistant TB and extensively drug-resistant TB necessitates the development of new anti-TB drugs and therapeutic strategies. In terms of treatment, new drugs such as bedaquiline, delamanid, and pretomanid have shown significant efficacy against MDR-TB and XDR-TB.

Unveiling the potential of antimicrobial peptides to combat Mycobacterium tuberculosis.

Tuberculosis (TB) is a severe disease that causes considerably high morbidity and mortality in low- and middle-income developing nations. The causative agent of TB, Mycobacterium tuberculosis (Mtb), has evolved medication resistance, which has hampered the treatment of TB. The need for new anti-TB medications has been driven by the growth of multi-drug-resistant (MDR) mycobacterial strains, resistance to current medicines, and a rising financial burden. In this context, antimycobacterial peptides obtained from diverse sources, including human cells, bacteria, mycobacteriophages, plants, and an…

First isolation of Mycobacterium brisbanense from humans in the Americas.

BACKGROUND: Mycobacterium brisbanense is a rare nontuberculous mycobacteria and was for the first time detected in the Americas in humans. However, this, like several other species of mycobacteria, may be underreported worldwide. Therefore, their study is increasingly important. OBJECTIVES: The aim of this article is to report and analyse the first seven human isolates of M. brisbanense in the Americas, derived from six Brazilian patients. METHODS: We sequenced the genes hsp65, rpoB and 16s rRNA, of seven mycobacterial clinical isolates, constructed a phylogenetic tree, and determined their …

Extrapulmonary Mycobacterium malmoense infection in a patient with minimal immunosuppression.

Mycobacterium malmoense is an emerging non-tuberculous mycobacterial (NTM) pathogen that usually causes extrapulmonary disease, particularly, in significantly immunosuppressed patients. Recent reports from Scotland and the Netherlands, however, document an increasing proportion of pulmonary NTM infections due to M. malmoense, indicating a rising incidence in parts of Northern Europe-although still predominantly associated with pulmonary disease. We report a 78-year-old woman from north-eastern Germany with chronic tenosynovitis caused by M. malmoense under minimal long-term prednisone therapy.…

Cutaneous infection with Mycobacterium chelonae in a patient with multiple sclerosis.

Cutaneous infections caused by Mycobacterium chelonae can present with a variety of clinical symptoms, depending on the patient's immune status. Here, we report a case involving a 46-year-old woman with multiple sclerosis who developed a cutaneous infection caused by M. chelonae. The initial presentation included skin discoloration on her right wrist, which progressed to a granuloma. Following surgical intervention, the infection led to tissue atrophy and the formation of a deep cavity at the site. Upon identification of the causative pathogen, a treatment regimen consisting of clarithromycin …

Mycobacterium paratuberculosis: A HERV Turn-On for Autoimmunity, Neurodegeneration, and Cancer?

Human endogenous retroviruses (HERVs) are remnants of ancient retroviral infections that, over millions of years, became integrated into the human genome. While normally inactive, environmental stimuli such as infections have contributed to the transcriptional reactivation of HERV-promoting pathological conditions, including the development of autoimmunity, neurodegenerative disease and cancer. What infections trigger HERV activation? Mycobacterium avium subspecies paratuberculosis (MAP) is a pluripotent driver of human disease. Aside from granulomatous diseases, Crohn's disease, sarcoidosis a…

First recorded isolation of Mycobacterium kansasii in a nonendemic setting.

Nontuberculous mycobacteria (NTM) are a diverse group of environmental mycobacteria found ubiquitously in soil and water, causing infections that typically arise from environmental exposure rather than person-to-person transmission. This case report documents the first known detection of Mycobacterium kansasii in our region, emphasizing the clinical challenges and diagnostic complexities associated with NTM infections. The case involves a 37-year-old male with a significant smoking history who presented with progressively worsening cough, significant weight loss, and bilateral cavitary lung le…

Efficacy of epetraborole against Mycobacterium abscessus is increased with norvaline.

Mycobacterium abscessus is the most common rapidly growing non-tuberculous mycobacteria to cause pulmonary disease in patients with impaired lung function such as cystic fibrosis. M. abscessus displays high intrinsic resistance to common antibiotics and inducible resistance to macrolides like clarithromycin. As such, M. abscessus is clinically resistant to the entire regimen of front-line M. tuberculosis drugs, and treatment with antibiotics that do inhibit M. abscessus in the lab results in cure rates of 50% or less. Here, we identified epetraborole (EPT) from the MMV pandemic response box as…

The tuberculin specificity in humans of Mycobacterium tuberculosis antigen 5.

Mycobacterium tuberculosis antigen 5 is a protein antigen limited in distribution to M. tuberculosis and M. bovis and capable of eliciting typical delayed tuberculin skin test reactions in humans. A single large batch of this antigen was purified by immunoabsorbent affinity chromatography and used to skin test patients with tuberculosis and other mycobacterial infections and healthy persons in general populations in geographic areas where nonspecific tuberculin reactivity is frequently encountered. Antigen 5 was found to be no more specific as a tuberculin antigen than PPD. If the available da…

Leucine auxotrophic marker for genetic manipulation of Mycobacterium abscessus.

UNLABELLED: Mycobacterium abscessus is a clinically important multidrug-resistant pathogen for which genetic manipulation remains challenging. Here, we developed a second-generation genetic toolbox based on leucine auxotrophy that enables antibiotic-free positive selection. A ΔleuB mutant, lacking the gene encoding isopropyl malate dehydrogenase in the leucine biosynthesis pathway, was generated by targeted gene deletion. This mutant requires exogenous leucine for growth and can be complemented by plasmid-borne leuB, establishing a robust auxotrophy-based selection system. To support genetic m…

The Pyridoxal-5'-Phosphate-Dependent Enzymes of Mycobacterium tuberculosis.

Enzymes that depend on the cofactor pyridoxal 5'-phosphate (PLP) catalyze a remarkable variety of biochemical reactions in all organisms. In particular, the genome of Mycobacterium tuberculosis, the causative agent of tuberculosis (TB), encodes 45 bona fide PLP-dependent enzymes plus a few related proteins that presumably do not have enzymic function. The large majority of the 45 enzymes have been characterized in terms of catalytic activity and structure. Several of them have been shown to be central to the bacterium's survival and pathogenicity, while some of these enzymes are targets of an …

Reducing mapping reference and lineage bias in Mycobacterium tuberculosis.

Whole-genome sequencing provides a vast amount of genetic information, but its use in clinical and epidemiological studies often depends on the accurate inference of genomic variants. Comparative genomic studies in Mycobacterium tuberculosis typically involve mapping short reads from a diverse population to the same reference genome. This approach can lead to the incorrect characterization of many genomic regions that are susceptible to mapping bias when the reference is too distantly related to the sample. We analysed the consequences of mapping reads from different lineages of M. tuberculosi…
