Congenital Zika Infection and the Risk of Neurodevelopmental, Neurological, and Urinary Track Disorders in Early Childhood. A Systematic Review.

It was late 2015 when Northeast Brazil noticed a worrying increase in neonates born with microcephaly and other congenital malformations. These abnormalities, characterized by an abnormally small head and often neurological impairment and later termed Congenital Zika Syndrome, describe the severity of neurodevelopmental and nephrological outcomes in early childhood, and the implication of microcephaly at birth. The purpose of the study was to describe the neurodevelopmental outcomes in children exposed to Zika virus during fetal life, with and without microcephaly at birth. The systematic revi

Patient, nurses, and physicians collaborating in the management of a patient following autotransplant parathyroidectomy.

The nephrology nurses in collaboration with the nephrologist worked closely to avoid severe symptomatology related to calcium phosphorus imbalance. The surgical team support was discontinued 48 hours after the removal of the autotransplant. The patient exhibited classic signs of secondary hyperparathyroidism, as indicated by laboratory tests, anemia, and pruritus. Despite the level of anemia, the patient did not complain of fatigue or dyspnea. A hypocalcemic crisis was avoided by the ongoing assessment and intervention the patient received from the nursing staff. Four of the five stated goals …

Physical examination of arteriovenous fistulae by a renal fellow: does it compare favorably to an experienced interventionalist?

Physical examination (PE) has been highlighted to detect vascular access stenosis with high degree of accuracy when performed by an interventional nephrologist (IN) with expertise in physical examination. This study examines the accuracy of PE compared with angiography when performed by a nephrology fellow (NF). It also compares NF results to that of IN. Didactic and hands-on PE training was provided to a renal fellow for 1 month during an interventional nephrology rotation. Forty-five and 142 consecutive cases of arteriovenous fistula dysfunction were examined by the NF and IN, respectively. …

Kidney biopsy in the diagnosis of immunoglobulin A nephropathy.

Immunoglobulin A nephropathy (IgAN) is a kidney disease characterized by deposition of galactose-deficient IgA1-containing immune complexes in the glomerular mesangium. It is the most common primary glomerular disease worldwide, affecting approximately 2.5 persons per 100,000 population annually. A definitive IgAN diagnosis requires a kidney biopsy. However, non-specific clinical signs, limited disease awareness, and lack of validated biomarkers can delay nephrology referral from primary care providers, prolonging time to diagnosis and negatively impacting patient outcomes. In this perspective…

Chronic Kidney Disease in Balkan Countries-A Call for Optimal Multidisciplinary Management.

The treatment of chronic kidney disease (CKD) has been considerably transformed in the last couple of years. However, effective management of patients with CKD is still not achieved, despite clear guidelines promoting active screening of high-risk patients, immediate diagnosis based on laboratory markers, and early initiation or intensification of pharmacotherapy like sodium/glucose cotransporter 2 (SGLT2) inhibitors, which showed reliable results in preventing disease progression, complications, and mortality. Following a recent initiative on early diagnosis, nephrology experts from Bulgaria,…

Calcium and Phosphate Disorders: Core Curriculum 2024.

Maintaining normal calcium and phosphate homeostasis is essential for optimal cellular, metabolic, and organ function. Parathyroid hormone, fibroblast growth factor 23, and 1,25-dihydroxyvitamin D regulate calcium and phosphate homeostasis via multiple interlinked feedback loops, receptors, ion channels, and transporters. Following an initial overview of the stimuli and effects of the different hormonal regulators, this installment of AJKD's Core Curriculum in Nephrology reviews the physiology and pathophysiology of calcium and phosphate disorders through the lens of a series of illustrative c…

Diuretics in States of Volume Overload: Core Curriculum 2022.

Volume overload, defined as excess total body sodium and water with expansion of extracellular fluid volume, characterizes common disorders such as congestive heart failure, end-stage liver disease, chronic kidney disease, and nephrotic syndrome. Diuretics are the cornerstone of therapy for volume overload and comprise several classes whose mechanisms of action, pharmacokinetics, indications, and adverse effects are essential principles of nephrology. Loop diuretics are typically the first-line treatment in the management of hypervolemia, with additional drug classes indicated in cases of diur…

Comprehensive geriatric assessment and optimisation in advanced chronic kidney disease: a systematic review and narrative synthesis examining clinical outcomes and decision-making.

BACKGROUND: Older adults with advanced chronic kidney disease (CKD) frequently experience frailty, multimorbidity, cognitive impairment and functional decline, yet these geriatric syndromes are not routinely evaluated in nephrology services. Comprehensive Geriatric Assessment (CGA) can support personalised treatment decisions and optimise outcomes, but its role in CKD stages 4-5 remains unclear. This systematic review examined the impact of CGA on clinical outcomes, decision-making, and implementation within nephrology services. METHODS: A systematic review was conducted in accordance with PR…

Taking the "Нет данных" out of CKDu-optimizing approaches to uncover the cause(s) of epidemic-level kidney disease in low- and middle-income settings: a report from the ISN's International Consortium of CKDu Collaborators (ISN i3C).

Chronic kidney disease of undetermined etiology (CKDu) is a major cause of mortality that affects communities, health systems, and national economies. Despite 2 decades of descriptive and analytical research, the causes of CKDu epidemics remain unclear. In February 2025, at the World Congress of Nephrology, the International Society of Nephrology's International Consortium of CKDu Collaborators met to discuss the obstacles to determining the cause(s) of the epidemics. Several key themes were identified: First, differences in CKDu burden between regions are poorly described, and this is complic…

Standardized Definitions of Changes in Kidney Function in Trials of Heart Failure: JACC Expert Panel From the HF-ARC.

Perturbations in kidney function are frequently encountered in heart failure (HF) across its spectrum in both chronic and acute settings with distinct implications for patient management and prognosis. Lack of consensus on the threshold for clinically meaningful changes in kidney function has led to heterogeneity in the clinical characteristics and background therapies of individuals enrolled in clinical trials, and in multiple aspects of trial design. A meaningful and collaborative interaction among the disciplines of cardiology and nephrology, clinical trialists, industry sponsors, and regul…
