Six patients who had gastric reduction for morbid obesity suffered severe complex neurologic disturbances that included confusion and inappropriate behavior. All were profoundly weak or paraplegic, and recovery was delayed and incomplete. Encephalopathy was documented by EEG and neurologic examination. The cause is uncertain. Acute catabolism of lipid may predispose to damage of the nervous system, but relative vitamin deficiency is a more obvious and treatable explanation.

A patient was admitted with frequent episodes of transient neurologic symptoms formerly described as "capsular warning syndrome". The neurologic examination revealed right-sided hemiplegia, diminished pinprick, decreased light touch and extensor right plantar response. The only documented risk factor was hypertriglyceridemia, and diagnosis was anterior choroidal artery territory infarction. This report critically reviews the clinical features of this vascular lesion and the MRI findings.

The case of a patient with pathologic Stage 1A Hodgkin disease is reported in whom subacute paraneoplastic myelopathy developed before treatment. Myelography, computed tomography, magnetic resonance imaging, and lumbar puncture examination showed no evidence of central nervous system involvement. The patient was treated with combination chemotherapy (to avoid spinal cord exposure to radiation) without neurologic improvement. The patient's neurologic condition responded well to intrathecal dexamethasone.

OBJECTIVES: Cardiac transplant is the best available therapy for patients with end-stage heart failure. Neurologic complications occur at a … diac transplant between 2004 and April 2016. We recorded the demographic data, neurologic symptoms, neurologic examination findings, laboratory test results, brain imaging study results, and treatments received of the patients.
RESULTS: Of the 70 patients enrolled, 55 were male and 15 were female patients. The age range was 18 to 63 years, and the mean age was 42.4 yea …

UNLABELLED: Children with Guillain-Barré Syndrome (GBS) often do not present like adults with an ascending paralysis and sensory abnormalities, but typically have pain and gait difficulties as predominant symptoms. We present a case of paediatric GBS that was not diagnosed until late in the course because of limited neurological examination, erroneous interpretation of newly acquired data and insufficient familiarity with the disorder in children. Through this case, essentials of paediatric GBS are reviewed.
CONCLUSION: Pain and gait difficulties can be the main features of paediatric GBS at

Dynamic posturography comprises a series of balance control tests which help physicians overcome numerous diagnostic and treatment challenges arising when examining patients complaining of a debilitating balance disorder. These challenges include the specific differential diagnosis, documentation of symptoms and assessment of functional disability. It must be determined whether the cause of the disability is an organic sensory, deficit, a central nervous system (CNS) lesion or a non-organic (that is, possibly psychogenic or just overtly simulated) disorder. This review is targeted towards prov

The majority of obstetric patients are concerned about labor pains but also have fears regarding regional anesthesia and its potential effec … he topics of infections, obstetric- and anesthetic-related neurologic deficits, and special tips on neurologic examination. The most common neurologic complication of spinal and epidural anesthesia, postdural puncture headache, will not be discussed in detail here.

The cauda equina syndrome in ankylosing spondylitis (the CES-AS syndrome) is marked by slow, insidious progression and a high incidence of dural ectasia in the lumbosacral spine. A high index of suspicion for this problem must be maintained when evaluating the patient with ankylosing spondylitis with a history of incontinence and neurologic deficit on examination. There has been disagreement in the literature as to whether surgical treatment is warranted for this condition. A meta-analysis was thus performed comparing outcomes with treatment regimens. Our results suggest that leaving these pat

We report four cases of difficult defaecation and/or faecal incontinence revealing a neurologic disease. These anorectal symptoms were associated with urinary disorders, but urinary symptoms always appeared several years after anorectal complaints. Clinical neurologic examination immediately led us to suspect a neurologic aetiology in two patients with the following signs: perineal hypoesthesia, absence of voluntary anal contraction and anal reflexes in one subject (final diagnosis: L1 neurinoma), and in the other a weakness in both thighs with absent tendon reflexes in the four limbs (final d…

CASE REPORT: A 3-year-old girl was transferred to our hospital with a history of persistent open anterior fontanel. The patient was conscious and had no neurological deficits. Upon arrival, the patient appeared normal for her age and had no defects or anomalies other than the aforementioned lesion. The initial skull X-ray and CT were significant for a 20-mm open anterior fontanel. All other findings were normal.
OUTCOME: After a follow-up period of 1.5 years, the anterior fontanel was still open, with a slight decrease in size to 15 mm. Delayed closure of the anterior fontanel without intracr

A careful and complete headache history supplemented by a neurologic and general physical examination, as appropriate, enables the astute physician to diagnose most headaches correctly without diagnostic testing. When indications are present (see Box 1), some headache patients with a normal physical examination require testing even though the yield may be low. Failure to test may result in misdiagnosis of potentially serious and life-threatening causes of headaches, such as brain tumors, chronic meningitis, SAH, and temporal arteritis.

The foot is a remarkable structure that provides stable support for the body, lessens impact loading during gait, and allows for forward motion that is energy efficient. Because of the enormous loads concentrated at the joints in the foot and ankle with normal activities, frequent injuries occur and present to the primary care provider. Systemic conditions and improper footwear also contribute to the number of foot injuries that require medical attention. This article provides the clinician with the practical tools necessary for a complete examination of the foot and ankle. This article also i

Although the neuropathologic findings in patients who have died of systemic lupus erythematosus (SLE) are well documented, the cytomorphologic reactions present in the cerebrospinal fluid (CSF) of living patients with transient neurologic disturbances associated with SLE are poorly understood. Short of brain biopsy, a CSF cytomorphologic examination may provide the best clue to the nature of the cellular inflammatory process in these transient CNS disorders. Cells in the CSF from a patient with SLE who developed transient cortical blindness with denial of visual loss (Anton's syndrome) were su
