Continuous Intraoperative Nerve Monitoring in Thyroid Surgery: Can Amplitude Be a Standardized Parameter?

The objective of this study is to evaluate electromyographic waveforms related to vagus monitoring. We collected data from patients undergoing thyroidectomy with CIONM, regardless of vocal cord response amplitude initially measured. We divided data of 193 nerves into three groups, according to initial amplitude value: ≥500 µV (Group 1,110 pt.), between 100 and 500 µV (Group 2, 79 pt.), and <100 µV (Group 3, 4 pt.). ROC curve showed a high diagnostic accuracy of final amplitude absolute value in vocal cord paralysis detection in both groups (89 and 86%). An increase of vocal cord paralysis risk

Outcomes of hippocampus-sparing lesionectomy for temporal lobe epilepsy and the significance of intraoperative hippocampography.

OBJECTIVES: We investigated hippocampal-sparing lesionectomy (HSL) outcomes in temporal lobe epilepsy (TLE) and the significance of high-frequency oscillations (HFOs) detected by hippocampography in HSL. METHODS: We retrospectively reviewed data from patients who underwent HSL for lesional TLE. Patients were included when MRI confirmed (i) a lesion limited to the temporal lobe with normal hippocampi preoperatively and (ii) hippocampal integrity postoperatively. Factors possibly related to outcomes were collected. Intraoperative hippocampography was reviewed, and spikes, ripples, and fast ripp

The prediction of intraoperative cervical cord function changes by different motor evoked potentials phenotypes in cervical myelopathy patients.

BACKGROUND: Surgery is usually the treatment of choice for patients with cervical compressive myelopathy (CCM). Motor evoked potential (MEP) has proved to be helpful tool in evaluating intraoperative cervical spinal cord function change of those patients. This study aims to describe and evaluate different MEP baseline phenotypes for predicting MEP changes during CCM surgery. METHODS: A total of 105 consecutive CCM patients underwent posterior cervical spine decompression were prospectively collected between December 2012 and November 2016. All intraoperative MEP baselines recorded before spin

Acute effects of adaptive Deep Brain Stimulation in Parkinson's disease.

BACKGROUND: Beta-based adaptive Deep Brain Stimulation (aDBS) is effective in Parkinson's disease (PD), when assessed in the immediate post-implantation phase. However, the potential benefits of aDBS in patients with electrodes chronically implanted, in whom changes due to the microlesion effect have disappeared, are yet to be assessed. METHODS: To determine the acute effectiveness and side-effect profile of aDBS in PD compared to conventional continuous DBS (cDBS) and no stimulation (NoStim), years after DBS implantation, 13 PD patients undergoing battery replacement were pseudo-randomised i

Correlation Between Electrophysiological Changes and Outcomes of Vocal Cord Function in 1764 Recurrent Laryngeal Nerves with Visual Integrity During Thyroidectomy.

Background: The correlation between the injured recurrent laryngeal nerve (RLN) with incomplete loss of signal (LOS) and the outcomes of vocal cord function is still not well understood. This large cohort validation study was aimed to evaluate the effectiveness of the warning criterion, the reduction of the R2p/R2d ratio, in neuromonitoring during thyroidectomy. Methods: A total of 1108 consecutive patients (1764 nerves) with normal vocal cord function undergoing monitored thyroidectomy were included. Standardized intraoperative neuromonitoring procedures were strictly followed, after complete

Electrophysiological Mapping and Monitoring during an Awake Craniotomy for Low-Grade Glioma: Case Report.

Awake craniotomy is advocated for the resection of supratentorial low-grade gliomas (LGG). The combination of neurophysiological electrical mapping techniques and performing the craniotomy awake has demonstrated increased total and supratotal resection of LGG, as well as increased overall survival rates. We present an illustrative case where the patient's gross motor function deteriorated during the resection of a LGG and mapping techniques using the phase reversal technique and Taniguchi direct cortical stimulation technique while the patient was awake proved to be valuable in determining con

Awake microvascular decompression with fat-teflon sandwich technique: Clinical implications of a novel approach for cranial nerve neuralgias.

Re-appearance of trigeminal neuralgia (TN) pain following microvascular decompression (MVD) is a challenging issue. A selective ablation with MVD provides the best response in such recurrences. The absence of intra-operative indicator for immediate correction of sub-optimal decompression is the primary factor for failure. We analysed the effectiveness and safety of awake MVD in minimizing failure, by tailoring the procedure according to intra-operative response with re-exploration or additional procedure like internal neurolysis in the same setting, especially in patients without vascular comp

Intraoperative Erb's Point-Vertex recording increases brainstem auditory evoked potential wave V amplitude.

OBJECTIVE: Recording derivations for intraoperative brainstem auditory evoked potential (BAEP) monitoring consist of a preauricular electrode referenced to Cz'. These derivations are prone to unfavorable signal amplitude. This study analyses whether an alternative noncephalic electrode positioned over ipsilateral Erb's point, thereby generating a new Erb's point-vertex recording derivation, improves BAEP recordings. METHODS: Electrodes were placed preauricularly (A1/A2) and at left and right Erb's point (EP1/EP2). They were referenced to Cz'. Click sound stimulation (80-95 dB above hearing le

Hearing preservation after removal of small vestibular schwannomas by retrosigmoid approach: comparison of two different ABR neuromonitoring techniques.

BACKGROUND AND OBJECTIVE: Goals of small vestibular schwannoma (VS) microneurosurgery are as follows: radical resection, facial nerve (FN) preservation, and hearing preservation (HP). Microsurgical advances make HP possible in many patients with preoperative socially useful hearing (SUH). We evaluated postoperative HP in VS with maximum diameter < 2 cm monitored with two different auditory brainstem response (ABR) techniques. MATERIALS AND METHODS: Twenty-eight consecutive non-randomized patients with SUH suffering from small VS underwent keyhole microneurosurgery by retrosigmoid (RS) approac

Introduction of Continuous Video EEG Monitoring into 2 Different NICU Models by Training Neonatal Nurses.

BACKGROUND: Continuous video electroencephalographic (EEG) (cvEEG) monitoring is emerging as the standard of care for diagnosis and management of neonatal seizures. However, cvEEG is labor-intensive and the need to initiate and interpret studies on a 24-hour basis is a major limitation. PURPOSE: This study aims at establishing consistency in monitoring of newborns admitted to 2 different neonatal intensive care units (NICUs) managed by the same neurocritical care team. METHODS: Neonatal nurses were trained to apply scalp electrodes, troubleshoot technical issues, and identify amplitude-integ

Management of a 3-year-old with an unstable C6-C7 diastasis without quadriplegia.

BACKGROUND: Caring for pediatric spine trauma patients places spine surgeons in situations that require unique solutions for complex problems. Recent case reports have highlighted a specific injury pattern to the lower cervical spine in very young children that is frequently associated with complete spinal cord injury. METHODS: This report describes the presentation and treatment of a C6-C7 dislocation in a 3-year-old patient with an incomplete spinal cord injury. The highly unstable cervical injury and the need to prevent neurologic decline added complexity to the case. RESULTS: A multi-sur

Intraoperative recurrent laryngeal nerve monitoring in revision thyroidectomy.

Recurrent laryngeal nerve (RLN) palsy is one of the most important complications after thyroid and parathyroid surgery. There is controversy in the literature regarding whether or not intraoperative nerve monitoring decreases the risk of injury. We report our experience using Nerve Integrity Monitoring (NIM) system in cases of revision thyroidectomy, providing clear indications for its use. A series of 97 patients who underwent revision thyroidectomy with and without intraoperative NIM alternately was evaluated. There were 121 RLN patients at risk. Use of NIM during revision thyroidectomy was
