Over 25 years ago survival was the sole aim in the management of cancer patients. However, over the past 25 years quality of life (QOL) as well as survival has become increasingly important in the treatment of cancer. In other words, treatment modalities without good QOL are no longer accepted. Thus, supportive therapies for cancer patients are very important. These supportive therapies fall into two categories. One is the management of adverse events induced by cancer therapies such as surgery, radiotherapy or chemotherapy. Adverse events induced by the administration of anti-cancer drugs in

PURPOSE OF REVIEW: Undernutrition and sarcopenia are common in patients with cirrhosis and independently predict increased morbidity and mortality. Despite recent guidelines emphasizing nutritional intervention, implementation in clinical practice remains inconsistent. This review is informed by key guidelines including the 2025 ACG guidelines on malnutrition and perioperative management in cirrhosis and the 2024 AASLD practice guidance on acute-on-chronic liver failure, and supplemented by newer meta-analyses and reviews, linking evidence-based recommendations to practical patient care strate

INTRODUCTION: Advances in treatment strategies for gastric and esophageal cancer have led to improved long-term outcomes, however the local and systemic effects of the primary tumor, neoadjuvant therapies and surgery, result in specific nutritional challenges. Comprehensive nutritional evaluation and support represents a core component of multidisciplinary holistic care for this patient population.
AREAS COVERED: We provide a detailed overview of nutritional challenges in gastric and esophageal cancer, with a focus on malignant obstruction, preoperative optimization and survivorship. We discu

PURPOSE OF REVIEW: Critical care nutrition remains a high-stakes and error-prone domain, particularly given the complex metabolic demands and heterogeneity of ICU populations. This review explores recent progress in integrating artificial intelligence with nutritional therapy in ICUs, highlighting its evolution and potential benefits in precision-guided support, along with current implementation challenges.
RECENT FINDINGS: Widely used in adult and neonatal ICUs, parenteral nutrition faces persistent challenges including ordering errors, practice variability, and insufficient robust long-term

Malnutrition is the most frequent nutritional disorder in patients with gastrointestinal cancer and is associated with cachexia syndrome, worsening of prognosis, and shortened survival rate. Early nutrition screening, assessment, and intervention are able to favorably modify the clinical evolution of affected patients. The adequate provision of nutritional requirements has been associated with improvement of immunologic status, and avoidance of further complications related to poor nutritional status, surgical treatment, and anticancer therapy. In malnourished patients, the supplementation of

In the past year, some relevant papers related to the diagnosis of malnutrition and its pathogenesis in cirrhosis have been published. The value of anthropometrics in the nutritional assessment of end-stage cirrhotic patients has been reinforced. Also, the role of bioelectrical impedance analysis in these patients has been redefined. Several papers have investigated the relationship between leptin and malnutrition in chronic liver disease, particularly the role of alcoholism in hyperleptinaemia, and the importance of protein-bound leptin in these patients. In other papers, the impact of both u

In the last three decade very important advances in venous access, enteral feeding techniques and parenteral and enteral nutrient formulations have made it possible to provide sufficient nutritional support to almost all patients. The clinical nutritional therapy became a progressive medical subspecialty. Despite the widespread use of nutritional management of different patient groups, many fields of nutritional support remain controversial. The critical review of the current medical literature is necessary evaluating the effectiveness of nutritional support in the perioperative period. This d…

BACKGROUND & AIMS: A pragmatic trial and its secondary analyses have demonstrated that nutritional care not only reduces complications but also significantly improves survival in medical patients at risk of malnutrition. In contrast, for critically ill patients comparable evidence is scarce. Consequently, many propositions for refining the research agenda and study design in the field of critical care nutrition have already been made. The aim of this paper is to elucidate further critical problems in nutritional care.
METHODS: Critical appraisal of the literature from the past 70 years.

BACKGROUND: Alcoholic liver disease (ALD) is associated with a high risk of morbidity and mortality. Malnutrition accompanies this condition and may be both a consequence of and contributor to the pathology. Many trials have investigated the benefits of providing supplemental nutrition in the management of patients with ALD. The present study is a meta-analysis of the available evidence.
METHOD: A meta-analysis of randomized controlled studies comparing nutritional supplementation plus a normal hospital diet versus diet alone.
RESULTS: Seven randomized controlled studies including 262 patien

Inflammatory Bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), are chronic gastrointestinal conditions that are prone to malnutrition due to poor oral intake, intestinal compromise of nutrient absorption, and increase in metabolic demand. Screening and diagnosing malnutrition in this population is necessary to treat and prevent worsening malnutrition. The use of Oral Nutritional Therapy (ONS) can provide the macronutrients that patients need to maintain their nutrition, however their role in within stages of diseases, active disease, remission, perioperative, and

Chronic obstructive pulmonary disease (COPD) is a complex and heterogeneous lung condition characterized by persistent airflow obstruction, associated with cardiovascular, metabolic and musculoskeletal comorbidities. In 2022, approximately 480 million people were affected by this disease, making it the third leading cause of mortality worldwide. Projections indicate that by 2050, this number could rise to 600 million. Nutrition status is a fundamental component in managing COPD patients, as it is a critical prognostic factor for morbidity and mortality. Patients with COPD may display different

PURPOSE OF REVIEW: Nutrition remains a key focus in the preoptimization of patients undergoing cancer surgery. Given the catabolic nature of cancer, coupled with the physiological insult of surgery, malnutrition (when assessed) is prevalent in a significant proportion of patients. Therefore, robust research on interventions to attenuate the detrimental impact of this is crucial.
RECENT FINDINGS: As a unimodal prehabilitation intervention, assessment for malnutrition is the first step, as universal supplementation has not been shown to have a significant impact on outcomes. However, targeted n

Numerous strategies for perioperative nutrition therapy for patients undergoing pancreaticoduodenectomy (PD) have been proposed. This systematic review aimed to summarize the current relevant published randomized controlled trials (RCTs) evaluating different nutritional interventions via a traditional network meta-analysis (NMA) and component network meta-analysis (cNMA). EMBASE, MEDLINE, the Cochrane Library, and ClinicalTrials.gov were searched to identify the RCTs. The evaluated nutritional interventions comprised standard postoperative enteral nutrition by feeding tube (Postop-SEN), preope

The majority of symptomatic patients with congestive heart failure have been shown to be significantly malnourished. Myocardial and skeletal muscle energy reserves are also diminished. Total daily energy expenditure in these patients is less than that in control individuals, and high protein-calorie feeds do not reverse the abnormalities; thus, the wasting that occurs in patients with congestive heart failure is metabolic rather than because of negative protein-calorie balance. Several specific deficiencies have been found in the failing myocardium: a reduction in the content of L-carnitine, c

UNLABELLED: A COMMON PROBLEM: Undernutrition occurs when nutrient intake does not meet nutritional needs. Selective food intake induced micronutrient deficits (moderate undernutrition) and can later lead to protein calorie malnutrition (PCM). PCM is often discovered during acute illness (increased nutritional needs). PCM is observed in 30 to 50% of the institutionalized population and in 2-4% of the elderly living at home. Micronutrient deficits are far more frequent and concern 4 million elderly persons in France. AGE-RELATED CHANGES: Decreased smell and taste capacities and the inability to
