The ophthalmologic manifestations of the cardio-facio-cutaneous syndrome.

The cardio-facio-cutaneous (CFC) syndrome is an uncommon multiple congenital anomalies/mental retardation syndrome whose major manifestations are congenital heart defects, relative macrocephaly, stunted growth, ectodermal dysplasia, characteristic facial appearance, and psychomotor developmental delay. All described cases were sporadic and cytogenetically normal. We report three additional patients with this diagnosis. All three patients developed strabismus, requiring extraocular muscle surgery. Two of our patients also had nystagmus. Combining the 18 previously reported cases with our additi

Ophthalmologic assessment of young patients with Alport syndrome.

BACKGROUND: Alport syndrome is an X-linked disease affecting basement membrane collagen. It is characterized by nephritis associated with high-tone sensorineural hearing impairment and ophthalmic signs. Although ocular changes have been described in adults, few data exist regarding the incidence of abnormal ocular features in adolescence and childhood. METHODS: Fifteen male and five female patients with Alport syndrome underwent ophthalmologic, audiologic, and nephrologic assessments. All patients studied had hematuria and a positive family history of Alport syndrome. Thirteen patients had a

Mitochondrial mutations in neuro-ophthalmological diseases. A review.

Mutations in the genetic material of mitochondria have been described in patients with a range of neuro-ophthalmological and neuromuscular disorders. Many cases of Leber's hereditary optic neuropathy are caused by a single point mutation, for example, and Kearns-Sayre syndrome, chronic external ophthalmoplegia, and other mitochondrial cytopathies are frequently associated with large-scale deletions of mitochondrial genes. A knowledge of the role of the mitochondrial genome and of the precise nature of these mutations is important in understanding the etiology of such diseases and is already le

[Neurologic and ophthalmologic complications of mitral valve prolapse].

The author investigated the incidence of cerebrovascular attacks in a group of 68 patients with mitral valve prolapse. In 10 of them (14.7%) he detected neurological and opthalmological symptoms. In six patients this symptomatology appeared for the first time, in four it had been recorded already in the past. Consistent with data in the literature, the author is in favour of the view that the neurological symptomatology in mitral with mitral valve prolapse is conditioned by a thromboembolic mechanism. The basis for the formation of micro- and marco- thrombi is an impaired continuity of the end

Comparative ophthalmologic studies on children and adults with craniopharyngiomas.

The subjects of the study are 74 patients with craniopharyngiomas treated at the Clinic of Neurosurgery, Bulgarian Medical Academy, within the period 1960-1989. Fifty of the patients are children and 24 adults. The diagnosis of all patients is made on the base of clinical, X-ray and histological studies. The ophthalmologic state was studied in all patients, prior to operation, via routine methods. Reduced visual acuity was established in 69.6% of the children and 81.8% of the adults. The presence of congestive optic papilla was diagnosed in 22% of the children and 12.5% of the adults. Optic at

[The characteristics of the therapeutic procedure in ophthalmological operations on diabetics].

The paper presents data about the influence of presurgical preparation, a special tactics of management on the day of operation and the first postoperative days on the incidence of complications, visual acuity and the term of the patient's stay in a specialized bed after cataract extraction and antiglaucomatous operations in patients of elderly and senile age, suffering from diabetes mellitus. It is substantiated that specialized ophthalmological hospitals of a surgical profile should have a therapist on staff with obligatory endocrinologic preparation.

Ophthalmologic findings in the Cornelia de Lange syndrome.

Cornelia de Lange Syndrome (CDLS) is characterized by mental retardation, growth retardation, skeletal anomalies, and a characteristic facies. We examined 22 children with CDLS to ascertain the relative frequencies of oculofacial and ophthalmologic abnormalities. We confirm prior reports of the frequent findings of brow hypertrichosis, synophrys, and long arcuate eyelashes. In addition we documented the association with ptosis, nystagmus, and high myopia. Contrary to previous reports, we did not find blue sclera or corneal, pupillary, or retinal disorders to be part of this syndrome. Hypertelo

Clinical ophthalmological tests for the diagnosis of keratoconjunctivitis sicca.

According to the Copenhagen criteria for Sjögren's syndrome, the diagnosis of keratoconjunctivitis sicca requires the performance of 3 objective ocular tests of which 2 should give pathological results. The tests presently in use are: Break-up time, Schirmer-1-test and Rose-Bengal score. A list of clinical ophthalmological tests has been established in order to enable various SS-study groups to compare results. The list has 3 groups, each containing tests evaluating mainly one characteristic function of the lacrimal system: Tear production, tear quality and ocular surface condition. The tests

The clinical neuro-ophthalmologic spectrum of temporal arteritis.

The range of neuro-ophthalmologic signs in temporal arteritis is broad and includes diverse presentations of ischemic optic neuropathy, retinal infarction, transient ischemic phenomena, ophthalmoparesis, pupillary autonomic and anterior ocular segment dysfunction, cortical blindness and associated post-chiasmal field defects, and complex visual hallucinations. Neurovascular compromise can follow arteritic lesions at multiple neuroanatomic sites, and reflects different pathogenetic mechanisms and displays distinctive clinical features. A variety of temporal clinical profiles and differential re

Ophthalmologic manifestations of X-linked childhood adrenoleukodystrophy.

The ophthalmologic findings in 15 patients with childhood adrenoleukodystrophy (ALD) are reviewed. In this X-linked demyelinating disease with adrenal dysfunction, relentlessly progressive visual loss followed by optic atrophy occurs months to years after the diagnosis is established based on neurologic symptoms and biochemical abnormalities. Visual loss is mainly due to central nervous system (CNS) demyelination involving the visual tracts, but primary retinal ganglion cell degeneration may also be operative. All patients in this study were male. Vision ranged from 20/20 to no light perceptio

Nevus sebaceus associated with major ophthalmologic abnormalities.

Nevus sebaceus rarely occurs as part of a syndrome consisting of central nervous system and ophthalmologic abnormalities. We describe a case of nevus sebaceus associated with an epibulbar complex choristoma and colobomas of the optic disc and peripapillary choroid, and review the dermatologic, ophthalmologic, and neurologic literature on the nevus sebaceus syndrome. When associated with other developmental abnormalities, nevus sebaceus and epidermal nevus have erroneously been considered to be a single entity. In this article, we delineate and emphasize the ophthalmologic abnormalities associa

The role of medical imaging in the practice of neuro-ophthalmology.

The neuro-ophthalmologist is concerned with the myriad afflictions that can cause transient or permanent visual loss, derangement of ocular motor function, pupillary dysfunction, abnormality of lid position or function, and headache or facial pain syndrome. Once preliminary clinical evaluation has implicated dysfunction of a certain anatomic locale, the practitioner must precisely confirm or eliminate possible etiologies. This article examines pathology in six areas: orbital contents and optic nerve; cavernous sinus; chiasm; posterior visual pathway and cerebral cortex; brain stem and posterio

[Ophthalmologic findings in 11 q-deletion syndrome].

A 5-year-old boy with deletion of the long arm of chromosome 11 presented with trigonocephaly, bilateral ptosis, epicanthus, antimongoloid lid axes, and bilateral iris coloboma. In order to avoid complication by a torticollis a Guyton-Friedenwald fadenoperation was performed. In 1981 Grosse et al. listed among the pediatric symptoms retarded development, ventricular septum defect, mitral stenosis, and skeletal and urogenital anomalies.

Ophthalmological signs and prognosis in patients with a subarachnoid haemorrhage.

In cases of subarachnoid haemorrhage from various causes, ophthalmological signs can be a feature. In the clinical examination they can be of assistance in determining the site of the aneurysm or other responsible lesion, and they can also be of prognostic value. In a series of 48 cases the significance of these ocular signs is evaluated and discussed.

Optic tract syndrome. Neuro-ophthalmologic considerations.

A review of the case histories of ten patients with optic tract lesions, an uncommon clinical entity, showed the primary characteristic to be a homonymous visual field defect that may be complete or incomplete. When the defect is incomplete, there is relative incongruity. When it is complete, there is an associated contralateral relative afferent pupillary defect. Visual acuity and color vision are preserved, unless there is bilateral involvement or anterior extension to involve the optic nerve or chiasm. When of sufficient duration, the contralateral fundus demonstrates band or "bow tie" atro

The ophthalmological significance of the basal cell naevus syndrome.

Ten cases of the basal cell naevus syndrome (BCNS) are reported. The study is based on clinical findings with no investigations beyond two skull radiographs. The diagnosis is made on some or all of the following findings: family history, jaw cysts, progressive development of basal cell carcinomas (BCCs), pitting of the palms and soles, frontal bossing, abnormality of the skeletal system and ectopic calcification. Eight of the cases are in one family covering three generations. With the exception of a one-year-old child in the third generation (not included) all of this group have the syndrome.
