Disease severity scoring systems in mucosal lichen planus: A systematic review.

Several scoring systems have been developed to evaluate disease severity in mucosal lichen planus, but only a few have been validated to ensure reproducible and accurate assessment of disease severity. The current systematic review was undertaken to identify clinical severity scoring systems in mucosal lichen planus that have undergone validity or reliability testing and to describe their operating characteristics. We performed a bibliographic search in five databases from their inception to October 2022 for severity scoring systems in mucosal lichen planus that have undergone validity or reli

The association between presenting complaints and clinical outcomes in emergency department patients of different age categories.

BACKGROUND AND IMPORTANCE: Although aging societies in Western Europe use presenting complaints (PCs) in emergency departments (EDs) triage systems to determine the urgency and severity of the care demand, it is unclear whether their prognostic value is age-dependent. OBJECTIVE: To assess the frequency and association of PCs with hospitalization and mortality across age categories. METHODS: An observational multicenter study using all consecutive visits of three EDs in the Netherlands Emergency department Evaluation Database. Patients were stratified by age category (0-18; 19-50; 51-65; 66-8

Uric acid could be a marker of cardiometabolic risk and disease severity in children with juvenile idiopathic arthritis.

UNLABELLED: In addition to disease-specific complications, juvenile idiopathic arthritis (JIA) has been linked to metabolic impairments in adults. Recent data supported the usefulness of uric acid (UA) as risk factor for cardiometabolic derangements. Given the lack of pediatric evidence in this field, we aimed to explore this association in a cohort of children diagnosed with JIA. We retrospectively evaluated 113 children diagnosed with JIA classified according to the International League of Association for Rheumatology (ILAR) criteria attending our Rheumatology Clinic. Both clinical and bioch

The spectrum of staphylococcal scalded skin syndrome: a case series in children.

Staphylococcal scalded skin syndrome (SSSS) is a disease caused by certain toxigenic strains of Staphylococcus aureus. While the classic severe phenotype is widely recognized in children, SSSS in fact exists on a spectrum with mild and moderate variants. Misunderstanding the phenotypic spectrum of SSSS may result in misdiagnosis of an otherwise treatable condition. To increase awareness of the heterogeneity of SSSS, we report four cases that together represent a range of clinical presentations.

[The concept of infectious diseases pathogenesis].

In this article a concept of infectious disease pathogenesis as consisted with clinical symptoms is provided. The course of disease, immediate and long-term consequences depend on the mode of entry. If the infection comes via oropharynx, airway, gastrointestinal tract or via skin, the immune system provides adequate immune response. This leads to typical symptoms, cyclical clinic progression and usually to the recovery with the formation of full sterile immunity. In case of parenteral way of infection, which includes perinatal way, there is no full mode of entry, the disease takes chronic cour

Corticosteroid-induced central serous chorioretinopathy in patients with ocular inflammatory disorders.

BACKGROUND: Development of central serous chorioretinopathy (CSC) following the administration of corticosteroids by diverse routes is a well-known fact. We report acute visual loss after the use of systemic corticosteroids in three patients with long-standing ocular inflammatory disorders in whom CSC could initially be misinterpreted as a worsening of the primary inflammatory condition. METHODS: We analyzed the clinical findings and the fluorescein and indocyanine green (ICG) angiographic signs in those three patients. RESULTS: The first patient had birdshot chorioretinopathy with minimal f

[Ophthalmological experiences with automobile drivers with inadequate vision].

A total of 369 ophthalmological practices were asked to report the number of patients seen within one test week who drove their cars but did not satisfy minimum legal requirements for diurnal visual acuity applicable for applicants for a West German Class 3 driver's license. There were 471 such patients. From this number it can be calculated that approximately 570 000 individuals (+/- 9%) drive cars in the Federal Republic of Germany with inadequate visual acuity. An analysis of the questionnaire produced the following results: Fifty percent of these patients are less than 62 years of age. The

Recurrent and persistent refractive errors after restarting isotretinoin treatment: a case report.

BACKGROUND: Isotretinoin is a vitamin A derivative widely used for moderate-to-severe acne, and is known to cause multiple systemic and ocular adverse events. Refractive errors, such as myopia and astigmatism, are commonly considered reversible after discontinuation of isotretinoin treatment. However, the visual changes and subsequent prognosis after re-exposure to isotretinoin are rarely reported. CASE PRESENTATION: A 21-year-old male initiated oral isotretinoin at 10 mg twice daily for acne vulgaris. Twelve days later, the patient developed acute visual deterioration. The eye examination re
