Causes of community stillbirths and early neonatal deaths in low-income countries using verbal autopsy: an International, Multicenter Study.

OBJECTIVE: Six million stillbirths (SB) and early neonatal deaths (END) occur annually worldwide, mostly in rural settings distant from health facilities. We used verbal autopsy (VA), to understand causes of non-hospital, community-based SB and END from four low-income countries. STUDY DESIGN: This prospective observational study utilized the train-the-trainer method. VA interviewers conducted standardized interviews; in each country data were reviewed by two local physicians who assigned an underlying causes of deaths (COD). RESULT: There were 252 perinatal deaths (118 END; 134 SB) studied

The Canadian Perinatal Network: a national network focused on threatened preterm birth at 22 to 28 weeks' gestation.

OBJECTIVE: The Canadian Perinatal Network (CPN) maintains an ongoing national database focused on threatened very preterm birth. The objective of the network is to facilitate between-hospital comparisons and other research that will lead to reductions in the burden of illness associated with very preterm birth. METHODS: Women were included in the database if they were admitted to a participating tertiary perinatal unit at 22+0 to 28+6 weeks' gestation with one or more conditions most commonly responsible for very preterm birth, including spontaneous preterm labour with contractions, incompete

Comparison of Perinatal Outcome of Preterm Births Starting in Primary Care versus Secondary Care in Netherlands: A Retrospective Analysis of Nationwide Collected Data.

Introduction. In Netherlands, the obstetric care system is divided into primary and secondary care by risk level of the pregnancy. We assess … secondary care. Multivariable logistic regression analyses were performed to calculate the risk of perinatal mortality and Apgar score ≤4. Results. Of all preterm deliveries, 42% had labor onset and 7.9% had also delivery in primary care. Women with labor onset between 34(+0) and 36(+6) weeks who were referred before delivery to secondary care had the lowest risk of …

Perinatal outcomes in growth-restricted twins compared with age-matched growth-restricted singletons.

The objective of this study was to compare the perinatal outcomes in growth-restricted (IUGR) twins with a matched group of IUGR singletons. We performed a retrospective cohort study evaluating the perinatal outcomes in IUGR twins matched for gestational age at delivery to the next four IUGR singletons (1:4) in our perinatal database. The perinatal outcomes evaluated included rates of uncorrected perinatal mortality (PMR), admission to the neonatal intensive care unit (NICU), respiratory distress syndrome, grades 3 or 4 intraventricular hemorrhage, necrotizing enterocolitis, periventricular le…

Ethnic differences in stillbirth and early neonatal mortality in The Netherlands.

BACKGROUND: Ethnic disparities in perinatal mortality are well known. This study aimed to explore the contribution of demographic, socioeconomic, health behavioural and pre-existent medical risk factors among different ethnic groups on fetal and early neonatal mortality. METHODS: We assessed perinatal mortality from 24.0 weeks' gestation onwards in 554 234 singleton pregnancies of nulliparous women in the linked Netherlands Perinatal Registry over the period 2000-2006. Logistic regression modelling was used. RESULTS: Considerable ethnic differences in perinatal mortality exist especia…

Evaluation of an optimal gestational age cut-off for the definition of early- and late-onset fetal growth restriction.

OBJECTIVE: The terms early- and late-onset fetal growth restriction (FGR) are commonly used to distinguish two phenotypes characterized by differences in onset, fetoplacental Doppler, association with preeclampsia (PE) and severity. We evaluated the optimal gestational age (GA) cut-off maximizing differences among these two forms. PATIENTS AND METHODS: A cohort of 656 consecutive singleton pregnancies with FGR was created. We used the decision tree analysis to evaluate the GA cut-off that best discriminated perinatal mortality, association with PE and adverse perinatal outcome (fetal demise, …

Social equity in perinatal survival: a case-control study at hospitals in Kigali, Rwanda.

AIM: Rwanda has invested heavily in improving maternal and child health, but knowledge is limited regarding social equity in perinatal survival. We analysed whether perinatal mortality risks differed between social groups in hospitals in the country's capital. METHODS: A case-control study was carried out on singleton births aged at least 22 weeks of gestation and born in district or tertiary referral hospitals in Kigali from July 2013 to May 2014. Perinatal deaths were recorded as they occurred, with the next two surviving neonates born in the same hospital selected as controls. Conditional …

Quality assessment of perinatal regionalization by multivariate analysis: Illinois, 1991-1993.

OBJECTIVE: To identify (1) those elements in the infrastructure of a regionalized perinatal network that have independent effects on the variation in perinatal mortality among nontertiary units (member level I and II hospitals) and (2) shortcomings, if any, in a traditional perinatal data base that impede quality assessment of contemporary regionalized care. METHODS: We analyzed perinatal surveillance data for 3 years, from 1991 to 1993, in the state of Illinois, representing more than 190,000 annual births. Fetal death and neonatal mortality rates for the 97 nontertiary hospitals studied wer…

Infants with single umbilical artery studied in a national registry. 2: Survival and malformations in infants with single umbilical artery.

Registry data on all infants born in Sweden between 1983 and 1986 are reviewed to describe perinatal mortality and malformation rate of infants with single umbilical artery (SUA). Since SUA is much more common in infants with chromosomal anomalies and in twins this analysis is confined to the 1694 singletons who were not recorded as having chromosomal anomalies. There was significantly increased risk of perinatal mortality among these infants largely due to an increased frequency of congenital malformations, low birthweight and preterm delivery. Nevertheless, at least 36% of the perinatal mort…

[Single umbilical artery: review of 12 cases].

The umbilical cord normally consists of three vessels, two arteries and one vein. The absence of one umbilical artery or single umbilical artery (SUA) has been associated with some adverse perinatal events, such as low birth weight, prematurity, congenital malformations and perinatal mortality. The authors present 12 cases of SUA confirmed by histologic examination and the principal maternal and perinatal features of these cases are analyzed. In our study there were a prematurity rate of 58%, a low birth weight rate of 66% and a perinatal mortality rate of 580/1000. Among our patients, 42% of …

Maternal and perinatal outcome of conservative management of severe preeclampsia in midtrimester.

Pregnancies complicated by severe preeclampsia in the second trimester are associated with a high incidence of perinatal morbidity and morta … e 60 pregnancies resulting in a stillborn infant, and 21 resulting in a neonatal death, for a total perinatal mortality rate of 87%. Thus, the results of this retrospective study do not support the use of conservative management for pregnancies complicated by severe preeclampsia in the second trimester.
