Placental transport in response to altered maternal nutrition.

The mechanisms linking maternal nutrition to fetal growth and programming of adult disease remain to be fully established. We review data on changes in placental transport in response to altered maternal nutrition, including compromized utero-placental blood flow. In human intrauterine growth restriction and in most animal models involving maternal undernutrition or restricted placental blood flow, the activity of placental transporters, in particular for amino acids, is decreased in late pregnancy. The effect of maternal overnutrition on placental transport remains largely unexplored. However

The role of the invasive, placental trophoblast in human pregnancy.

During early pregnancy the placenta-derived extravillous trophoblast starts to invade the maternal uterus in order to regulate adequate blood flow and nutrient supply to the growing fetus. A unique set of events including plugging and remodelling of maternal vessels, regulation of oxygen levels, as well as the crosstalk with maternal decidual cells are thought to be precisely controlled by the invading extravillous trophoblasts. However, defects in these processes can lead to severe complications during pregnancy threatening the well-being of both the mother and the developing fetus. For insta

Placental mesenchymal dysplasia and an estimation of the population incidence.

Placental mesenchymal dysplasia (PMD) is a rare placental malformation of as yet undetermined etiology. We report a single center's experience of this diagnosis and present an estimation of the population incidence. Within our institution, all placentae are examined within a pathology department that provides a dedicated perinatal service. In this study, we evaluated the incidence of PMD over a period of 18 years following the description and recognition of PMD as a pathological diagnosis. During the period 1991-2009, only two cases were identified amongst over 95 000 deliveries at our institu

PP118. Is the placental mass associated with the severity of preeclampsia?

INTRODUCTION: Preeclampsia contributes to an increased perinatal morbidity and mortality of both the mother and the fetus. The exact pathogenesis of PE is unclear; it is generally believed, however, that the placenta plays a pivotal role in this process. OBJECTIVES: The aim of this study was to analyse the effect of the placental mass on the severity of PE. METHODS: Following PE placentae were analyzed and their weights were compared with those of normal pregnancies [1]. Moderate and severe PE as well as HELLP syndrome were defined according to international guidelines. To compare placental

Endocrine, paracrine, and autocrine placental mediators in labor.

Considering that preterm birth accounts for about 6-10% of all births in Western countries and of more than 65% of all perinatal deaths, elucidation of the particularly complicated mechanisms of labor is essential for determination of appropriate and effective therapeutic interventions. Labor in humans results from a complex interplay of fetal and maternal factors, which act upon the uterus to trigger pathways leading gradually to a coordinated cervical ripening and myometrial contractility. Although the exact mechanism of labor still remains uncertain, several components have been identified

Drugs and medicines in pregnancy: the placental disposition of opioids.

Over the past decade therapeutic interventions during pregnancy, including the use of drugs, has increased; for example, the prescription use of opioids and nonopioid analgesics for pain management during pregnancy has increased by about 40%. Additionally, women also self-medicate during pregnancy with opioids, and are treated with methadone and buprenorphine which are used as substitution therapy for substance dependence during pregnancy. A nationally representative survey noted that ~74% of women of reproductive age reported substance use during the previous year, decreasing to ~63% amongst

Placental expression of angiogenic factors in Trisomy 13.

OBJECTIVE: Increased levels of soluble fms-like tyrosine kinase (sFlt-1) in Trisomy 13 pregnancies are thought to be mediated by the placenta. This study aimed to compare sFlt-1 expression in Trisomy 13 (n = 7) placentas with that in control placentas (Trisomy 21, n = 11, and euploid, n = 6). STUDY DESIGN: This was a retrospective case-control study analyzing paraffin-embedded placental blocks that were stained with hematoxylin and eosin and antibodies to sFlt-1. Their staining intensity was compared using a semiquantitative technique. The Kruskal-Wallis test and Wilcox rank sum test were use

Is heparin a placental anticoagulant in high-risk pregnancies?

Randomized control trials show beneficial effects of heparin in high-risk pregnancies to prevent preeclampsia and intrauterine growth restriction. However, the lack of placental pathology data in these trials challenges the assumption that heparin is a placental anticoagulant. Recent data show that placental infarction is probably associated with abnormalities in development of the placenta, characterized by poor maternal perfusion and an abnormal villous trophoblast compartment in contact with maternal blood, than with maternal thrombophilia. At-risk pregnancies may therefore be predicted by

An imbalance of COX level is not related to placental abruption.

AIMS: Muscularised basal plate arteries (MA) or chorioamnionitis (CA) are often present in placental abruption. The aim of this study was to evaluate the placental expression of COX 1 and COX 2 in cases of placental abruption with MA or CA hypothesising that an imbalance in COX placental expression might be implicated in its pathogenesis. METHODS: COX 1 and COX 2 placental immunostaining was analysed in 16 placentas with abruption (nine with MA, seven with CA), in 26 normal placentas and in 10 gestational age-matched MA or CA cases without abruption. RESULTS: COX 1 and COX 2 protein expressi

Placental transfer and pharmacokinetics of thiopentone in newborn infants.

BACKGROUND AND OBJECTIVES: Thiopentone, a short-acting barbiturate, has been introduced as premedication for intubation in newborn infants. The objectives of this study were to assess the pharmacokinetics of thiopentone in newborn infants, and to unravel whether placental transfer of the drug should be taken into account if administered to infants exposed to it during delivery. METHODS: Plasma concentrations were assessed with high-pressure liquid chromatography in samples from delivering mothers (n=27) receiving a median dose of 5.5 mg/kg (range 3.8-7.7) thiopentone for Caesarean section in

Pkd1 and Pkd2 are required for normal placental development.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is a common cause of inherited renal failure that results from mutations in PKD1 and PKD2. The disorder is characterized by focal cyst formation that involves somatic mutation of the wild type allele in a large fraction of cysts. Consistent with a two-hit mechanism, mice that are homozygous for inactivating mutations of either Pkd1 or Pkd2 develop cystic kidneys, edema and hemorrhage and typically die in midgestation. Cystic kidney disease is unlikely to be the cause of fetal loss since renal function is not required to complete

Risk of placental abruption in relation to migraines and headaches.

BACKGROUND: Migraine, a common chronic-intermittent disorder of idiopathic origin characterized by severe debilitating headaches and autonomic nervous system dysfunction, and placental abruption, the premature separation of the placenta, share many common pathophysiological characteristics. Moreover, endothelial dysfunction, platelet activation, hypercoagulation, and inflammation are common to both disorders. We assessed risk of placental abruption in relation to maternal history of migraine before and during pregnancy in Peruvian women. METHODS: Cases were 375 women with pregnancies complica

Placental abruption: epidemiology, risk factors and consequences.

Placental abruption, classically defined as a premature separation of the placenta before delivery, is one of the leading causes of vaginal bleeding in the second half of pregnancy. Approximately 0.4-1% of pregnancies are complicated by placental abruption. The prevalence is lower in the Nordic countries (0.38-0.51%) compared with the USA (0.6-1.0%). Placental abruption is also one of the most important causes of maternal morbidity and perinatal mortality. Maternal risks include obstetric hemorrhage, need for blood transfusions, emergency hysterectomy, disseminated intravascular coagulopathy a

Maternal undernutrition influences placental-fetal development.

Maternal nutrition during pregnancy has a pivotal role in the regulation of placental-fetal development and thereby affects the lifelong health and productivity of offspring. Suboptimal maternal nutrition yields low birth weight, with substantial effect on the short-term morbidity of the newborn. The placenta is the organ through which gases, nutrients, and wastes are exchanged between the maternal-fetal circulations. The size, morphology, and nutrient transfer capacity of the placenta determine the prenatal growth trajectory of the fetus to influence birth weight. Transplacental exchange depe
