Our objective was to describe the yield of actionable thoracic spine lesions for a novel magnetic resonance imaging (MRI) protocol including evaluation of the thoracic spine among patients presenting to the Emergency Department (ED) with symptoms consistent with epidural compression syndrome. Our ED and Department of Radiology together designed a novel rapid MRI protocol entailing 3D volumetric T2 weighted sequences through both the thoracic and lumbar spine obtained in the sagittal plane to assess for both lumbar and thoracic spine lesions. We recorded study outcomes for all patients undergoi

STUDY DESIGN: Retrospective review of a prospectively collected, multicenter database.
OBJECTIVE: To assess rates of new neurologic deficit (NND) associated with spine surgery.
SUMMARY OF BACKGROUND DATA: NND is a potential complication of spine surgery, but previously reported rates are often limited by small sample size and single-surgeon experiences.
METHODS: The Scoliosis Research Society morbidity and mortality database was queried for spinal surgery cases complicated by NND from 2004 to 2007, including nerve root deficit (NRD), cauda equina deficit (CED), and spinal cord deficit (SCD)

INTRODUCTION: Candida infection limited to the central nervous system is extremely rare, and may be confused with tuberculosis on the grounds of the clinical and cerebrospinal fluid findings.
CASE REPORT: A 23-year-old immunocompetent drug addict presented with alternating sciatica over a period of several months, followed by multiple cranial nerve involvement in the setting of marked weight loss. The histopathologic examination of a leptomeningeal neurosurgical biopsy was required to establish the diagnosis of neuromeningeal infection with Candida albicans.
CONCLUSION: This case report unde

STUDY DESIGN: A retrospective multicenter study of series of 20 patients with myxopapillary ependymomas, which underwent surgery. OBJECTIVE.: To evaluate the postoperative outcomes of patients surgically treated for spinal myxopapillary ependymoma. Neurologic deterioration after tumor resection is discussed in terms of the tumor encapsulation. SUMMARY OF BACKGROUND DATA.: Myxopapillary ependymomas occur most commonly in the cauda equina and/or conus medullaris region. Most series published in the literature have covered postoperative tumor recurrence and role of adjuvant radiotherapy. Few cont

PURPOSE: Remote recurrence of craniopharyngioma is an unusual postoperative complication. It occurs either along a previous surgical route or by seeding via cerebrospinal fluid (CSF).
METHODS: A 49-year-old male had been operated on twice for a suprasellar craniopharyngioma in September 1999 and February 2003. From August 2003, he noted progressive low-back pain followed by ischialgia along the right S1 dermatome, which was symptomatic of an intradural tumor at the level of the first sacral segment.
RESULTS: Total resection was performed and the tumor was histologically confirmed to be an ad

STUDY DESIGN: Measurement (validity) study.
OBJECTIVE: To develop and examine reproducibility of a criterion measure of walking capacity for use with lumbar spinal stenosis (LSS) patients, and use this criterion to examine the validity of a treadmill test for the same purpose.
SUMMARY OF BACKGROUND DATA: To date, there is no criterion measure of walking capacity advocated for use with LSS populations. Treadmill tests of walking have become more common in LSS literature and research, yet there is insufficient evidence to support the use of these tests as valid outcome measures. Therefore, our

AIMS: For electrodiagnostic assessment of patients with suspected peripheral sacral nerve lesions, sphincter muscle electromyography (EMG) and sacral reflex testing are mainly used. However, it has not been established whether a single test or both tests are necessary in individual patients. The present study aimed to clarify this dilemma.
METHODS: Fifty-two men with clinical, electrodiagnostic and radiological signs supportive of chronic cauda equina or conus medullaris lesions were included. Clinical examination, including perianal sensation and penilo-cavernosus reflex testing, and neuroph

Central nervous system (CNS) prophylaxis has led to a significant improvement in the outcome of patients with acute lymphocytic leukemia (ALL). Liposomal cytarabine (Enzon Pharmaceuticals, Piscataway, NJ; Skye Pharma, San Diego, CA), an intrathecal (IT) preparation of cytarabine with a prolonged half-life, has been shown to be safe and effective in the treatment of neoplastic meningitis. Liposomal cytarabine was given for CNS prophylaxis to 31 patients with newly diagnosed ALL. All patients were treated concurrently with hyper-CVAD chemotherapy (fractionated cyclophosphamide, vincristine, doxo

Cervical spondylotic arthRosis may lead to a myelopathy and the question which can rise is: posterior surgical decompression is adequate regarding the others surgical technics discussed? In this paper the authors describe the postoperative results of 42 patients who have been operated on by laminectomy from 1971 to 1995 at Fann Hospital in Dakar. The median age was 42 years and the median delay between the onset of neurological disorders and operation was 5.9 months. All patients showed symptoms of spinal cord and root disturbances. Neuro-imaging studies with plan X-rays and myelography reveal

Patterns of demyelination are described in several autoimmune and virus-induced demyelinating diseases of the peripheral and central nervous system. Myelin can be destroyed by injuries that affect either the myelin-supporting cells and/or the myelin lamellae. After destruction of the supporting cells, the related disintegrating sheaths are stripped off axons by invading phagocytes. Virus-induced cytolysis can occur with or without participation of immune responses, as demonstrated in subacute sclerosing panencephalitis and progressive mutlifocal leukoencephalopathy, respectively. Autoimmune de

Lymphocytes from patients with Bell's palsy were shown to undergo significant stimulation when cultured in vitro in the presence of a purely neuritogenic basic protein (P1L) isolated from human peripheral nerve myelin. No sensitization was observed to other neural antigens, namely, another periperal nerve myelin basic protein (P2) and the central nerve myelin basic encephalitogenic protein (BE). A similar pattern of response was also demonstrated in patients with Guillain-Barré syndrome (GBS). Lymphocytes from patients suffering from other neuropathies or other diseases involving the face show

BACKGROUND: Arachnoid cysts of spine are a very rare occurrence. The aetiology still remains unclear, but the most accepted explanation is the existence of areas of weakness in the spinal dura. Symptoms depend on the location in the spine. Magnetic resonance imaging is used for its diagnosis. Management depends of clinical presentation, and the surgery is reserved for patients with neurological impairment.
CLINICAL CASE: A case is described of 67 year-old male with myelopathy and radiculopathy symptoms, both diagnosed simultaneously. The magnetic resonance imaging was used to diagnose a thora

STUDY DESIGN: We analyzed retrospectively whether early surgery for cauda equina syndrome (CES) within 24, 48, or 72 hours of onset of autonomic symptoms made any difference to bladder function at initial outpatient follow-up.
OBJECTIVE: CES potentially causes loss of autonomic control including bladder dysfunction, resulting in significant disability. There is significant debate regarding appropriate timing of surgery.
SUMMARY OF BACKGROUND DATA AND METHODS: We conducted a retrospective cohort study of 200 patients between 2000 and 2011 who underwent decompressive surgery for CES at a regio

PURPOSE: Giant cell tumor of the sacrum is usually silent in initial stages and not diagnosed until achieving a large size. Intralesional curettage of the tumor has lower risk of neurological damage but is with high recurrence rate. Zoledronic acid-loaded cement was demonstrated to have cytotoxic effect on the cell line of giant cell tumor. This study evaluate if zoledronic acid-loaded bone cement would reduce the recurrence rate of sacral giant cell tumor after intralesional curettage.
METHODS: Four patients were diagnosed as sacral giant cell tumor and received intralesional curettage with

An elongated sacral lamina has been described as one of the contributing factors for dogs with cauda equina syndrome due to degenerative lumbosacral stenosis (DLSS); however, published evidence is lacking on the accuracy of radiographic screening for the presence of this lesion. Objectives of this prospective, cross-sectional cadaver study were to describe the accuracy and repeatability of detection of the cranial sacral lamina margin on plain lateral radiographs of the lumbosacral junction in dogs. Twenty-five medium and large breed canine cadavers were radiographed before and after placement

BACKGROUND: Treatment for lumbar disc disease and spinal stenosis is the most common reason patients seek neurosurgical consultation in rural sub-Saharan Africa. Yet the misperception remains that lacking access to magnetic resonance imaging/computed tomography and a "spine surgeon," neither a definitive diagnosis can be made nor corrective treatment instituted. To combat such therapeutic nihilism, the author has supervised the on-site training of general surgeons in rural Kenya for the past 15 years with the intent of making spine surgery available for patients in outlying provincial areas.

This report describes the case of a 55-year-old woman with varicella-zoster virus (VZV) sacral meningoradiculitis (Elsberg syndrome) who presented with herpes zoster in the left S2 dermatome area, urinary retention, and constipation. Lumbar magnetic resonance imaging showed the left sacral nerve root swelling with enhancement. Thereafter, she suddenly showed massive hematochezia and hemorrhagic shock because of a rectal ulcer. To elucidate the relation between Elsberg syndrome and rectal ulcer, accumulation of similar cases is necessary. To avoid severe complications, attention must be devoted
