BACKGROUND AND OBJECTIVE: To characterize the clinical features, etiology and prognosis of pontine hemorrhages (PH).
PATIENTS AND METHOD: Descriptive study of 14 patients with PH included in the Sagrat Cor Hospital of Barcelona Stroke Registry over a 19-year period. The vascular risk factors and clinical profiles in PH and subcortical hemorrhage group (n = 133) were compared.
RESULTS: PH accounted for 0.36% of all acute strokes (n = 3,808) and 3.4% of all intracerebral hemorrhages. Hypertension was present in 78.6%, diabetes mellitus in 21.4% and hyperlipidemia in 21.4%. Pure motor hemipares

Central neurogenic hyperventilation in patients with a normal level of consciousness is uncommon. This condition occurs in bilateral pontine tegmental lesions, particularly tumors such as CNS lymphomas and glioma, as well as traumatic lesions. The physiopathological mechanisms are unknown and no there is no effective treatment for this entity. We report a case of central neurogenic hyperventilation associated to a unilateral basal pontine infarction.

We report a patient with anosognosia for hemiplegia associated with a right pontine infarction. A 51-year-old woman with histories of hypertension and diabetes mellitus was admitted because of weakness of her left upper and lower extremities. On neurologic examination, she was alert and oriented without dementia. Visuospatial hemineglect was not present. Hemiparesis of her left upper and lower extremities was noted. Her brain MRIs showed a large infarction in the right pons. On admission, she could recognize her illness but was indifferent to her hemiplegia, so she said that there was not well…

Experience in three pontine hemorrhage cases which were treated by open surgery is recorded here. They were operated on by trans-fourth ventricular, subtemporal transtentorial on retromastoid suboccipital approaches. The neurological symptom improved noticeably in one case who was operated on through the retromastoid suboccipital approach seven days after the onset. The pontine hemorrhage can even be cured with open surgery which is performed in the early stage and through the operative approach.

A recent study reporting pontine lesions on magnetic resonance imaging in three patients with idiopathic narcolepsy postulated that the lesions represented tissue change secondary to long duration of disease. We studied twelve narcoleptic patients with MRI to validate these findings. The diagnosis of narcolepsy was established with both clinical and electrophysiological criteria in all twelve patients. Pontine lesions were present in two patients, both of whom were hypertensive. The lesions were associated with similar lesions in the hemispheres and were indistinguishable from ischemic change.

The essential difference between classical multiple sclerosis and both Devic's neuromyelitis optica and acute disseminated encephalomyelitis are outlined. Advances in identifying the multiple gene mutations responsible for adrenoleukodystrophy and possible mechanisms for the genotypic/phenotypic variability are reviewed.

The case of a 48 year-old woman presenting with a primary pontine hematoma is reported. The clinical evolution simulated a brain stem tumor and the diagnosis of hematoma was not established before the operation. The preoperative neurological deficit improved except for a facial nerve palsy. These hematomas are thought to be due to rupture of "cryptic" arteriovenous malformations and should be differentiated from those secondary to systemic hypertension. The diagnosis of brain stem hematomas should be considered in any case of brain stem lesion, especially in young and normotensive patients.

The clinicopathologic findings in two cases of infarct in the territory of the anterior inferior cerebellar artery are presented. A distinction is made between a minimal and a maximal syndrome. In the first case the infarct in the basis pontis correlated well with the existence of a pure motor hemiplegia. In the second case more extensive pontine infarction gave rise to a more classical brainstem syndrome. In 52 anatomical specimens the origin, course and angioarchitecture of the anterior inferior cerebellar artery were studied, showing the variability in supply area. Only the paramedial and i

A case of trigeminal neuralgia which was apparently caused by compression of the trigeminal nerve by a pontine vein is presented. Complete relief followed the coagulation and dissection of the vein.

5 patients with cerebral anoxia of various etiology had postanoxic coma with the syndrome of periodic synchronous and sterotyped myoclonus first mentioned by Lance and Adams. These authors observed action myoclonus as a sequel to this syndrome which is rarely survived. Most reports have dealt mainly with its bad prognostic significance. Although the localization of action myoclonus has much been discussed, and is still controversial, little attention has been paid to the possible localizing significance of its acute precursor syndrome. Most patients with this type of myoclonus (all in our seri
