Anosognosia as motivated unawareness: the 'defence' hypothesis revisited.

Anosognosia for hemiplegia has seen a century of almost continuous research, yet a definitive understanding of its mechanism remains elusive. Essentially, anosognosic patients hold quasi-delusional beliefs about their paralysed limbs, in spite of all the contrary evidence, repeated questioning, and logical argument. We review a range of findings suggesting that emotion and motivation play an important role in anosognosia. We conclude that anosognosia involves (amongst other things) a process of psychological defence. This conclusion stems from a wide variety of clinical and experimental invest

Proposal for a short version of the Beck Hopelessness Scale based on a national representative survey in Hungary.

In our study we assessed the frequency of reported hopelessness and suicide attempts in the national representative survey Hungarostudy 2002. The randomly selected sample consisted of 14,000 individuals over the age of 18. We created a short version of the widely used Beck Hopelessness Scale for screening purposes in suicide prevention. The short version of the BHS consists of four items and has high internal consistency (Cronbach's alpha = 0.85). Moreover, we conducted an investigation into psychological, somatic, sociological and socio-economic as well as cultural variables that show a posit

[Parkinson's disease: psychological and behavioral disorders].

Many patients with Parkinson's disease (PD) have clinically significant anxiety, depression, apathy, fatigue and sleep disturbances. These non motor symptoms of Parkinson's disease are still not well recognized in clinical practice, either in primary or in secondary care, and are frequently missed during routine examination, but often contribute to impair the functional abilities in PD patients. Awareness of behavioral and psychological symptoms in PD should generate new approaches to improve their diagnosis accuracy and facilitate timely therapeutic interventions.

Symptom Clusters Predictive of Quality of Life Among Jordanian Women with Breast Cancer.

OBJECTIVES: This study was conducted to explore symptom clusters among women with breast cancer in Jordan. DATA SOURCES: A cross-sectional survey of 516 women with breast cancer who were recruited from three hospitals. CONCLUSION: This study demonstrated that women with breast cancer experienced several symptoms at the same time. These symptoms tend to cluster in five main groups, and patients experiencing the psychological, nausea and vomiting, and pain clusters are expected to have a lower mean score of quality of life. IMPLICATIONS FOR NURSING PRACTICE: Nurses need to assess and manage s

Postconcussion Exertion Evolution: Clinical and Behavioral Considerations.

The philosophy and practices concerning concussion management have evolved from passive to active strategies that incorporate immediate, guided rest followed by early integration of physical and cognitive activity as tolerated by symptoms. Recent research and clinical evidence support guidance that symptom tolerable and clinically guided activity is beneficial postconcussion both acutely and in the longer term. Furthermore, recent studies illustrate benefits of targeted deficit-based therapies (vestibular, cervicogenic, visual, psychological, etc.) postconcussion subacutely and in those with p

The role of emotion in patient safety: Are we brave enough to scratch beneath the surface?

Healthcare professionals work in emotionally charged settings; yet, little is known about the role of emotion in ensuring safe patient care. This article presents current knowledge in this field, drawing upon psychological approaches and evidence from clinical settings. We explore the emotions that health professionals experience in relation to making a medical error and describe the impact on healthcare professionals and on their professional and patient relationships. We also explore how positive and negative emotions can contribute to clinical decision making and affect responses to clinica

Current perspectives on the psychological stress-immune axis in asthma: Pathophysiology, interventions, and significance for patient care.

Psychological stress and psychiatric comorbidities are highly prevalent in patients with asthma, with evidence supporting bidirectional links. Excessive psychological stress is associated with immune dysregulation, hypothalamic-pituitary-adrenal (HPA) axis dysfunction, and neuroimmune crosstalk that increases Type 2 inflammation associated with poorly controlled asthma. Anxiety and depression worsen asthma control and quality of life through complex and bidirectional mechanisms. Both cognitive-behavioral therapy (CBT) and mindfulness-based stress reduction (MBSR) can help reduce distress and i

Understanding psychological flourishing across the lifespan: an integrative review of theory and evidence.

Psychological flourishing is increasingly recognised as a multidimensional construct within contemporary positive psychology. It extends beyond the mere absence of psychopathology to encompass the active presence of positive functioning across emotional, cognitive, social, and existential dimensions. Despite decades of theoretical development and empirical investigation, a coherent integrative account of how flourishing unfolds, varies, and can be cultivated across the entire human lifespan remains elusive. This integrative review synthesizes theoretical frameworks and empirical evidence perta

Case Report: A complex case of neuropathy and temporomandibular disorders manifesting as otalgia.

Patients with temporomandibular disorders (TMDs) often present with otological symptoms as their primary complaint. Nervus intermedius neuropathy is a rare condition characterized by paroxysmal episodes of sharp, deep ear pain. The presence of overlapping symptoms of TMD and nervus intermedius neuropathy, associated with psychological comorbidities and sleep disturbances, poses significant diagnostic and management challenges for clinicians. Therefore, we report the case of a young female patient who presented to the orofacial pain clinic with debilitating ear pain affecting her quality of lif

Threatening Beliefs About Self and Others Moderate the Association Between Psychoticism and Psychological Distress.

Psychoticism is a multidimensional personality construct involving odd or eccentric behavior, quasi-psychotic experiences, mistrust, interpersonal detachment, and liability for schizophrenia-spectrum disorders, as well as significant distress. Recent advances suggest it can be understood as a dimension that is continuously distributed in the population, leading to questions about factors that contribute to distress and dysfunction among people with a schizotypal liability. We investigated in a large nonclinical sample of young adults whether associations between psychoticism and psychological

Care in pregnancy after stillbirth.

Pregnancy after stillbirth is associated with increased risk of stillbirth and other adverse pregnancy outcomes including fetal growth restriction, preeclampsia, and preterm birth in subsequent pregnancies. In addition, pregnancy after stillbirth is associated with emotional and psychological challenges for women and their families. This manuscript summarizes information available to guide clinicians for how to manage a pregnancy after stillbirth by appreciating the nature of the increased risk in future pregnancies, and that these are not affected by interpregnancy interval. Qualitative studi

Mediating Trauma and Anxiety: Letters to Françoise Dolto, 1976-1978.

Françoise Dolto (1908-88) was a prominent French cultural figure thanks to her practice of dispensing psychoanalytically-informed child-rearing advice via the radio. From 1976 to 1978, on her show Lorsque l'enfant paraît, she responded to thousands of letters sent in by listeners requesting help with parenting problems and personal questions of a psychological nature. The article explores Dolto's cultural position as a child psychoanalyst - understood in the 1960s and 1970s as a radical profession - but from a conservative, Catholic background. It then examines a sample of the letters sent in

Genetic Counseling in Huntington's Disease: Potential New Challenges on Horizon?

Huntington's disease (HD) is a rare, hereditary, neurodegenerative and dominantly transmitted disorder affecting about 10 out of 100,000 people in Western Countries. The genetic cause is a CAG repeat expansion in the huntingtin gene (HTT), which is unstable and may further increase its length in subsequent generations, so called anticipation. Mutation repeat length coupled with other gene modifiers and environmental factors contribute to the age at onset in the offspring. Considering the unpredictability of age at onset and of clinical prognosis in HD, the accurate interpretation, a proper psy

Diagnosis and management of pancreatic cancer in adults: A summary of guidelines from the UK National Institute for Health and Care Excellence.

To enable standardisation of care of pancreatic cancer patients and facilitate improvement in outcome, the United Kingdom's National Institute for Health and Care Excellence (NICE) developed a clinical guideline for the diagnosis and management of pancreatic cancer in adults. Systematic literature searches, systematic review and meta-analyses were undertaken. Recommendations were drafted on the basis of the group's interpretation of the best available evidence of clinical and cost effectiveness. There was patient involvement and public consultation. Recommendations were made on: diagnosis; sta

Identification, assessment and management of breast oedema after treatment for cancer.

Advances in breast cancer management, including breast-conserving surgery, have resulted in an increase in the incidence of breast oedema. However, this is addressed by professionals less often than arm lymphoedema. Breast oedema results in significant physical and psychological morbidity, makes finding a comfortably fitting bra very difficult, and can act as a reminder of the initial breast cancer diagnosis. Psychological difficulties can be compounded by clinicians' failure to acknowledge the impact. Assessing and managing breast oedema requires a sensitive, skilled approach to ensure all th

"When my Moods Drive Upward There Is Nothing I Can Do about It": A Review of Extreme Appraisals of Internal States and the Bipolar Spectrum.

The integrative cognitive model provides a comprehensive account of bipolar disorder (BD) that, if empirically supported, has important potential implications for psychological therapies. This article is the first to review the evidence for this model. We evaluate the evidence (up to 2017) for four hypotheses derived uniquely from the model: extreme positive and negative appraisals of internal states are associated with (1) risk of developing BD; (2) BD diagnosis; (3) relevant clinical and functional outcomes including hypomanic and depressive mood symptoms; and (4) outcomes over time. Reductions in Goal-Directed Cognition as a Consequence of Being the Target of Empathy.

Although empathy is widely promoted as a beneficial practice across both intergroup and interpersonal contexts, the implications of being the target of empathy for the target's own psychological state are unclear. Three experiments examined how being the target of empathy affects goal-directed cognition outcomes related to a psychological sense of power, namely, the ability to maintain goal focus and readiness to ask for more in negotiations. We reasoned that because individuals typically empathize with others they perceive as disadvantaged and needing support, trying to empathize would raise

Psychosocial factors and their role in symptomatic gastroesophageal reflux disease and functional dyspepsia.

Patients with symptoms of GERD and dyspepsia are among the most common consulters in general practice and are different from their counterparts in the community who choose not to consult although they suffer from similar symptoms. They represent a heterogeneous group with considerable symptom overlap. They have a relatively poor quality of life and endoscopic findings can only explain symptoms in about half of these patients. Thus psychosocial factors which could contribute to their morbidity should be explored. While some studies have methodological shortcomings, main findings are that key ps

On the historical and conceptual background of the Wisconsin Card Sorting Test.

In this paper, we describe the development of the Wisconsin Card Sorting Test (WCST). We trace the history of sorting tasks from the studies of Narziss Ach on the psychology of thinking, via the work of Kurt Goldstein and Adhémar Gelb on brain lesioned patients around 1920 and subsequent developments, up to the actual design of the WCST by Harry Harlow, David Grant, and their student Esther Berg. The WCST thus seems to originate from the psychology of thinking ('Denkpsychologie'), but the test, as it is used in clinical neuropsychological practice, was designed by experimenters working within
