AIMS: Adequate sedation and analgesia are essential for catheter ablation of atrial fibrillation (AF) to ensure patient comfort and procedural safety. However, the availability of cardiac anaesthesiology is limited. This study evaluated the efficacy and safety of an operator-directed, nurse-administered (ODNA) sedation protocol using propofol and remifentanil during AF ablation.
METHODS AND RESULTS: In this prospective, single-centre observational cohort study, consecutive patients undergoing catheter ablation for AF from August 2023 to March 2024 were enrolled. Sedation-related adverse event

The fascia iliaca compartment is a potential interfascial space deep to the fascia iliaca and superficial to the iliacus and psoas muscles. Compared with infrainguinal techniques, the suprainguinal approach is anatomically favorable for cephalad spread beneath the fascia iliaca. Although this approach is well established in perioperative analgesia, detailed procedural descriptions adapted for ultrasound-guided fascial hydrodissection and interventional pain practice remain limited. This technical report describes an anatomy-based, ultrasound-guided suprainguinal fascia iliaca plane injection u

This first-in-man (FIM) study evaluated the feasibility and safety of a new peripheral plaque atherectomy system in patients with symptomatic lower extremity artery disease (LEAD). Ten patients with symptomatic LEAD (Rutherford class 2-5) were enrolled in a prospective, single-center study from March to April 2024. Patients aged 18-85 years with target lesions showing ≥70% stenosis and reference vessel diameters ≥1.8 mm underwent treatment using a novel atherectomy device with a "shaving" mechanism to excise calcified plaques. Outcomes included post-procedural stenosis improvement, 6-month pri

A review was conducted to assess the efficacy and safety of the intra-articular haematoma block (IAHB) for manipulation of ankle fractures in ED. Any study investigating the success of IAHB for ankle fracture reduction published in English was sought. Seven databases were searched. The Cochrane Risk of Bias tool was used to quality assess the included studies. Three studies met the inclusion criteria (n = 436 patients). Just one study was a randomised controlled trial (n = 42). The two non-randomised studies that included the majority of patients were assessed as at high overall risk of bias.

INTRODUCTION: Ureteropelvic junction obstruction (UPJO) is a condition characterized by partial or complete obstruction of urine transport from the renal pelvis to the ureter and can present with intermittent flank pain, recurrent urinary tract infections, renal stones, or renal dysfunction. While historically, open pyeloplasty was the gold standard for surgical management, laparoscopic methods to repair UPJO have largely taken over as the preferred approach for adolescent and adult patients. Despite near universal adoption of laparoscopic pyeloplasty among Canadian urologists, it remains a te

OBJECT: Tethered cord syndrome (TCS) encompasses a spectrum of neurological dysfunction related to excessive tension on the distal spinal cord resulting in anatomic deformation and metabolic disturbance. Symptoms typically manifest as back/leg pain, neurogenic bladder dysfunction, constipation, sphincter abnormalities, and scoliosis. To date, among the least well-described symptoms of TCS is pain or hypersensitivity in the perineal region. The authors reviewed their experience with spinal cord detethering to identify and further characterize those who present with perineal pain or hypersensiti

The quality of postoperative pain therapy in Germany shows a heterogeneous treatment practice and large differences in quality between individual institutions, The patient representatives in the Federal Joint Committee (G-BA) have therefore decisively campaigned for many years that instruments of non-legislative standards are employed in order to noticeably improve the quality of perioperative pain therapy for patients in Germany. As a result of these efforts, in October 2020 a binding specification for internal quality management was included in the quality management guidelines (QM-RL) by th

BACKGROUND: Local anesthesia is essential in common procedures in dermatology, but injection and infiltration of anesthetic often cause significant pain, reducing patient comfort and satisfaction.
OBJECTIVE: To review evidence-based techniques to reduce pain associated with local anesthetic injection and infiltration.
METHODS AND MATERIALS: A narrative review of the literature was performed, and techniques were summarized for clinicians to reference.
RESULTS: Technical methods to reduce pain include using smaller-gauge needles (30-33G), smaller syringes (1‒3 mL), buffering lidocaine with so

The last several decades have seen a marked increase in both the recognition and treatment of chronic pain. Unfortunately, patients frequently misunderstand both the nature of pain and the best practices for its treatment. Because primary care physicians treat the majority of chronic pain, they are ideally situated to provide evidence-based pain care. The majority of the medical evidence supports a biopsychosocial model of pain that integrates physical, emotional, social, and cultural variables. The goal of this primer is to assist primary care physicians in their understanding of pain, evalua

BACKGROUND: Patients with connective tissue disorders (CTDs), such as Ehlers-Danlos syndrome, can present unique challenges in the management of spinal pathology and procedural complications due to underlying tissue fragility and susceptibility to dural ruptures. Thus, there is a need for less invasive diagnostic and therapeutic care in this population.
CASE REPORT: We present a case of a 48-year-old woman with joint hypermobility who developed acute-on-chronic back pain and radicular symptoms. Imaging revealed an L5-S1 radiculopathy and a thoracic arachnoid cyst. During inpatient rehabilitat

This case report describes a 32-year-old male who underwent ketamine procedural sedation and experienced a generalized tonic-clonic seizure. Despite its rapid onset and favorable tolerance profile, this case emphasizes the potential convulsive side effects of ketamine, which is commonly used for procedural sedation. While ketamine has shown promise in treating acute pain, refractory status epilepticus, and treatment-resistant depression and schizophrenia, it is associated with side effects such as hallucinations, visual disturbances, dizziness, nausea, and vomiting. The patient in this case re

Uterine fibroid embolization (UFE) procedures performed from 2013 to 2019 were reviewed. Seventy-two patients were treated with a standard protocol consisting of sedation, ketorolac, ondansetron, and overnight parenteral analgesics and antiemetics. Ninety-six patients were treated with a new protocol, which added transdermal scopolamine, lorazepam, and intravenous acetaminophen. Outpatient uterine fibroid embolization (OP-UFE) not requiring hospitalization was successful in 81.4% and 2.7% of patients treated with the new and old protocols, respectively (odds ratio [OR], 141.4; P < .0001). Catheter ablation for atrial fibrillation is associated with a low rate of complications, with few reports of intracranial hemorrhage in the literature. Additionally, subarachnoid hemorrhage with spinal hematoma is also an uncommon pathology with less than 200 total cases to date described. A 78-year-old female with hypertension and atrial fibrillation on warfarin presented for catheter cryoablation and experienced periprocedural hypertension. Subsequently, post-procedure, the patient developed severe back pain, nausea, vomiting, altered consciousness, and paraplegia. Imaging demonstrated suba

INTRODUCTION: Subungual pyogenic granuloma (PG) can be caused by numerous medications including chemotherapeutic agents. These lesions grow rapidly, oftentimes causing significant pain and bleeding, prompting patients to seek treatment. The management of subungual PG ranges from topical steroids to surgical excision. However, patients with chemotherapy-induced PGs are prone to developing multiple or recurrent lesions. Therefore, finding a therapeutic option that effectively eliminates the tumor and prevents the need for repeated procedural interventions is important. The use of topical β-adren

BACKGROUND AND AIMS: Liver biopsy is the gold standard for evaluating liver diseases, the diagnosis of liver fibrosis or liver cirrhosis and malignancy. However, it is susceptible to complications, and safety data on liver biopsies remain scarce. The following study examined the complication rates following percutaneous liver biopsies.
METHODS: We performed a study using data collected by the German interventional radiology society (DeGIR) from 2018 to 2021 of elective percutaneous liver biopsies. Clinical and hematological parameters, technical features and adverse events were retrospectivel

BACKGROUND: Assessing pain in mechanically ventilated infants is challenging. The assessment of skin conductance (SC) is based on the sympathetic nervous system response to stress. This study purpose was to evaluate the validity of SC for assessing pain in mechanically ventilated infants.
METHODS: A prospective cross-sectional observational design was used to study SC and its relation to: the category of procedure (i.e., painful or non-painful); the phase of procedure (i.e., before, during and after), and referent pain measurements (i.e., Premature Infant Pain Profile-Revised (PIPP-R) and Neo

OBJECTIVES: Pan-retinal photocoagulation is a common outpatient procedure for diabetic retinopathy that often causes procedural pain and anxiety. Nonpharmacologic interventions such as music therapy may provide relief but are underexplored in this population. This study aimed to evaluate the effectiveness of a three-phase music therapy intervention in reducing pain, anxiety, and physiological responses during pan-retinal photocoagulation, and to explore its potential as a supportive outpatient strategy.
METHODS: Quasi-experimental study.
