Minimum Intervention Oral Care (MIOC) - Overcoming Implementation Barriers: An International Expert Consensus.

OBJECTIVES: The minimum intervention oral health care (MIOC) delivery framework shifts restorative dentistry from a predominantly operative, procedure-centred model towards a person-focused, prevention-oriented and tooth-preserving approach. This international consensus aimed to develop agreed, evidence-informed recommendations to support uptake of MIOC. METHODS: A structured expert consensus process was conducted. Sixteen international experts participated in a structured in-person meeting and consented on fourteen recommendations, all with 100% agreement. RESULTS: Implementation of MIOC re

Health coaching to encourage obese adults to enroll in commercially-available weight management programs: The path to health study.

Physicians are recommended to screen and refer obese patients to weight management programs (WMPs). There are often limited referral options for physicians, though commercially-available WMPs could be a potential solution. The purpose of this study (Path to Health) was to evaluate the efficacy of health coaching to promote enrollment in commercially-available WMPs through a two-arm, RCT with obese patients (n = 168) randomly assigned to intervention (n = 84) or control groups (n = 84). Intervention participants received phone health coaching to help them select and enroll in WMPs. We collected

Motivational interviewing as a component of a school-based obesity prevention program for adolescent girls.

Over the past decade, motivational interviewing has been used by health professionals to promote health behavior changes and help individuals increase their motivation or "readiness" to change. This paper describes a preliminary study that evaluated the feasibility of motivational interviewing as a component of New Moves, a school-based obesity prevention program. New Moves is a program for inactive adolescent high school girls who are overweight or at risk of becoming overweight due to low levels of physical activity. Throughout the 18-week pilot study, 41 girls, age 16 to 18 years, participa

The effectiveness of "Exercise on Prescription" in stimulating physical activity among women in ethnic minority groups in the Netherlands: protocol for a randomized controlled trial.

BACKGROUND: Lack of physical activity is an important risk factor for overweight, diabetes, cardiovascular disease and other chronic conditions. In the Netherlands, ethnic minority groups are generally less physically active and rate their own health poorer compared to ethnic Dutch. This applies in particular to women. For this reason women from ethnic minority groups are an important target group for interventions to promote physical activity. In the Netherlands, an exercise referral program ("Exercise on Prescription") seems successful in reaching women from ethnic minority groups, in particu

Improving Health Care Proxy Documentation for Patients With Cognitive Impairment in an Acute Care Setting: A Quality Improvement Project.

PURPOSE: Advanced care planning supports discussions about patients' preferences for end-of-life care, frequently resulting in the completion of health care proxy documentation (HCP). This quality improvement project aimed to increase HCP documentation rates for hospitalized patients with cognitive impairment, including those diagnosed with dementia and delirium. PRIMARY PRACTICE SETTING: This project was implemented at a 216-bed acute care, community-based teaching hospital in the northeastern United States. METHODOLOGY AND SAMPLE: The intervention targeted case managers and nursing staff i

Models of smoking cessation brief interventions in oral health.

The links between tobacco smoking, and periodontal disease and oral cancer make the inclusion of smoking cessation interventions at dental visits an important prevention strategy in oral health services. The 5As (Ask, Advise, Assess, Assist, Arrange), which utilises a stages of change model, is the most commonly recognised framework for the provision of smoking cessation brief interventions and is advocated widely. While the popularity of the 5As continues, increasingly evidence suggests that staged-based interventions in smoking cessation may not be the best approach. Lack of time and experti

Decentralising tuberculosis services from county tuberculosis dispensaries to township hospitals in China: an intervention study.

SETTING: County tuberculosis (TB) dispensaries are the basic health care unit for the provision of TB services in China. In the rural hills of the western provinces of China, however, county centres are often far from patients' homes. OBJECTIVE: To evaluate whether decentralisation would result in improvements in patient access to TB services while maintaining the quality of the DOTS strategy. METHODS: The present study compared TB services that were decentralised to township hospitals with two comparable control groups where township hospitals continued to routinely refer suspects to the co

Assessing the quality of care in sick child services at health facilities in Ethiopia.

BACKGROUND: Quality of care depends on system, facility, provider, and client-level factors. We aimed at examining structural and process quality of services for sick children and its association with client satisfaction at health facilities in Ethiopia. METHODS: Data from the Ethiopia Service Provision Assessment Plus (SPA+) survey 2014 were used. Measures of quality were assessed based on the Donabedian framework: structure, process, and outcome. A total of 1908 mothers or caretakers were interviewed and their child consultations were observed. Principal component analysis was used to const

Association between transitional care factors and hospital readmission after transcatheter aortic valve replacement: a retrospective observational cohort study.

BACKGROUND: Studies have shown that patients who undergo trans-catheter aortic valve replacement (TAVR) have high rates of hospital readmission. Our objectives were to identify the causes of readmission after TAVR, determine whether transitional care factors were associated with a reduction in readmission and to identify other predictors that could be used to target quality improvement efforts. METHODS: We conducted a chart abstraction study that included all patients who underwent TAVR in Ontario, Canada between 2007 and 2013 and survived to hospital discharge. These data were linked to prov

Reducing Readmissions After Stroke With a Structured Nurse Practitioner/Registered Nurse Transitional Stroke Program.

BACKGROUND AND PURPOSE: Our aim was to determine whether a standardized Transitional Stroke Clinic (TSC) led by nurse practitioners could reduce 30-day and 90-day readmissions for stroke or transient ischemic attack patients discharged home. METHODS: Phase I consisted of nurse practitioners calling only high-risk patients discharged home within 7 days and performing an office visit within 2 to 4 weeks of discharge. Phase II consisted of all patients discharged home receiving both a 2-day follow-up phone call by a registered nurse and a follow-up visit with a nurse practitioner within 7 to 14

Association of Outpatient Practice-Level Socioeconomic Disadvantage With Quality of Care and Outcomes Among Older Adults With Coronary Artery Disease: Implications for Value-Based Payment.

BACKGROUND: Medicare patients with coronary artery disease (CAD) have been a significant focus of value-based payment programs for outpatient practices. Physicians and policymakers, however, have voiced concern that value-based payment programs may penalize practices that serve vulnerable populations. This study evaluated whether outpatient practices that serve socioeconomically disadvantaged populations have worse CAD outcomes, and if this reflects the delivery of lower-quality care or rather, patient and community factors beyond the care provided by physician practices. METHODS AND RESULTS:

Emergency peripartal hysterectomy - a single-center analysis of the last 13 years at a tertiary perinatal care unit.

Background Peripartal hysterectomy (PH) is a challenging surgical procedure with elevated maternal morbidity. Methods From 2004 to 2016, 41 emergency PHs were performed at the tertiary care center of the Department of Gynecology and Obstetrics at University Hospital Ulm. In our retrospective analysis, the incidence of PH in our hospital was 12.8 per 10,000 deliveries with a maternal mortality of 2.4%. PH followed in 80.5% after cesarean section (c-section). Underlying causes/indications for PH were abnormal placentation (53.7%; n=22), uterine atony (26.8%; n=11), uterine lacerations (14.6%; n=

Building an electronic health record integrated quality of life outcomes registry for spine surgery.

OBJECTIVE: Demonstrating the value of spine care requires adequate outcomes assessment. Long-term outcomes are best measured as overall improvement in quality of life (QOL) after surgical intervention. Present registries often require parallel data entry, introducing inefficiencies and limiting compliance. The authors detail the methodology of constructing an integrated electronic health record (EHR) system to collect QOL metrics and demonstrate the effect of data collection on routine clinical workflow. A streamlined approach to collecting QOL data can capture patient data without requiring d

Contracting out to improve the use of clinical health services and health outcomes in low- and middle-income countries.

BACKGROUND: Contracting out of governmental health services is a financing strategy that governs the way in which public sector funds are used to have services delivered by non-governmental health service providers (NGPs). It represents a contract between the government and an NGP, detailing the mechanisms and conditions by which the latter should provide health care on behalf of the government. Contracting out is intended to improve the delivery and use of healthcare services. This Review updates a Cochrane Review first published in 2009. OBJECTIVES: To assess effects of contracting out gove

The influence of context on quality improvement success in health care: a systematic review of the literature.

CONTEXT: The mixed results of success among QI initiatives may be due to differences in the context of these initiatives. METHODS: The business and health care literature was systematically reviewed to identify contextual factors that might influence QI success; to categorize, summarize, and synthesize these factors; and to understand the current stage of development of this research field. FINDINGS: Forty-seven articles were included in the final review. Consistent with current theories of implementation and organization change, leadership from top management, organizational culture, data i

Assessing social recovery of vulnerable youth in global mental health settings: a pilot study of clinical research tools in Malaysia.

BACKGROUND: A social recovery approach to youth mental health focuses on increasing the time spent in valuable and meaningful structured activities, with a view to preventing enduring mental health problems and social disability. In Malaysia, access to mental health care is particularly limited and little research has focused on identifying young people at risk of serious socially disabling mental health problems such as psychosis. We provide preliminary evidence for the feasibility and acceptability of core social recovery assessment tools in a Malaysian context, comparing the experiential pr
