Two homosexual men presented with primary malignant lymphomas of the anorectum. Each patient presented with a painful mass, diagnosed by biopsy and immunologic cell marker studies. One patient, who had a reversed T4/T8 cell ratio, developed a local relapse of the tumor 11 months after radiotherapy, and Pneumocystis carinii pneumonia. Strongyloidiasis is a tenacious soil-transmitted nematode infestation endemic in the south-eastern United States. Thirty-three cases were diagnosed in a series of 1,290 stool examinations in 971 patients at Veterans Administration Medical Center, Mountain Home, Tennessee. Fifteen patients with advanced gynecologic malignancies were treated with high-energy proton beam radiation therapy (RT) at the Particle Radiation Medical Science Center (PARMS), Tsukuba University, Japan, from 1983 to 1987. Anorectal abscesses and fistulas are common maladies that are usually readily diagnosed and effectively treated. However, complex fistulous disease challenges even the most expert of surgical specialists. The management options in this subset of patients are suboptimal, with treatment often requiring multiple procedures, putting the patient at risk for continued symptoms and fecal incontinence. In this chapter, the clinical aspects of perianal suppurative disease are discussed and an attempt is made to deal with many of the more challenging aspects of patient management.

BACKGROUND & AIMS: In 2003, a sharp increase in cases of lymphogranuloma venereum (LGV) caused by Chlamydia trachomatis in men who have sex with men was reported in Europe and, recently, cases of LGV have been reported in North America.

There are two forms of anal fistulas arising from its pathogenesis: the acute stage is the abscess, whereas the chronic stage is the fistula in ano. The classification of the fistula in ano is named after Parks. Pathogenesis and classification are explained. For complete cure, every abscess needs precise examination to be able to show the course and shape of the fistula. The surgical procedure depends on the fistula tract. Most fistulas can be operated by means of a fistulotomy or fistulectomy. Recovery depends on locating the total fistula tract.

A 63-year-old, Dutch, HIV-seronegative man presented with anal pain and itch of 6 months' duration, a perianal ulcer and a solitary colon ulcer. Crohn's disease was suspected; the patient was treated with corticosteroids, but later died. Autopsy revealed disseminated histoplasmosis, a fungal disease rare in the Netherlands. The patient had visited Honduras. This case report illustrates that disseminated histoplasmosis may mimic Crohn's disease leading to a delay in the diagnosis.
