The therapy relationship with lesbian and gay clients.

This study investigated the role of therapy practices and the therapy relationship on lesbian and gay clients' feelings about their current therapist. Participants were 76 lesbian and 40 gay male clients ranging in age from 19 to 69 years. The real relationship was found to predict an additional 8% of variance in clients' positive feelings about their therapist above and beyond months in therapy, therapy practices, and the working alliance. However, therapy practices did not add significance in predicting lesbian and gay clients' feelings about their therapist beyond the working alliance and t

Reclaiming the patient's voice and spirit in dying: an insight from Israel.

In the latter half of the 20th century, Western medicine moved death from the home to the hospital. As a result, the process of dying seems to have lost its spiritual dimension, and become a matter of prolonging material life by means of medical technology. The novel quandaries that arose led in turn to medico-legal regulation. This paper describes the recent regulation of dying in Israel under its Dying Patient Law, 2005. The Law recognizes advance directives in principle, but limits their effect and form through complex medico-legal artifices. It reflects a culture that places high value on

Evaluation of the dissemination of active support training in staffed community residences.

Our aim was to replicate Active Support, a staff training intervention designed to increase the assistance given to adults with severe mental retardation living in community residences in order to increase their participation in activities. Training was conducted in 38 residences, involving 303 staff members and 106 persons with mental retardation. Active Support resulted in significant increases in assistance and engagement in activity. Significant correlations between participant abilities, receipt of assistance, and levels of engagement were found. Active Support was found to be of greater

The temporal relation of adherence and alliance to symptom change in cognitive therapy for depression.

This study attempted to replicate an earlier study (R. J. DeRubeis & M. Feeley, 1990) of the prediction of symptom change from process variables in cognitive therapy for depressed outpatients. Measures of in-session therapist behavior and therapist-patient interactions were correlated with prior and subsequent symptom change. One of the positive findings was confirmed, but the other received only marginal support. A "concrete" subset of theory-specified therapist actions, measured early in treatment, predicted subsequent change in depression. The therapeutic alliance was predicted by prior sym

Sequences of staff-child interactions on a psychiatric inpatient unit.

Six psychiatry inpatients were observed during mealtimes to determine and evaluate staff intervention techniques. To extend and further elaborate the findings of a previous work (Pines, Kupst, Natta, & Schulman, 1985), staff behaviors (positive, punitive, isolating, and neutral) were investigated for their potential relationship to subsequent child behaviors (positive, negative, and inactive) via a lag sequential analytic approach. Staff punitive and isolating behaviors tended to be associated with significant increases in the likelihood of subsequent child negative behaviors and with signific

Encounters between medical and lay knowledge in therapeutic patient education. A qualitative study based on an oral chemotherapy program.

OBJECTIVES: The growing number of cancer patients treated with Oral Chemotherapy (OC) at home, is prompting many healthcare centers to develop Therapeutic Patient Education (TPE) programs. This study aimed to 1) describe the different forms of knowledge shared and learned in these programs, and 2) better understand how self-care and psychosocial skills are promoted in the TPE context. METHOD: This study used qualitative data from the French "ONCORAL" program. Data collection was conducted with non-participant observations. The corpus comprised 42 TPE sessions. RESULTS: Analysis highlighted t

The consultative process used in outreach: a narrative account.

AIM: The aim of this paper was to provide a narrative account of the communication skills used in an effective outreach consultation utilizing Neighbour's consultative model. Other consultation models were considered; however, because of their overly comprehensive approach or emphasis on behaviour modification, these were deemed inappropriate. BACKGROUND: The nursing profession has endured significant changes of late and as a result is developing more autonomous roles in both the community and the acute health care settings. In the past, the term consultancy was used within the medical contex

The Role of Health Extension Workers in Linking Pregnant Women With Health Facilities for Delivery in Rural and Pastoralist Areas of Ethiopia.

BACKGROUND: Women's preference to give birth at home is deeply embedded in Ethiopian culture. Many women only go to health facilities if they have complications during birth. Health Extension Workers (HEWs) have been deployed to improve the utilization of maternal health services by bridging the gap between communities and health facilities. This study examined the barriers and facilitators for HEWs as they refer women to mid-level health facilities for birth. METHODS: A qualitative study was conducted in three regions: Afar Region, Southern Nations Nationalities and People's Region and Tigra

Medical 3-D Printing Within a Health Care Facility.

PURPOSE: Within the widespread use of medical 3-D printing at health care facilities (HCFs), the scope and complexity of patient-specific anatomic models and anatomic (surgical) guides are expanding. The authors formed a multidisciplinary panel to seek consensus regarding best practices for 3-D printing at a HCF. METHODS: At the request of the ACR, a 14-member panel was formed with members nominated by 10 professional societies. This multidisciplinary team represented those involved in health care 3-D printing. The panel made statements and recommendations in five areas: (1) general statement

Strategies for healthcare professionals to facilitate patient illness understanding.

AIMS AND OBJECTIVES: To describe how healthcare professionals facilitate patient illness understanding. BACKGROUND: Healthcare professionals and patients differ in their illness understanding. If the information provided by healthcare professionals is not adapted to the patient's daily life, it may be unusable for the patient. Previous research has found that healthcare professionals should individualise the information to enable the patient to apply the knowledge to the personal situation and to develop illness understanding. However, little is known of how healthcare professionals can facil

Living and dying: responsibility for end-of-life care in care homes without on-site nursing provision - a prospective study.

The aim of the study was to describe the expectations and experiences of end-of-life care of older people resident in care homes, and how care home staff and the healthcare practitioners who visited the care home interpreted their role. A mixed-method design was used. The everyday experience of 121 residents from six care homes in the East of England were tracked; 63 residents, 30 care home staff with assorted roles and 19 National Health Service staff from different disciplines were interviewed. The review of care home notes demonstrated that residents had a wide range of healthcare problems.

The effect of a patient portal with electronic messaging on patient activation among chronically ill patients: controlled before-and-after study.

BACKGROUND: It has been suggested that providing patients with access to their medical records and secure messaging with health care professionals improves health outcomes in chronic care by encouraging and activating patients to manage their own condition. OBJECTIVES: The aim was to evaluate the effect of access to a patient portal on patient activation among chronically ill patients. Further, the relationship between temporal proximity of a severe diagnosis and patient activation were assessed. METHODS: A total of 876 chronically ill patients from public primary care were allocated to eith

Exploring patients' and family members' perspectives on intensive care unit experiences through unsolicited feedback: A qualitative content analysis.

BACKGROUND: Intensive care unit (ICU) stays are profound and distressing experiences for critically ill patients and their family members, often resulting in physical, psychological, cognitive, and social sequelae. Existing knowledge stems largely from structured interviews or questionnaires addressing predefined questions, potentially overlooking how individuals spontaneously and autonomously choose to express their experiences without being prompted. OBJECTIVE: This study was conducted to explore the content of unsolicited feedback from former critically ill patients and family members, off
