Acromegaly with empty sella syndrome.

SUMMARY: Acromegaly is a rare, chronic progressive disorder with characteristic clinical features caused by persistent hypersecretion of gro … ases. We describe a 47-year-old male who presented with overt features of acromegaly (macroglossia, prognathism, increased hand and feet size). Biochemically, both the serum GH (21.6 μg/L) and insulin-like growth factor 1 (635 μg/L) were elevated. In addition, there was a paradoxical elevation of GH following a 75 g oral glucose load. Pituitary MRI demonstrat …

Assessment of bone healing after Le Fort I osteotomy with 3-dimensional computed tomography.

PURPOSE: The purpose of this study was to examine bone healing after Le Fort I osteotomy in Class III patients. PATIENTS AND METHODS: The study group consisted of 18 Japanese patients with mandibular prognathism with and without asymmetry, maxillary retrognathism or open bite. A total of 36 sides were examined. Le Fort I osteotomy was performed without a pterygoid osteotome, with an ultrasonic curette used to remove interference at the pterygomaxillary region. Titanium plates (Universal Mid-face fixation module, Stryker, Freiburg, German) were used for four patients, absorbable plates (poly-L…

The Usage of Reversed Köle-Type Mandibular Anterior Segmental Subapical Osteotomy: Radiological, Clinical, and Surgical Considerations.

The currently common procedures in orthognathic surgery are widely used. Different forms of skeletal malocclusion can be treated with great … Significant therapeutic complexity emerges when severe skeletal Class III malocclusion (mandibular prognathism) is compounded by transverse maxillary discrepancies and a markedly elevated mandibular anterior segment. This constellation of deformities leads to profound disturbances in the anterior facial height, a disrupted maxillomandibular interincisal rela …

Craniofacial and dental phenotype of two girls with osteogenesis imperfecta due to mutations in CRTAP.

Mutations in CRTAP lead to an extremely rare form of recessive osteogenesis imperfecta (OI). CRTAP deficient mice have a brachycephalic skull, fusion of facial bones, midface retrusion and class III dental malocclusion, but in humans, the craniofacial and dental phenotype has not been reported in detail. Here, we describe craniofacial and dental findings in two 11-year-old girls with biallelic CRTAP mutations. Patient 1 has a homozygous c.472-1021C>G variant in CRTAP intron 1 and a moderately severe OI phenotype. The variant is known to create a cryptic splice site, leading to a frameshift and

Craniofacial morphology of Homo floresiensis: description, taxonomic affinities, and evolutionary implication.

This paper describes in detail the external morphology of LB1/1, the nearly complete and only known cranium of Homo floresiensis. Comparisons were made with a large sample of early groups of the genus Homo to assess primitive, derived, and unique craniofacial traits of LB1 and discuss its evolution. Principal cranial shape differences between H. floresiensis and Homo sapiens are also explored metrically. The LB1 specimen exhibits a marked reductive trend in its facial skeleton, which is comparable to the H. sapiens condition and is probably associated with reduced masticatory stresses. However

Influence of orthognathic surgery on voice fundamental frequency.

Considering that orthognathic surgery promotes changes in orofacial structures constituting the resonating system, functional changes secondary to surgery are expected to affect speech, leading to the need for further speech and voice adjustments. Thus, understanding the possible relationships of these structures with voice production is important. Therefore, this study aimed to describe the changes in voice fundamental frequency of a patient submitted to orthognathic surgery and observe if there is a relationship with hyoid bone positioning at the different treatment periods. The results reve

Clinical treatment of oral manifestations of Beckwith-Wiedeman syndrome in a child.

Orthodontic treatment in a young patient with Beckwith-Wiedeman Syndrome (BWS) is reported and a multidisciplinary approach to the management of this syndrome is reviewed. The patient presented with a tendency to Class III malocclusion, an open-bite and a slight macroglossia, which was treated at an early age by glossotomy. It was decided to monitor growth without treatment and to wait for the best time to begin therapy. It was based only on functional rehabilitation, without any fixed appliances, in which optimum intercuspation of the teeth and the skeletal Class I relationship was achieved a

Relationship of the mandibular canal to the lateral cortex of the mandibular ramus as a factor in the development of neurosensory disturbance after bilateral sagittal split osteotomy.

PURPOSE: This study evaluated the location of the mandibular canal in the ramus of the mandible before bilateral sagittal split ramus osteotomy and examined its relationship with postoperative neurosensory disturbance. PATIENTS AND METHODS: The subjects consisted of 20 patients undergoing bilateral sagittal split ramus osteotomy. Before surgery, the region from a plane containing the lowest point of the mandibular foramen to 22 mm below it was observed on transaxial computed tomograms acquired with a slice thickness of 2 mm and a slice interval of 2 mm. The relationship between the distance f

Features in subjects with the frontal occlusal plane inclined toward the contralateral side of the mandibular deviation.

INTRODUCTION: The frontal occlusal plane of the maxilla generally inclines toward the ipsilateral side of the mandibular deviation in subjects with facial asymmetry; however, a few patients with facial asymmetry have their frontal occlusal planes inclined toward the contralateral side. We aimed to investigate the morphologic and functional features of such patients. METHODS: The subjects were 40 patients with facial asymmetry divided into 2 groups based on the inclination of the frontal occlusal plane toward the ipsilateral or the contralateral side. We analyzed lateral and posteroanterior ce

Influence of mandibular fixation method on stability of the maxillary occlusal plane after occlusal plane alteration.

In this study, we investigated how method of mandibular fixation influenced longterm postoperative stability of the maxilla in Class III cases. In particular, we investigated change in the maxillary occlusal plane after Occlusal Plane Alteration. Therefore, we focused on change in the palatal plane to evaluate stability of the maxillary occlusal plane, as the position of the palatal plane affects the maxillary occlusal plane. This study included 16 patients diagnosed with mandibular protrusion. Alteration of the occlusal plane was achieved by clockwise rotation of the maxilla by Le Fort I oste

The need for intermaxillary fixation in sagittal split osteotomy setbacks with bicortical screw fixation.

OBJECTIVE: The aim of this study was to decide whether use of bicortical screw fixation provides sufficient stability to dispense with intermaxillary fixation (IMF). STUDY DESIGN: Eighty-six patients who had undergone surgical setback of the mandible using bilateral sagittal split ramus osteotomies were examined. Group 1 (15 patients) received miniplate fixation with IMF for 6 weeks, and group 2 (71 patients) received bicortical screw fixation and immediate postoperative function. The 2 groups were evaluated radiographically for postsurgical changes of the pogonion 24 months postoperatively.

Modified intraoral maxillo-malar osteotomy: long-term results in 16 consecutive cases.

The authors report their experience with sixteen consecutive cases of maxillo-malar osteotomy via an intraoral approach. Some technical expedients and modifications to the original technique are described here. This procedure was associated in all cases with bilateral sagittal splitting of the mandible and in two cases with vertical reduction of the chin. Results from this series confirm the reliability of this technique and the stability of functional and aesthetic results.
