The vestibular system provides inputs to many neurons in the brain stem that participate in autonomic control. This multiplicity of vestibular-autonomic connections plays a variety of roles. Whereas it has been known for decades that unilateral vestibular lesions can result in motion sickness, recent data suggest that the vestibular system participates in making adjustments in blood pressure and respiration that are necessary to maintain homeostasis during movement and changes in posture. Animals with bilateral vestibular lesions are more susceptible to posturally related hypotension than vest

1. In systemic hypertension the pulmonary vessels show an excessive tone at rest and hyper-react to adrenoceptor stimulation. Alterations in Ca2+ handling by the vascular smooth muscle cells seem to underlie these disorders. Alveolar hypoxia also constricts pulmonary arteries, increasing the intracellular Ca2+ availability for smooth muscle contraction. This suggests the hypothesis that hypoxic pulmonary vasoconstriction depends on similar biochemical disorders, and that the response to the hypoxic stimulus may be emphasized in high blood pressure. 2. In 21 hypertensive and 10 normotensive men

The examination included 35 patients (21 women, 14 men, mean age 47.2 +/- 2.4 years) with anxious-phobic disorders (APD) and dyspnea phenomena combined bronchial asthma (BA) and hyperventilation syndrome (HVS). There was established that APD in these cases develops more frequently similarly to the panic attacks (PA) and HVS was PA component. Three PA types with dyspnea phenomena were recognized: cognitive ones (with prevalence of cognitive anxiety--8 cases); somatized PA (with prevalence of somatized anxiety--18 cases) and PA with both cognitive and somatized anxiety (9 cases). PA of the first

OBJECTIVE: Carbonation as a sensory enhancement strategy for prevention of aspiration of thin liquids has not been thoroughly studied. The aim of our study was to examine the effect of carbonation on penetration-aspiration and pharyngeal residue in dysphagia patients using Fiber-Optic Endoscopic Evaluation of Swallowing (FEES) and to identify parameters associated with a response to carbonation.
METHODS: A cross-sectional study of patients undergoing FEES in a dysphagia clinic. Patients were offered 100 cc of dyed water. Penetration-aspiration was scored using the penetration-aspiration scale

The Deakin/Graeff hypothesis proposes that different subpopulations of serotonergic neurons through topographically organized projections to forebrain and brainstem structures modulate the response to acute and chronic stressors, and that dysfunction of these neurons increases vulnerability to affective and anxiety disorders, including panic disorder. We outline evidence supporting the existence of a serotonergic system originally discussed by Deakin/Graeff that is implicated in the inhibition of panic-like behavioral and physiological responses. Evidence supporting this panic inhibition syste

Patients with neuromuscular disease are an important group at risk of frequently suffering acute or chronic respiratory failure, which is their main cause of death. They require follow-up by a pediatric respiratory medicine specialist from birth or diagnosis in order to confirm the diagnosis and treat any respiratory complications within a multidisciplinary context. The ventilatory support and the cough assistance have improved the quality of life and long-term survival for many of these patients. In this paper, the authors review the pathophysiology, respiratory function evaluation, sleep dis

The patient is a previously healthy Eastern-African man in his late 20s, with unknown vaccination status who presented to a US Army Combat Hospital in Iraq with acute upper and lower extremity weakness progressing to respiratory distress requiring intubation over the course of 12 h. His only antecedent symptoms were weakness and nausea. Laboratories including complete blood count, C reactive protein, cerebrospinal fluid analysis, venous blood gas, rapid HIV and chemistry were normal. Non-contrast head CT was normal. The patient was extubated after 24 h but continued to exhibit 2/5 strength in

In 1969 a new syndrome was described by Joubert et al. It is a very rare syndrome as only about 30 cases have been reported, and none in Yugoslavia. The main clinical characteristics are as ballows. - Disturbed respiratory rhythm alternating periods of hyperapnea and apnea) which tend to disappear as the child grows; - Abnormal eyemovements (jerky, rotatory, pendular); - Mental retardation (usually severe and which deteriorates with the age); and - Ataxia (sometimes also severe). The main neuropathological finding is dysplasia of the vermis cerebelli. Typical CT - findings consist of a very la

This paper contains a review of the signs and symptoms of giant cell arteritis, especially speech and respiratory manifestations of the disease, which are demonstrated by a new case history. An elderly woman presented with speech that was falsetto, breathy, and marked by downward pitch breaks and phonation breaks. In addition, she had visual loss, headache, edema along the scalp and pharyngeal arteries, polymyalgia rheumatica, elevation of Westergren erythrocyte sedimentation rates, and positive arterial biopsy results. Her speech disorder recurred during an exacerbation. A vascular mechanism

A peculiar case of neuro-sarcoidosis is reported. Clinical symptoms associated ocular dysfunction, hypopituitarism due to hypothalamic disorders, hypersomnia and alveolar hypoventilation. Dynamic tests have proved hypothalamic involvement. The patient died swiftly and suddenly. Neuropathological examination showed typical granuloma consistent with sarcoidosis mainly distributed to the area of the third ventricle and spread hyperplastic gliosis of pons and medulla.

OBJECTIVES: Inclusion body myositis (IBM) can result in deadly respiratory consequences. Yet, the mechanism driving this issue remains equivocal. We mapped the literature to identify physiological respiratory characteristics in IBM and the types of respiratory assessments used.
METHODS: We performed a scoping review using seven databases and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines.
RESULTS: Our search yielded 381 studies, of which 17 studies were reviewed. Case studies/series predominated (53%). Studies main

BACKGROUND: Respiratory dysfunction in people with multiple sclerosis (PwMS) is often attributed to respiratory pump failure, yet central respiratory drive and automatic breathing control remain underexplored. We assessed the prevalence of breathing automatic-voluntary dissociation (AVD) in PwMS.
METHODS: We analyzed prospectively collected data from a tertiary care rehabilitation center observational cohort. All adult PwMS, who underwent pulmonary function testing, including a CO₂ ventilatory response test, while relapse-free for ≥ 3 months, between January 2018 and December 2024 were screen

BACKGROUND: In extremely preterm infants, persistence of cardioventilatory events is associated with long-term morbidity. Therefore, the objective was to characterize physiologic growth curves of apnea, periodic breathing, intermittent hypoxemia, and bradycardia in extremely preterm infants during the first few months of life.
METHODS: The Prematurity-Related Ventilatory Control study included 717 preterm infants <29 weeks gestation. Waveforms were downloaded from bedside monitors with a novel sharing analytics strategy utilized to run software locally, with summary data sent to the Data Coor

Pressure support ventilation (PSV) is a commonly used mode. It is patient-triggered, pressure-limited, and (normally) flow-cycled. Triggering difficulty occurring during PSV is usually due to intrinsic positive end-expiratory pressure. The airway pressure generated at the initiation of inhalation is determined by the pressure support setting and the pressure rise time (pressurization rate) settings on the ventilator. The rise-time setting is clinician-adjustable on many current-generation ventilators. Flow delivery during PSV is determined by the pressure support setting, the pressure generate

In awake goats, 29% bilateral destruction of neurokinin-1 receptor-expressing neurons in the pre-Bötzinger complex (pre-BötzC) area with saporin conjugated to substance P results in transient disruptions of the normal pattern of eupneic respiratory muscle activation (Wenninger JM, Pan LG, Klum L, Leekley T, Bastastic J, Hodges MR, Feroah T, Davis S, and Forster HV. J Appl Physiol 97: 1620-1628, 2004). Therefore, the purpose of these studies was to determine whether large or total lesioning in the pre-BötzC area of goats would eliminate phasic diaphragm activity and the eupneic breathing patter
