[Assessing various aspects of the motivation to eat that can affect food intake and body weight control].

Over the last 30 years, several questionnaires have been developed and validated in order to assess many aspects of the motivation to eat that might be susceptible to impair adequate food intake and body weight control. A few of such questionnaires are described here, in particular, the "Three Factor Eating Questionnaire" also called the "Eating Inventory", and the "Dutch Eating Behavior Questionnaire". Critical aspects of the motivation to eat assessed by these tools are presented, such as dietary restraint, disinhibition, hunger, vulnerability to eat in response to external cues or emotional

Recent-Onset Altered Mental Status: Evaluation and Management.

Potential precipitating factors for the recent onset of altered mental status (AMS) include primary central nervous system insults, systemic infections, metabolic disturbances, toxin exposure, medications, chronic systemic diseases, and psychiatric conditions. Delirium is also an important manifestation of AMS, especially in older people who are hospitalized. Clinicians should identify and treat reversible causes of the AMS, some of which require urgent intervention to minimize morbidity and mortality. A history and physical examination guide diagnostic testing. Laboratory testing, chest radio

Self-reported incidence of verbal and physical violence against emergency medical services (EMS) personnel in Singapore.

BACKGROUND: Western studies have highlighted alarmingly high rates of work-related violence experienced by emergency medical services (EMS) staff. As there is a paucity of Asian studies, we aimed to investigate the incidence of physical and verbal violence against the EMS crew in the Singapore Civil Defence Force (SCDF). METHODS: This was a cross-sectional study, utilising an online survey made available to all active paramedics in the SCDF. Paramedics with at least one year of work experience were eligible to participate. All responses were kept anonymous. RESULTS: The response rate was 50.

[[Violence against the elderly--by the elderly: does it exist?--Medical aspects] ].

Thus far there has been very little research on violence in relation to age. This is due to various problems: the definition and delimitation of the term "violence" is unclear, and violence is considered an interactive event in which everyone involved is both victim and perpetrator. Violence is a taboo topic which people prefer to conceal, and those involved do not talk about the violence they experience. The different forms of violence are physical violence, psychic violence, neglect, restraint, financial exploitation and structural violence. Views on the incidence of violence vary: one certa

Psychobiological impact of a progressive weight loss program in obese men.

Psychobiological effects associated with a progressive body weight loss may help to improve the understanding of psychological barriers to weight loss. We hypothesized that a moderate body weight loss (about 10% of initial body weight) is accompanied with some negative psychological impact. Four questionnaires [SF-36 Health Survey, Three-Factor Eating Questionnaire (TFEQ), Beck Depression Inventory (BDI) and Pittsburgh Sleep Quality Index (PSQI)] were administered at each phase of a body weight loss program that consisted of a supervised diet and exercise clinical intervention to 11 obese men

The association between different types of childhood trauma and orthorexia nervosa: an observational study.

OBJECTIVE: Orthorexia Nervosa (ON) is characterized by the pursuit of perfect eating, leading to severe physical, psychological, and social problems. Although not yet formally recognized as an official diagnosis, ON shares etiological factors with traditional eating disorders, such as adverse childhood experiences (CT). This study explores the association between different CT types and ON, a relationship scarcely investigated in the literature. METHODS: A sample of 175 participants (age 18-74 years, M = 44.10; SD = 12.85) completed the Italian Düsseldorf Orthorexia Scale (I-DOS) and the Child

Nonoperative management of a patient with a two-part minimally displaced proximal humerus fracture: A case report.

Proximal humerus fractures account for up to 10% of all fractures; however, the literature lacks any detailed nonoperative management protocols or treatment progression guidelines that are based on large randomized controlled trials. Several studies support conservative treatment of minimally displaced fractures, yet they do not document the specific interventions used nor do they provide rationale for progression. This case report describes the conservative rehabilitation of a patient following a traumatic, minimally displaced, two-part proximal humerus fracture. The patient was a 58-year-old

Development of hand-arm bimanual intensive training (HABIT) for improving bimanual coordination in children with hemiplegic cerebral palsy.

Constraint-induced (CI) movement therapy is a physical intervention that has been receiving increasing attention in pediatric rehabilitation. So far, the evidence suggests that practice associated with CI therapy may improve impaired unimanual hand function in some children with hemiplegic cerebral palsy (CP). However, CI therapy has several important limitations. Most importantly, children with hemiplegia have impairments in bimanual coordination beyond their unilateral impairments. Thus, an intervention approach to increase functional independence during activities of daily living by using b

Role of endothelin-1 in stress response in the central nervous system.

Endothelin (ET)-1 is a 21-amino acid peptide that induces a variety of biological activities, including vasoconstriction and cell proliferation, and its likely involvement in cardiovascular and other diseases has recently led to broad clinical trials of ET receptor antagonists. ET-1 is widely distributed in the central nervous system (CNS), where it is thought to regulate hormone and neurotransmitter release. Here we show that CNS responses to emotional and physical stressors are differentially affected in heterozygous ET-1-knockout mice, which exhibited diminished aggressive and autonomic res

Use of a Novel Standardized Administration Protocol Reduces Agitation Pro Re Nata (PRN) Medication Requirements: The Birmingham Agitation Management (BAM) Initiative.

BACKGROUND: Agitation management is a principal challenge on inpatient psychiatric units. Overreliance on common prescribing strategies of pro re nata (PRN) medication administration is problematic, given the tendencies to have overlapping or unclear indications. OBJECTIVE: Piloted project to determine whether a standardized protocol for agitation intervention may reduce PRN medication administration. METHODS: The Birmingham Agitation Management (BAM) interdisciplinary team uniquely connected the Brøset Violence Checklist (BVC) for assessment of agitation severity to a standardized PRN medic

Risk factors for adult overweight and obesity in the Quebec Family Study: have we been barking up the wrong tree?

The aim of this study was to determine the independent contribution of previously reported risk factors for adult overweight and obesity. A cross-sectional (n=537) and a longitudinal (n=283; 6-year follow-up period) analysis was performed for nine risk factors for overweight and obesity assessed in adult participants (aged 18-64 years) of the Quebec Family Study (QFS). The main outcome measure was overweight/obesity, defined as a BMI>or=25 kg/m2. Using logistic regression analysis adjusted for age, sex, and socioeconomic status, short sleep duration, high disinhibition eating behavior, low die

Pathophysiology and Therapeutic Management of Bone Loss in Patients with Critical Illness.

Patients with critical illnesses are at higher risk of comorbidities, which can include bone mineral density loss, bone turnover marker increase, and fragility fractures. Patients admitted to intensive care units (ICUs) have a higher risk of bone fractures. Since hypermetabolism is a characteristic of ICU patients, such patients are often rapidly affected by systemic deterioration, which often results in systemic wasting disease. Major risk factors for ICU-related bone loss include physical restraint, inflammation, neuroendocrine stress, malnutrition, and medications. A medical history of crit

Forced running-induced rhabdomyolysis in the Sprague-Dawley rat: towards a rodent model of capture myopathy.

Capture myopathy (CM) is a metabolic disease associated with mortality in mass boma captured (MBC) wildlife. The condition is induced by the forced pursuit, capturing, and restraint of wild animals, although its causal biology remains to be confirmed. A core feature of MBC-CM is rhabdomyolysis, which is associated with myoglobinuria and hyperthermia. Towards developing a translational model of CM-associated rhabdomyolysis, we investigated forced treadmill running to induce physical exhaustion and trigger rhabdomyolysis in Sprague Dawley (SD) rats. Twenty-four (24) SD rats (12 per sex) were sub

Examining the distinctiveness of body image and self-related constructs in eating disorders using virtual reality: the role of shape concerns, physical self-worth, and global self-worth.

INTRODUCTION: In Western culture, the female body is commonly socially perceived as an object of evaluation, causing women to frequently evaluate their self-worth based on their physical appearance. Since the last decade, the use of virtual reality (VR) helped clarify the intricate interplay between broader self-related dimensions and dysfunctional eating attitudes and behaviors in women with eating disorders (EDs). The first objective of this study explores the role of body image attitudes (i.e., perceived physical attractiveness, body shape concerns), global self-worth, and physical self-wor

Restrictive use of Restraints and Delirium Duration in the Intensive Care Unit (R2D2-ICU): protocol for a French multicentre parallel-group open-label randomised controlled trial.

INTRODUCTION: Physical restraint (PR) is prescribed in patients receiving invasive mechanical ventilation in the intensive care unit (ICU) to avoid unplanned removal of medical devices. However, it is associated with an increased risk of delirium. We hypothesise that a restrictive use of PR, as compared with a systematic use, could reduce the duration of delirium in ICU patients receiving invasive mechanical ventilation. METHODS AND ANALYSIS: The Restrictive use of Restraints and Delirium Duration in ICU (R2D2-ICU) study is a national multicentric, parallel-group, randomised (1:1) open-label,

Posterior cruciate ligament biomechanics and options for surgical treatment.

Treatment algorithms for posterior cruciate ligament (PCL) injury have evolved over the past two decades as the natural history of these injuries has become clearer. Whether they are isolated injuries or occur with other ligament trauma, PCL ruptures substantially alter knee kinematics. Because of the effects of PCL injury and the less than optimal results after nonsurgical treatment, new surgical techniques have been developed. Several surgical techniques currently are available for reconstruction of the PCL. Most recent reports suggest that the tibial inlay technique is the best choice for r
