The effect of adjunctive hyperbaric oxygen therapy in patients with central retinal artery occlusion.

PURPOSE: Central retinal artery occlusion (CRAO) is an ophthalmic emergency with poor prognosis, despite diligent conventional treatment. According to the clinical recommendations of the Undersea and Hyperbaric Medical Society, hyperbaric oxygen (HBO2) is a potentially beneficial treatment; however, the benefit of adjunctive HBO2 in patients with CRAO in Korea remains unclear. The present study aimed to evaluate the effect of adjunctive HBO2 in patients with CRAO. METHODS: This registry-based observational study included adult patients who presented to the emergency department or ophthalmolog…

Circumpapillary collateral vessel development in iatrogenic central retinal artery occlusion observed using OCT angiography.

PURPOSE: It has been reported that peripapillary loops develop after central retinal artery occlusion (CRAO). Although cilioretinal anastomoses have been shown by fluorescein angiography (FA) and indocyanine green angiography, these examinations cannot confirm the structural continuity between the retinal arteries and the posterior ciliary arteries. In the current report, we followed a patient with iatrogenic CRAO in which circumpapillary collaterals formed and assessed the connection between these two vascular systems using optical coherence tomography angiography (OCTA). OBSERVATION: Iatrog…

Rate of strokes 1 year after retinal artery occlusion with analysis of risk groups.

BACKGROUND: The risk of developing stroke after retinal artery occlusion was reported to be increased. The aim of our study was to assess the rate of strokes/transitory ischemic attacks after retinal artery occlusion in a European population and to identify the risk groups. METHODS: All patients, diagnosed with branch or central retinal artery occlusion at our outpatient department since 2014, were asked to participate in this prospective case-control study. At the initial examination, the medical history was documented and 1 year after retinal artery occlusion, patients were called by teleph…

Central retinal artery occlusion and cerebral infarction associated with Mycoplasma pneumonia infection in children.

BACKGROUND: Central retinal artery occlusion (CRAO) is an arterial ischemic stroke, rarely occurred in children accompanied with asymptomatic cerebral infarction and almost never involved in severe pneumonia related to Mycoplasma pneumonia infection. CASE PRESENTATION: An 8-year-old boy with severe pneumonia related to Mycoplasma pneumonia infection that developed loss of vision in the left eye on the 14th day. No light perception and no pupillary reaction to light were found in the left eye. The fundus examination revealed a cherry red spot with severe retinal edema at the macular and peripa…

Retrobulbar Spot Sign Predicts Thrombolytic Treatment Effects and Etiology in Central Retinal Artery Occlusion.

BACKGROUND AND PURPOSE: Transorbital sonography may help establish diagnosis of central retinal artery occlusion (CRAO). Next to Doppler sonographic proof of CRAO, an intra-arterial spot sign can be detected in some cases. We hypothesized that it reflects calcified components. It may be associated with embolization from atherosclerotic plaques and may negatively influence thrombolysis. METHODS: Prospective monocenter study of 46 patients with ophthalmologically confirmed CRAO. Systemic tissue-type plasminogen activator thrombolysis was performed when appropriate. All patients received etiolog…

Central retinal artery occlusion occurring intraoperatively during angiography for subarachnoid hemorrhage: a case report.

PURPOSE: To report a case of central retinal artery occlusion that occurred during cerebral angiography for the treatment of vasospasm secondary to subarachnoid hemorrhage. RESULTS: Examination was notable for 20/Hand Motion vision and an afferent pupillary defect. Fundus examination at the time of the event revealed a cherry red spot typical of central retinal artery occlusion with surrounding edema of the retina and severe attenuation of the arterioles. There were no emboli noted. Review of the angiography confirmed flow to the ophthalmic artery and retina at the start of the procedure, wit…

Acute central retinal artery occlusion associated with livedoid vasculopathy: a variant of Sneddon's syndrome.

Livedoid vasculopathy (LV) is characterized by a long history of ulceration of the feet and legs and histopathology indicating a thrombotic process. We report a case of acute central retinal artery occlusion in a 32-year-old woman who had LV. She showed no discernible laboratory abnormalities such as antiphospholipid antibodies and no history of cerebrovascular accidents. Attempted intra-arterial thrombolysis showed no effect in restoring retinal arterial perfusion or vision. The central retinal artery occlusion accompanied by LV in this case could be regarded as a variant form of Sneddon's sy…

Sequential bilateral central retinal artery occlusion as the primary manifestation of systemic lupus erythematosus.

Bilateral central retinal artery occlusion (CRAO) has been rarely reported as the primary manifestation in patients with systemic lupus erythematosus (SLE). The severe retinal vaso-occlusive diseases usually cause devastating and permanent damage to visual function in spite of vigorous treatment. A 42-year-old Chinese woman presented with abrupt bilateral vision loss. The diagnosis of bilateral CRAO was suggested by the ocular presentation and fluorescein angiography. Laboratory studies showed positive results of antinuclear antibody, anti-Ro/SSA anti-La/SSB; decreased levels of C3, C4 complem…

A case report of embolic retinal artery occlusion: the lost measurement of healthcare quality.

In this case, we report a 43-year-old African-American woman who presented with acute vision loss of the left eye. The patient was found to have central retinal artery occlusion (CRAO). Clinical examination showed characteristic physical findings of severe mitral stenosis. Echoardiographic findings confirmed the diagnosis. Laboratory testing was negative for hypercoagulability diseases. The patient suffered permanent vision loss in her left eye due to thromboembolic phenomenon secondary to mitral stenosis. Few case reports in literature, most of which date back to more than half a century ago,…

DWI findings of optic nerve ischemia in the setting of central retinal artery occlusion.

A 67-year-old African-American male with untreated hypertension, hyperlipidemia, and diabetes mellitus presented with sudden, staggering, progressive loss of vision in his left eye over the course of 8 days. Ophthalmologic and fluorescein angiography exams confirmed central retinal artery conclusion, but revealed no embolus. Magnetic resonance imaging of the brain serendipitously revealed restricted diffusion within the distal left optic nerve, illustrating a more proximal occlusion, which matched the fluorescein angiographic findings. Extensive workup revealed no embolic source, postulating p…

Central retinal artery occlusion and recurrent papillitis in a patient with incomplete Behçet disease.

A 40-year-old man presented with painless sudden visual loss due to a central retinal artery occlusion (CRAO) in his right eye. Three months later, he had painless acute visual loss in his left eye with visual acuity of 20/100 and a swollen optic disc. After oral prednisone treatment, the disc swelling resolved and acuity recovered to 20/20. Five months later, the patient experienced another episode of vision loss in the left eye associated with optic disc edema. With steroid therapy, he regained 20/20 acuity once again. With a history of recurrent oral ulcers, fluorescein angiography showing …

A case of central retinal artery occlusion following embolization procedure for juvenile nasopharyngeal angiofibroma.

A 23-year-old male patient with right nasal Juvenile Nasopharyngeal Angiofibroma (JNA) developed Central Retinal Artery Occlusion (CRAO) during embolization of the tumor using polyvinyl alcohol particles before endoscopic excision. Classic CRAO management was initiated by an ophthalmologist after 12 h. Retrospective evaluation of the angiograms revealed a tiny communication between the external carotid and ophthalmic arteries which had not been noticed before embolization. During endoscopic excision, the tumor was found to originate extraordinarily from midline structures. It was concluded tha…
