Retroperitoneal fibrosis is an uncommon inflammatory disease which is characterized by the development of a fibrous process that surrounds the major vessels and organs located within the retroperitoneum. Isolated as an idiopathic expression or associated with several other diseases in its secondary form, retroperitoneal fibrosis remains relatively of rare occurrence and as a result, is devoid of either diagnostic or therapeutic definitive recommendations. We report the observation of one case admitted in the emergency room for abdominal pain induced by left hydronephrosis. Radiographic explora…

Sclerosing mesenteritis is a rare inflammatory disease of the bowel mesentery of unknown aetiology. It poses a diagnostic challenge for clinicians, as it can be mistaken for malignancy. We report a case of sclerosing mesenteritis initially presented with abdominal pain, tender abdominal mass and small bowel obstruction. Emergency laparotomy revealed fibrous thickening of the small bowel mesentery mimicking small bowel lymphoma. An ileo-ileal bypass procedure was performed. Six months later, the patient developed retroperitoneal fibrosis, manifesting as bilateral obstructive uropathy. Our case …

A 64-year-old woman presented with thrombosis of the iliac veins and the inferior caval vein. CT revealed retroperitoneal fibrosis as a probable cause.

The conventional treatment of patients with ureteric obstruction due to retroperitoneal fibrosis (RF) is surgery in combination with long-term corticosteroids. This report describes 11 cases of RF with ureteric obstruction treated with methyl-prednisolon pulse therapy (MPPT) in combination with azathioprine or penicillamine following initial insertion of ureteral stents. The medial treatment suggested was successful in 7 patients, but only moderately effective in the last 4 patients. This principle of non-operative management of RF has not been previously reported.

We report 2 patients in whom regression of ureteral obstruction due to recurrent idiopathic retroperitoneal fibrosis was obtained with steroids. Steroids are a valid alternative to surgery in the initial treatment of biopsy-proven benign (idiopathic) retroperitoneal fibrosis.

Idiopathic retroperitoneal fibrosis is a rare finding usually confined to the retroperitoneal space. Symptoms caused by this fibrotic process are usually secondary to compression and constriction of local anatomic structures. The ureters are the most common site of involvement, although the biliary tree, duodenum, and vasculature may also be affected. This case report describes a patient with retroperitoneal fibrosis involving the pancreas and common bile duct, and whose clinical course and investigative results stimulated cancer of the pancreas.

Based on 8 cases observed the author attempted to specify the possiblities of diagnosis and therapy of the idiopathic retroperitoneal fibrosis. If the pathogeny is very dark this illness is accompanied by an unspecific chronic inflammatory process. The radiology (U.I.V. and U.P.R.) does permit to affirm the affection. The ureterolysis with bilateral intraperitonization or, in unilateral cases, the extraperitonization of the ureter fixing the peritoneum to the psoas are the most effective surgical methods. They can be combined with the cortico-therapy which permits a recovery of the renal funct…

To investigate the diagnostic value of contrast-enhanced ultrasound (CEUS) in idiopathic retroperitoneal fibrosis (IRPF) activity and establish an optimal ultrasound-supplemented scoring method to evaluate the activity of IRPF. The retrospective study included 139 CEUS examinations of 59 IRPF patients treated in our hospital from January 2017 to January 2025. They were divided into the IRPF active group (66 examinations) and the inactive group (73 examinations). The parameters between the 2 groups were compared by univariate analysis. Spearman rank correlation was used to analyze the associati…

OBJECTIVES: Idiopathic retroperitoneal fibrosis (IRF) is a rare autoimmune-mediated condition characterized by fibro-inflammatory tissue development around the abdominal aorta and iliac arteries. Ureteral entrapment and hydronephrosis are the most common manifestations and acute renal failure or chronic renal insufficiency may occur as the consequence. Glucocorticoids and immunosuppressants may be effective but the therapeutic response and outcome are heterogeneous and hard to predict. This study aimed to investigate the factors associated with early remission in patients with IRF.

Retroperitoneal fibrosis (RPF) is a rare cause of postrenal acute kidney injury (AKI). We present a clinical case of a 65-year-old patient who developed acute right-sided hydronephrosis with postrenal AKI. Contrast-enhanced computed tomography showed dilation of the ureter, pelvis and calyxes on the right, diminished size of the left kidney and signs of RPF, including fat tissue thickening, compaction and stranding along the aorta and its visceral branches, mesenteric sinuses, thickening of the peritoneum at the level of the paracolic gutters, pelvic tissue compaction, descending and infrarena…
