Robotic-assisted transoral removal of a submandibular megalith.

The majority of salivary stones are less than 8 mm in size and most frequently occur in the submandibular gland. Traditional management of larger stones involves gland resection. Sialendoscopy combined with an external or a transoral sialolithotomy, also called the combined approach technique, permits stone removal and gland preservation. A 31-year-old male presented to our service with a 20-mm megalith in the left submandibular gland. Here we report the first description of a combined approach using the da Vinci Si Surgical System to facilitate transoral stone removal and salivary duct repair

Safety and learning curve in robotic colorectal surgery.

Robotic surgery has recently started to be used for minimally invasive colorectal surgery. Because of limited access and high cost, very few colorectal units are available in the US. We describe our experience with benign and malignant disease since September 2008 in a dedicated colorectal practice. A prospective collected robotic database was queried for colon and rectal procedures. Anonymized demographic, intraoperative, and postoperative data, and pathology information, were collected and analyzed. A total of 48 robotic procedures for colorectal maladies were performed in the study period.

Transoral robotic resection of recurrent nasopharyngeal carcinoma.

Transoral robotic nasopharyngectomy was carried out for a patient who developed recurrent nasopharyngeal cancer after radiotherapy. The tumor in the left lateral wall of the nasopharynx was exposed after splitting the soft palate and a curative resection, including the medial crus of the eustachian tube was carried out under the three-dimensional 0° camera. The operative procedure was uncomplicated, blood loss was minimal and the patient recovered smoothly. The camera of the surgical robot provided superb visualization of the operative field and the maneuverability of the robotic arms with the

Skill generalization relevant to robotic neuro-rehabilitation.

Upper limb extremity rehabilitation practices are increasingly involving robotic interaction for repetitive practice, and there is increasing skepticism whether such systems can provide the relevant practice that can be generalized (or transferred) to functional activities in the real world. Most importantly, will patients be able to generalize in three critical ways: (1) to unpracticed directions, (2) to unpracticed movement distances, and (3) to unpracticed weight-eliminated conditions? Rather than presuming that patients could generalize in three conditions, this study tested whether there

[Technical aspects of laparoscopic robot-assisted pyeloplasty].

From 2000, the robot-assisted laparoscopic approach has been developed for the management of ureteropelvic junction obstruction (UJO) with equivalent outcomes to conventional laparoscopic access regarding functional results. This system has simplified the suturing and has improved the precision of operative technique. The main surgical steps of the transperitoneal laparoscopic robot-assisted pyeloplasty are as follows: four or five port arrangement; initial dissection and early identification of the ureteropelvic junction; renal pelvis section; transection of the ureter and preparation of a sp

Robot-assisted thoracoscopic resection of intralobar sequestration.

In this paper, we report the first case of a robot-assisted thoracoscopic resection of intralobar sequestration. By virtue of greater dexterity and three-dimensional visualization, the da Vinci robot enables a safer, more precise dissection of sequestered tissue in the face of chronic inflammatory adhesions than conventional video-assisted thoracoscopic surgery does. Thus, in expert hands, such robotic technology is likely to result in less bleeding complications and less conversions to open surgery in cases of sequestration.

Natural orifice surgery with an endoluminal mobile robot.

Natural orifice transgastric endoscopic surgery promises to eliminate skin incisions and reduce postoperative pain and discomfort. Such an approach provides a distinct benefit as compared with conventional laparoscopy, in which multiple entry incisions are required for tools and camera. Endoscopy currently is the only method for performing procedures through the gastrointestinal tract. However, this approach is limited by instrumentation and the need to pass the entire scope into the patient. In contrast, an untethered miniature robot inserted through the mouth would be able to enter the abdom

Tubal anastomosis by robotic compared with outpatient minilaparotomy.

OBJECTIVE: To compare tubal anastomosis by robotic system compared with outpatient minilaparotomy. METHODS: In this retrospective case-control study, women were identified by current procedural terminology code for tubal anastomosis. We included all cases of tubal anastomosis for reversal of a prior tubal ligation by either outpatient minilaparotomy or robotic system technique. Cases performed by laparoscopy without aid of the robot were excluded. Comparisons were based on Fisher's exact, chi(2), and Wilcoxon rank sum tests. RESULTS: There were 26 cases of tubal anastomosis performed with th

Care of the patient undergoing robotic-assisted prostatectomy.

Prostate cancer has many treatment options. In addition to open retropubic and perineal approaches to radical prostatectomy, laparoscopic robotic prostatectomy is available as a newer surgical option. Potential advantages of robotic surgery include reduced pain and trauma, less blood loss, reduced infection risk, shorter hospital stay, faster recovery, and less scarring (Intuitive Surgical, 2005). A variety of nursing care considerations involving pre-operative education and preparation, intra-operative and immediate postoperative care issues, and long-term followup must be understood to meet

Laparoscopic vesicovaginal fistula repair with robotic reconstruction.

INTRODUCTION: To describe, to our knowledge, the first case report of robotic-assisted laparoscopic repair of a vesicovaginal fistula. A 44-year-old woman presented with a vesicovaginal fistula after vaginal hysterectomy. She had been noted to have a bladder injury that was repaired at that time. A vesicovaginal fistula developed several weeks later, and she was referred for repair. The location of the fistula was deemed amenable to repair using a robot-assisted laparoscopic approach. TECHNICAL CONSIDERATIONS: The total operative time was 280 minutes, including placement of ureteral catheters

Robotic laparoscopic radical prostatectomy with a single assistant.

INTRODUCTION: As experience with robot-assisted laparoscopic radical prostatectomy (rLRP) grows, the importance of the role of the assistant cannot be overstated. Key elements of the operation, dependent on the assistant, have not been reported. The focus of this report was to describe the key elements, which include positioning of the patient and assistant port sites, appropriate equipment, and the assistant-dependent steps of the procedure. TECHNICAL CONSIDERATIONS: During the course of the initial 100 cases, we reviewed the patient and port site positioning and reviewed the videotapes to i

Assessing human-robotic performance for vocational placement.

This paper describes the results of an exploratory study of the use of standard occupational therapy assessment tests to measure the effective manipulation ability of individuals with disabilities using a robotic aid. Robotic manipulators have been explored for use as a vocational accommodation to support the job placement of individuals with severe manipulation disabilities. One of the factors that has impeded the transfer of this work is the lack of practical information that is relevant to the vocational placement process. The preliminary performance data presented in this paper provides an

Voice-controlled robotic arm in laparoscopic surgery.

AIM: To report on our experience with a voice-directed robotic arm for scope management in different procedures for "solo-surgery" and in complex laparoscopic operations. METHODS: A chip card with orders for the robotic arm is individually manufactured for every user. A surgeon gives order through a microphone and the optic field is thus under direct command of the surgeon. RESULTS: We analyzed 200 cases of laparoscopic procedures (gallbladder, stomach, colon, and hernia repair) done with the robotic arm. In each procedure the robotic arm worked precisely; voice understanding was exact and f

Laparoscopic and Robotic Device Malfunction: A Scoping Review.

Laparoscopic and robotic surgeries have become the gold standard for many operations. Device malfunctions during these procedures represent a preventable source of patient harm; however, the trends in malfunction reporting and their implications for surveillance systems, surgical training, and reporting infrastructure have not been comprehensively reviewed. Our aim is to conduct a scoping review of device malfunctions during minimally invasive abdominopelvic surgeries. Four databases (Web of Science, Scopus, Embase, and PubMed) were searched for relevant articles published between January 1, 1
