The authors report the case of a delayed presentation of a traumatic diaphragmatic rupture in a 22-year-old patient admitted to hospital for a minor surgical procedure under general anaesthesia. Nine months before, he had a road traffic accident with a minor thoracic trauma. Three days after surgery, the patient was readmitted for a tension hydrothorax due to the herniation and the perforation of the stomach into the left pleural cavity. Such a delayed presentation of a traumatic diaphragmatic rupture remains uncommon. The peroperative ventilatory factors involved in the development of the her…

We have surgically treated six patients with bronchial rupture caused by blunt chest injury in the past 5 years. All injuries resulted from traffic accidents, except in one patient who was hit by a crane. Clinical manifestations included chest pain (n = 6), subcutaneous emphysema (n = 4), and dyspnea (n = 6). Roentgenographic findings were tension (n = 3) or nontension (n = 3) pneumothorax, subcutaneous emphysema (n = 4), pneumomediastinum (n = 3), deep cervical emphysema (n = 5), and delayed collapse of the affected lung (n = 3). Three patients had associated injuries: right clavicle and rib …

A 72-year-old man developed a total atelectasis of the right lung six weeks after a blunt trauma in a motor vehicle accident. Bronchoscopy demonstrated a total fibrotic occlusion of the right main bronchus. By re-examination of earlier case sheets and chest x-rays it was suspected that the patient had suffered a partiel bronchial rupture from the accident. One and a half centimetres of the bronchus were resected with a good result. Findings and treatment of this rare complication are discussed.

Chronic rupture of the posterior tibial tendon is a common disorder causing a progressive flat foot deformity. MR imaging is an excellent modality for assessing dysfunction of the tendon. It can easily and accurately detect and grade the tears. A variety of secondary soft tissue and bony abnormalities are associated with the tear and can be useful in confirming the diagnosis.

BACKGROUND: Spontaneous rupture of a hyperplastic parathyroid gland or adenoma resulting in extracapsular hemorrhage is extremely rare.
METHODS: We report a case of traumatic rupture of a cervical parathyroid gland adenoma.
RESULTS: The patient had progressive stridor, neck swelling, and chest and back pain with evidence of ongoing blood loss resulting in airway compromise. Exploration of the neck and superior mediastinum revealed hypercellular parathyroid tissue consistent with adenoma or hyperplasia.
CONCLUSIONS: Traumatic rupture of an enlarged parathyroid gland is a distinct and potenti…

Hydatid cysts are common in Turkey. Authors have seen 248 cases of lung hydatid cysts; Fifty hydatid cyst of lung had ruptured. Their symptoms and detailed findings are reported. Rupture caused asphyxia in 3, haemoptysis in 20, lung abcess in 16, rupture in pleural cavity in 4 and anaphylactic reaction in one case.

Atraumatic spontaneous vesical fracture is an uncommon occurrence. Here we present the case of a female patient with spontaneous vesical fracture secondary to urinary tract tuberculosis. A review of current literature showed another published case similar to ours. A brief comment is made on presentation signs and symptoms and picture evolution, as well as a discussion on the predisposing factors of spontaneous vesical fracture and its treatment.

Five cases of spontaneous rupture of the oesophagus are reported. All cases had surgery within 12 hours, and all survived, illustrating the value of early diagnosis in this rare condition. None had their diagnosis made before admission to the hospital. Myocardial infarction is the commonest misdiagnosis and frequently results in delayed treatment. We believe that a simple direct question to enquire of the patient whether or not vomiting preceded the onset of the severe pain would significantly reduce the rate of misdiagnosis.

Hitherto the size of abdominal aortic aneurysms (AAA) has been considered the most important factor in determining the risk of rupture. For this reason most interest has been devoted to physical, echographic and tomographic analyses of the shape of AAA. However, it is known that rupture can also occur in small AAA. Other factors must be considered to have an important role in the natural history of aneurysms. The aim of this study was to characterise the mechanical stress in the wall of an AAA due to pressure in the presence of atherosclerosis, intraluminal thrombus and anatomical restraints. …

The subject of this report is a 57-year-old obese, hypertensive woman who had been well until the onset of severe chest pain and hypotension … ay, she had a bout of severe dyspnea. A new systolic murmur was heard and the clinical diagnosis of ruptured papillary muscle was made and confirmed by echocardiography, and later at autopsy. All three coronary arteries were severely atherosclerotic and, in addition, the right coronary artery was completely closed by a thrombus. This case clearly illustra …

UNLABELLED: In the time interval 1979-1991, 150 cases of juvenile sudden death (< or = 35 years) were studied. Among these, 9 were due to aortic rupture within the pericardial cavity. Two were mycotic and 7 dissecting aneurysms. The latter, all male, age ranging from 17 to 31 years (mean 24), presented the following risk factors: Marfan syndrome in 2, isthmal coarctation associated with bicuspid aortic valve (BAV) in 3, isolated and normally functioning BAV in 2. Histology of the dissected aortic wall showed an equal severity of degenerative changes consisting of elastic fragmentation, cystic …

We present the case of a drug addict who was admitted with abdominal pain but gave no history of trauma. He subsequently left the hospital against medical advice, only to be readmitted a few days later with persistent abdominal pain. He was found to have splenic rupture. This case emphasizes the importance of ruling out intra-abdominal trauma in any drug addict presenting with acute abdomen.

In the period 1978-1990, 100 patients (44 males, 56 females) with various pathology, had a catheterisation of a central vein in the Hospital of Susa (TO). Subclavian vein was chosen with a percutaneous infraclavicular route. In 7 cases (7%) there were some mechanical complications, one of them was the accidental breakage of the catheter and its migration to the right heart. The authors describe this case and the possible cure by examining the most recent literature data.

A case of aortic valve rupture as a result of blunt trauma by motorcycle accident is presented. A previously healthy 18-year-old male developed severe congestive heart failure 46 days after the initial trauma. There were crescentic tears in the right and non-coronary cusps parallel to and along the whole length of the cusps. The valve was excised and replaced with a 21 mm St. Jude Medical prosthesis. Following the procedure he made an relatively uneventful recovery and was discharged on the 83rd postoperative day. The possibility of damage to the heart valves in traffic accidents must be borne…

The most common site of traumatic aortic tear is at the isthmus, and there have been few reports of successful surgical repair of traumatic rupture of the aortic arch. We have experienced a 18-year-old patient who sustained a blunt chest trauma in a traffic accident. After admission to our hospital, his general condition became stabilized by conservative therapy. But the chest X-ray showed widening of mediastinum and aortography demonstrated leakage of contrast media outside of the aortic arch lumen between the innominate and the left common carotid arteries. The patient underwent reconstructi…

Excluding labor, the greatest risks to the fetus from preterm PROM are umbilical cord accidents and infection. Heretofore, the clinical options for the PROM patient have been limited. With the advent and refinement of amnioinfusion, the utility of expectant management may be greatly increased. Through the use of amnioinfusion or its permutations, a number of diagnostic and therapeutic procedures previously unavailable to the PROM patient may become routine, including amniotic fluid volume expansion and direct in utero prophylaxis/treatment of amnionitis.
