The number of carpal bones may be increased or decreased by the fact of anatomical variants or true congenital anomalies. Numerical increment arises from additional or from split bones. Over twenty accessory carpal bones have been described but the commonest are the os centrale carpi, the os radiale externum, the triangular bone and the styloideum bone. Additional carpal bones usually result from a failure of fusion of their ossification centers. A congenital origin is not clearly established for all these ossicles. The scaphoid and lunate may split into two or three bones and several cases of

Carpal instability can be understood as a disturbed anatomical alignment between bones articulating in the carpus. This disturbed balance occurs either only dynamically (with movement) under the effect of physiological force or even statically at rest. The most common cause of carpal instability is wrist trauma with rupture of the stabilizing ligaments and adaptive misalignment following fractures of the radius or carpus. Carpal collapse plays a special role in this mechanism due to non-healed fracture of the scaphoid bone. In addition degenerative inflammatory alterations, such as chondrocalc…

AIMS: To compare complications associated with dorsal percutaneous and limited dorsal approaches in the surgical treatment of fractured scaphoid bone.
METHODS: A total of 51 patients with acute type A2, B2 and B3 scaphoid fractures were treated by limited dorsal approach. During follow-up examinations we analysed the functional outcome and per- and post-operative complications, and we compared them with studies using the dorsal percutaneous approach.
RESULTS: We found fewer complications using the limited dorsal approach.
CONCLUSIONS: We found no advantages of the dorsal percutaneous approa…

BACKGROUND: The purpose of this retrospective comparative study was to assess whether a complementary treatment by pulsed electromagnetic field could increase the bone-healing rate of scaphoid non-union without SNAC (scaphoid non-union advanced collapse) treated by retrograde percutaneous screw fixation.
CASE DESCRIPTION: Eighteen patients with scaphoid non-union were included in this retrospective study. The group 1 was made of nine cases (seven stage IIA and two stage IIB) of scaphoid non-union treated by retrograde percutaneous screw fixation and pulsed electromagnetic fields (Physiostim®)

OBJECTIVE: The objective of this study was to compare the diagnostic value of panoramic and conventional radiography in the detection of fractures of the carpal scaphoid bone.
MATERIALS AND METHODS: Panoramic (orthopantomographic) and conventional radiographs of 90 patients with acute or chronic wrist trauma were reviewed retrospectively. Images were analyzed and reviewed independently by four observers: two radiologists and two traumatologists. The kappa statistic was used to calculate intraand interobserver agreement and the correlation between the two imaging techniques.
RESULTS: Panorami…

Fracture of the scaphoid is the most common injury of the carpus. Inadequate treatment frequently leads to non-union, which causes pain and serious disability. In the present study 134 scaphoid fractures treated during the period 1961-70 are reviewed. In fresh fractures less than 4 weeks old, bony union occurred in 95% with proper conservative treatment. The vexed question of whether a high or low circular cast should be used may not really be so important. In the present study, both methods of external splintage have given favourable results. It is indisputable, however, that other factors, s

The scaphoid is the most commonly fractured carpal bone. With high clinical suspicion and negative radiographs, expedient evaluation by CT or MRI has been recommended. When treating nondisplaced or minimally displaced scaphoid waist and distal pole fractures, immobilization below the elbow without inclusion of the thumb is an option. Comparatively, early surgical intervention for nondisplaced or minimally displaced scaphoid waist fractures allows for quicker return of function, but with increased risk of surgical complications and no long-term outcomes differences compared with cast immobiliza

PURPOSE: The aim of this prospective study was to evaluate the diagnostic accuracy of multidetector computed tomography (MDCT) in detecting occult scaphoid fractures.
MATERIALS AND METHODS: A total of 54 patients with a clinically suspected scaphoid fracture and negative initial conventional radiographs were evaluated with 64-row MDCT wrist examinations within 1 week of the trauma. The gold standard used was the diagnosis on MRI done within 1 week after MDCT. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of MDCT were calculated.
RESULTS: M

The study was performed to determine the diagnostic value of different MR systems and field strengths in patients with occult scaphoid and wrist fractures. Twelve patients with clinical suspicion of a scaphoid fracture but normal plain radiographs were examined by MRI. A dedicated 0.2-T unit (Esaote) and a 1.0-T unit (NT10, Philips) were used. Coronal T1W-SE, STIR, and T2*W-GE sequences were obtained with both systems. Images were evaluated for a bone marrow abnormality, a trabecular or cortical fracture line, and were compared to the 6-week follow-up radiographs. Seven wrist fractures were fo

Introduction Scaphoid fractures are the most common carpal bone injury and a frequent reason for wrist immobilisation following trauma. However, many patients with clinical suspicion of scaphoid fracture are immobilised despite negative radiographs, and ultimately have alternative diagnoses. This study aimed to evaluate early management, imaging, and outcomes for patients presenting with suspected scaphoid fractures at a District General Hospital (DGH) in the United Kingdom (UK), comparing practice with current British Society for Surgery of the Hand (BSSH) standards. Methods A retrospective a

Occult fractures of the carpals and metacarpals occur frequently and are associated with major disability. The cross-sectional display and superior resolution of CT are well suited to the demonstration of these fractures. Sixteen patients with pain and negative conventional radiographs were chosen for further evaluation by CT. Imaging was performed in two planes and showed 21 fractures. The transaxial plane was the most revealing imaging plane. Coronal imaging was equal or superior to the transaxial plane in most fractures of the scaphoid, lunate, capitate, and pisiform bones and of the bases

INTRODUCTION: Scaphoid fractures are the commonest carpal bone fracture. If untreated they pose significant risk to patients, thus if a scaphoid fracture is suspected, patients are managed with immobilisation. Although scaphoid fractures may be difficult to diagnose on plain radiography, sometimes for months after injury, ongoing radiographic surveillance is preferred due to its low upfront cost. Patients in immobilising casts for long periods experience significant personal and social ramifications such as difficulty working and self-caring. This study examines whether cross-sectional imaging

BACKGROUND: The literature describes the treatment of scaphoid fractures comparing the volar and dorsal approaches, the advantages and disadvantages of percutaneous screw fixation, as well as the treatment of scaphoid nonunions using different types of cancellous or corticocancellous bone grafts. Yet, to date no studies are available comparing the outcome of rotational stability in screw-fixed scaphoid fractures to angular stable systems. The purpose of this study is to provide reliable data about rotational stability in stabilised scaphoid fractures and to gain information about the rigidity

Idiopathic avascular necrosis of the scaphoid, or Preiser disease, is a rare condition. Both the etiology and the pathology are still not well understood. We here present 2 cases with this disease and discuss the recommended treatment according to the stage of disease progression and by reference to previous studies in the literature. Similar to Kienböck disease, the recommended treatment should be selected according to the degree of disease progression. We believe that vascularized bone graft should be restricted to Herbert stage I or II cases with no evidence of radiocarpal arthritis or carp

We describe a case of a bilateral stress fracture of the carpal scaphoid in a 13-year-old boy. The patient played as a goalkeeper from the age of 5 years and presented with a 2-year history of intermittent pain in both wrists. Our patient underwent open reduction and bone grafting from the iliac crest in both wrists (6 months apart). He subsequently became asymptomatic and resumed sport activities. This patient presented with what was considered a rare case of a stress fracture of both scaphoid bones secondary to repetitive dorsiflexed movements. The patient denied the presence of acute sympto…

Background. Lack of proper union in scaphoid fractures - particularly in the form of pseudarthrosis - is a complication that continues to be an object of concern for orthopedic surgeons. Treatment failures result from non-union or osteoarthritis. Material and methods. In the years 1998-2005 we treated 31 patients in our clinic for delayed union or non-union of the scaphoid. Corticocancellous bone graft (the Matti-Russe method) was performed in 23 cases, the Beck operation in 4 cases, and proximal resection in another 4. Results. We achieved union in every case of delayed union, in 19 cases aft

To enhance the fixation of scaphoid fractures by providing precise alignment and compression at the fracture site, the Huene alignment guide has been used in conjunction with bone screws. In an effort to predict the compressive force generated by the Huene guide alone, these forces were measured with a calibrated, strain-gauge, instrumented device. Three orthopedic surgeons each tested the guide 10 times on one cadaver scaphoid bone specimen. The magnitude of the compressive forces generated ranged from 9.0 to 67.0 N (mean 36.0 +/- 15.0 N). Comparisons of the force measurements among the indiv…

Enchondroma is a common bone tumor of hand. It is rarely localized in scaphoid. We report a new case of this tumor revealed by fracture. The diagnosis was suggested by standard radiography and confirmed then by anatomic pathology examination. Enucleation and bone graft with osteosynthesis yielded good result.

Carpal injuries in children are uncommon. Radiographic diagnosis can be difficult because of late ossification of carpal bones. Scaphoid fracture occurs in 3% of hand and wrist fractures. Fractures in the distal third or at the waist are the most frequent fracture sites. In case of clinical and X-rays presumption, CT-scan and MRI often diagnose scaphoid fractures. Orthopaedic treatment with closed cast for 8 weeks, or more for adolescents, gives a high rate of union in non-displaced or minimally-displaced fractures. Non-union is the most frequent complication. Closed cast can be the treatment
