Revitalizing Recovery: Unveiling the Transformative Impact of Physiotherapy in Rehabilitating a Neglected Case of Shoulder Hemiarthroplasty.

The shoulder joint has a complex anatomy and biomechanics. It is a ball and socket joint made by the articulation surface of the humeral head (ball) and glenoidal fossa (socket) of the scapula. Shoulder arthroplasty is done when parts of the shoulder joint are severely affected and damaged beyond repair. The damaged parts are replaced with artificial parts. Prosthetic implants are typically made of metal or plastic material. Implants come in various sizes and shapes. There are three types of surgical arthroplasty: total shoulder arthroplasty, partial shoulder arthroplasty, and reverse arthropl

Clinical investigation for displaced proximal humeral fractures in the elderly: a randomized study of two surgical treatments: reverse total prosthetic replacement versus angular stable plate Philos (The DELPHI-trial).

BACKGROUND: Treatment for displaced proximal humeral fractures is still under debate. Few studies exist at the highest level of evidence. Although reversed total shoulder prosthesis has gained popularity and showed promising results in the treatment for proximal humeral fractures in the elderly patients, no randomized controlled trials exist to the authors' knowledge. METHODS/DESIGN: This study is a randomized semi-blinded controlled multicenter trial designed according to the Consort statement and the recommendations given by the Cochrane reviewers for proximal humeral fractures. The study w

Intramedullary nailing as a safe and efficient treatment option for all types of displaced proximal humeral fractures in geriatric patients: A retrospective evaluation of 49 patients treated with the new Targon PH+ nail.

BACKGROUND: Implant specific complications and outcome for the use of proximal humeral nails in different fracture types are not well described. OBJECTIVE: Evaluation of adverse effects and outcome of patients treated with the new Targon PH+ implant for proximal humeral fractures in a geriatric population. METHODS: A consecutive series of patients from a single institution was retrospectively evaluated. Adverse events (AE) were assessed from patients' files and during follow-up examination. Current pain level was evaluated using the Visual Analogue Scale (VAS). The disability score of arm, s

[Periprosthetic humeral fractures: from osteosynthesis to prosthetic replacement].

OBJECTIVE: Stabilization of the humeral shaft and the restoration of shoulder function dependent on the osseous integrity of the prosthetic stem component. INDICATIONS: In cases of a stable prosthesis stem, an isolated plate osteosynthesis is possible. Prosthesis stem replacement is indicated in cases of a loose stem. With sufficient bone stock of the proximal humeral segment, a change to a shorter humeral shaft component with subsequent plate fixation of the fracture is possible. If the bone stock is poor, conversion to a long revision stem is necessary. CONTRAINDICATIONS: Inoperability of

Use of a high-speed pencil-tip burr for removal of well-fixed humeral components in revision shoulder arthroplasty.

BACKGROUND: Removal of a well-fixed humeral stemmed component during revision total shoulder arthroplasty (TSA) remains technically challenging and carries a risk of intra-operative fracture and bone loss. Various osteotomy-based methods have been described to facilitate extraction, though each carries its own morbidity. The high-speed pencil-tip burr offers an alternative method for controlled disruption of the bone-implant or cement-bone interface, potentially reducing complications and bone loss. The purpose of this study was to evaluate radiographic and clinical outcomes following revision

[Fractures of the scapula].

Two thirds of the fractures of the scapula refer to its body, one third to its neck, and one forth to the glenoid. They are indicative of high-energy blunt trauma. Traffic injuries are their main cause. Associated ipsilateral upper torso injuries are common and have to be considered!Scapular fractures may be classified as being extra- or intraarticular. Extraarticular fractures concern the scapular body, its processes, or the scapular neck. A fracture of the surgical neck of the scapula accompanied by an injury to the coracoclavicular connection is regarded as a "floating shoulder". In this ca

Postoperative Morbidity by Procedure and Patient Factors Influencing Major Complications Within 30 Days Following Shoulder Surgery.

BACKGROUND: Little data are available to prioritize quality improvement initiatives in shoulder surgery. PURPOSE: To stratify the risk for 30-day postoperative morbidity in commonly performed surgical procedures about the shoulder completed in a hospital setting and to determine patient factors associated with major complications. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: This retrospective study utilized the National Surgical Quality Improvement Program (NSQIP) database from the years 2005 to 2010. Using Current Procedural Terminology codes, the database was queried for sh

[Not Available].

PURPOSE OF THE STUDY Fractures of the scapula are less frequent, with the incidence reported in literature of approximately 1% of all fractures and 3-5% fractures of shoulder. These fractures are predominantly treated non-operatively. Osteosynthesis is indicated in displaced intra-articular fractures and severely displaced extra-articular fractures of the scapular body, its lateral border in particular. Apart from open reposition and osteosynthesis, also minimally invasive osteosynthesis under fluoroscopic and arthroscopic control has recently been used to treat intra-articular fractures of th

Reverse arthroplasty compared to hemiarthroplasty and open reduction and internal fixation for displaced proximal humerus fracture in patients above 60: a Bayesian network meta-analysis.

INTRODUCTION: The present Bayesian network meta-analysis compared reverse total shoulder arthroplasty (rTSA) to hemiarthroplasty (HA) and open reduction and internal fixation (ORIF) for displaced proximal humeral fractures (PHF) in patients above 60. METHODS: This study was conducted according to the PRISMA extension statement for reporting systematic reviews incorporating network meta-analyses of healthcare interventions. In December 2024, PubMed, Web of Science, and Embase databases were accessed. No time constraint was set for the search. All clinical studies were accessed comparing rTSA, O

Glenohumeral arthritis as a risk factor for proximal humerus nonunion.

BACKGROUND: Proximal humerus fractures are common injuries and nonsurgical treatment has proven to yield good to excellent clinical results. A small percentage of these fractures go on to delayed or nonunion and the incidence and risk factors for this complication are poorly understood. We hypothesize that adjacent joint stiffness of the glenohumeral joint might lead to an increased rate of nonunion for proximal humerus fractures. METHODS: A search of the entire Medicare database from 2005 to 2011 was performed to identify 38,754 patients who sustained a proximal humerus fracture including 13

The AO Foundation and Orthopaedic Trauma Association (AO/OTA) scapula fracture classification system: focus on glenoid fossa involvement.

BACKGROUND: Fractures of the glenoid frequently require surgical treatment. A comprehensive and reliable scapula classification system involving the glenoid fracture patterns is needed to describe the underlying pathology. The AO Scapula Classification Group introduces an appropriate novel system that is presented along with its inter-rater reliability and accuracy. MATERIALS AND METHODS: An iterative consensus process (involving a series of face-to-face meetings and agreement studies) with an international group of 7 experienced shoulder surgeons was used to specify and evaluate a scapular f

Reverse shoulder replacement for patients with inflammatory arthritis.

PURPOSE: Patients with inflammatory arthritis frequently develop destructive shoulder arthritis and rotator cuff tearing. Reconstruction with anatomical shoulder replacement produces lesser results than for other etiologies such as osteoarthritis. We postulated that reconstruction with reverse shoulder prostheses would lead to early satisfactory results for these patients. METHODS: We carried out a retrospective review of 19 shoulders treated with reverse shoulder prostheses at a mean follow-up of 37 months. We rated patients' pain using a visual analog pain scale (VAPS) and noted active rang

Midterm outcome and complications after minimally invasive treatment of displaced proximal humeral fractures in patients younger than 70 years using the Humerusblock.

INTRODUCTION: The Humerusblock (HB) represents a minimally invasive implant allowing for the stabilisation of proximal humeral fractures after closed or percutaneous reduction. The aim of the study was to perform a general clinical and radiological midterm follow-up focusing on the quality and complications in a large series of patients of younger age (<70 years). PATIENTS AND METHODS: A total of 126 patients with an average age of 53.6 years treated surgically using the HB device were evaluated clinically using the Constant score (CS) and radiologically by biplanar radiographs after a mean f

Fixation of displaced midshaft clavicle fractures in skeletally immature patients.

BACKGROUND: There is ongoing debate in the adult literature regarding fixation of displaced, closed midshaft clavicle fractures. Functional outcomes of treatment of these fractures in skeletally immature patients have not been previously investigated. METHODS: We retrospectively reviewed 14 skeletally immature patients with closed, displaced, midshaft clavicle fractures treated with open reduction internal fixation. Baseline data acquisition included demographic and radiographic indices. Follow-up data included radiographic and functional outcomes assessment using the Quick Disability of Arm,

Complications in Shoulder Arthroplasty: Why Do They Happen and How to Manage Them.

Complications occur with total shoulder arthroplasty (TSA), and they can be daunting to diagnose and treat. It is important to review common TSA-related complications and to summarize risk factors along with causes of these complications and how to avoid them. The orthopaedic surgeon should be knowledgeable about how to successfully manage complications to achieve good patient outcomes and the etiologies and management of the painful and stiff shoulder arthroplasty, subscapularis failure after anatomic TSA, instability after reverse shoulder arthroplasty, and acromion stress fractures in the s

Prosthetic replacement for proximal humeral fractures.

The ideal management of complex proximal humeral fractures continues to be debatable. Evolution of proximal humeral fracture management, during the past decade, led to the implementation of many innovations in surgical treatment. Even though the pendulum of treatment seems to swing towards new trends such as locked plating, hemiarthroplasty remains a valid and reliable option that serves the patient's needs well. Hemiarthroplasty is indicated for complex proximal humeral fractures in elderly patients with poor bone stock and when internal fixation is difficult or unreliable. Hemiarthroplasty p

Surgical stabilization of the ipsilateral scapula and rib fractures using the mirror Judet approach: a preliminary result.

BACKGROUND: We report our preliminary results using a single approach, the mirror Judet approach, for patients with both ipsilateral scapula and multiple rib fractures. METHODS: Five consecutive patients [median age: 56 years (range: 44 ~ 60)] with ipsilateral scapula and multiple rib fractures that met the surgical indications were retrospectively reviewed. A single approach, the mirror Judet approach, was used for surgical stabilization of the scapula and targeted rib fractures. Thoracoscopic surgery was performed first for management of associated lung lesions and marking the targeted rib.
