After rotator cuff repair, stiffness--but not the loss in range of motion--increased transiently for immobilized shoulders in a rat model.

Although rotator cuff repair is often successful at relieving pain, the repaired insertion site frequently fails. Mechanical properties of the repair improved when the shoulder was immobilized in an animal model, but joint stiffness and range of motion were not evaluated. The objective of this study was to measure rotational mechanics before and after shoulders were immobilized after cuff injury and repair, not immobilized after cuff injury and repair, and immobilized without injury and repair. Humeral rotation was significantly less 4 and 8 weeks after injury and repair but did not decrease s

Ukrainian Orthopedic Trauma Experience in Managing Wartime Injuries of the Upper Extremity.

Warfare injuries of the upper extremity constitute approximately 45% of all injuries to the extremities. We review a warfare treatment algorithm for injuries to the shoulder, elbow, soft tissue, and peripheral nerves of the upper extremity. A very challenging problem is reconstruction of the elbow joint after gunshot injuries, with defects of distal humerus and proximal forearm. Warfare injuries of the peripheral nerves are part of a polystructural injury, with bone fractures and soft tissue defects. They are combined nerve injury patterns with the best results when treatment occurs between 3

Epidemiology of injuries in professional ice hockey: a prospective study over seven years.

BACKGROUND: Ice hockey injuries epidemiology is still poorly understood and very few studies are focused on it, especially about professional players. METHODS: Our prospective study collected all injuries occurring on ice during practice and games over 7 years (2006-2013) in a professional hockey team playing in the 1st division championship in Switzerland. RESULTS: During the 7 seasons, we recorded a total of 525 injuries and 190 injuries with time loss (TL). Mean injuries incidence was 5.93 (95% CI 5.28 to 6.27) injuries/1000 h/player and with time loss 2.14 (95% CI 1.79 to 2.39) injuries/

Upper Extremity Injuries in Tennis Players: Diagnosis, Treatment, and Management.

Upper extremity tennis injuries are most commonly characterized as overuse injuries to the wrist, elbow, and shoulder. The complex anatomy of these structures and their interaction with biomechanical properties of tennis strokes contributes to the diagnostic challenges. A thorough understanding of tennis kinetics, in combination with the current literature surrounding diagnostic and treatment methods, will improve clinical decision-making.

Upper extremity injuries in high school soccer goalkeepers.

BACKGROUND: Soccer participation continues to grow globally and in the United States, accompanied by an inherent risk of injury. Although most soccer injuries involve the lower extremities, goalkeepers experience disproportionately higher rates of upper extremity (UE) injuries due to position-specific demands. Although this pattern is well described at collegiate and professional levels, data among high school athletes remain limited. OBJECTIVE: To compare prevalence, characteristics, and outcomes of UE injuries between high school soccer goalkeepers and field players, as well as assess gende

Posterior shoulder dislocation: avoiding a missed diagnosis.

Posterior shoulder dislocation is a relatively uncommon event, with an incidence of 1% to 4% of all shoulder dislocations. Because of the infrequency of this condition, the diagnosis is often missed, with significant consequences to the patient Injury in the athlete is usually from a direct blow or fall onto an outstretched arm. After such an injury, symptoms may be confused with a shoulder contusion or rotator cuff injury. Significant complications such as chronic posterior dislocation and degenerative disease of the shoulder can occur if the diagnosis is missed. A careful history and physica

Estimating optimal shoulder immobilization postures following surgical repair of massive rotator cuff tears.

Although shoulder immobilization is commonly prescribed following surgical repair of rotator cuff tears there is no consensus about the best shoulder immobilization postures. A musculoskeletal model was used with a minimax algorithm to estimate optimal post-operative shoulder immobilization postures for massive rotator cuff tears involving the supraspinatus and infraspinatus. An optimal posture was defined as one in which the stresses in the repaired tendons and the angle of humerus elevation were minimized. The optimal post-operative shoulder immobilization postures were with the humerus elev

Volleyball-Related Injuries.

Volleyball is a highly popular women's sport played recreationally and competitively by a wide variety of ages. Game play requires agility, power, flexibility, and stamina to succeed. Although injury rate is lower compared to other team sports, athletes are still predisposed to acute injuries of the ankle and overuse injuries of the shoulder and knee. Injury patterns differ based on player subspecialty and playing surface. Most injuries are amenable to nonoperative treatment and require little time removed from play.

Acute traumatic posterior shoulder dislocation.

Posterior shoulder dislocation occurs rarely and is challenging to manage. The mechanisms of trauma are varied, which complicates diagnosis. Missed or delayed diagnosis and treatment can have serious deleterious effects on shoulder function. All cases of suspected posterior shoulder dislocation require a high level of suspicion and appropriate imaging. Identification of associated injuries, such as fractures and rotator cuff tears, is important to guide treatment. In the acute setting, most patients are treated with closed or open reduction with additional soft-tissue or bony procedures. Patie

Transfer of the rhomboid nerve for restoration of shoulder external rotation in partial brachial plexus palsy.

Restoration of shoulder external rotation in partial brachial plexus palsies is a real challenge. The transfer of the spinal accessory nerve to the suprascapular nerve remains the gold standard. This transfer, however, cannot be always performed. Therefore, in these cases, we previously proposed the transfer of the rhomboid nerve to the suprascapular nerve through a posterior approach. The goal of the present study is to assess this technique through a short series. Eight male patients had a partial plexus palsy. Five patients had C5, C6 root injuries, two patients had C5, C6, C7 root injuries

Neurolysis alone as the treatment for neuroma-in-continuity with more than 50% conduction in infants with upper trunk brachial plexus birth palsy.

OBJECT: The debate addressed in this article is that of surgical treatment methods for a neuroma-in-continuity. The authors of this study chose to test the hypothesis that more severe nerve injuries, as distinguished by < 50% conduction across a neuroma-in-continuity, could be treated with neuroma resection and grafting, whereas less severe nerve injuries, with > 50% conduction across the neuroma, could be treated with neurolysis alone. METHODS: The goal of this study was to compare preoperative and postoperative Active Movement Scale (AMS) scores in children with upper trunk brachial plexus

Restoration of shoulder function with nerve transfers in traumatic brachial plexus palsy patients.

Shoulder stabilization is of utmost importance in upper extremity reanimation following paralysis from devastating injuries. Although secondary procedures such as tendon and muscle transfers have been used, they never achieve a functional recovery comparable to that following successful reinnervation of the supraspinatus, deltoid, teres minor, and infraspinatus muscles. Early restoration of suprascapular and axillary nerve function through timely brachial plexus reconstruction offers a good opportunity to restore shoulder-joint stability, adequate shoulder abduction, and external rotation func

An Irreducible Posterior Fracture-Dislocation of the Shoulder: A Case Report.

Posterior fracture-dislocations of the shoulder are exceedingly rare orthopedic injuries. The management of these rare and complex injuries can be challenging from initial presentation through definitive management. Timely diagnosis of these injuries is critical to prevent devastating complications, yet the diagnosis is often delayed. Delays in surgery and poor fracture reduction are associated with a high risk of complications such as avascular necrosis. Additionally, these injuries have the potential to be irreducible. This may occur secondary to osteochondral humeral defects, soft tissue in

[Isolated coracoid fracture--open reposition and osteosynthesis--report of 3 cases].

UNLABELLED: Usually coracoid-fractures are "chain-injuries" in association with complex shoulder injuries. Isolated fractures of the coracoid process are uncommon. In the literature there is no consensus about the treatment. The conservative method is generally advocated. For better recognizing the particularities of coracoid-anatomy and fracture-analysis we performed dissections on cadaver shoulders and also evaluated a useful radiologic technique for diagnosis. Between 1995 and 1997 three patients with isolated displaced coracoid-fractures had open surgical repair. The postoperative course w…

Injuries associated with serious brachial plexus involvement in polytrauma among patients requiring surgical repair.

BACKGROUND: Brachial plexus injury occurs in up to 5% of polytrauma cases involving motorcycle crashes and in approximately 4% of severe winter sports injuries. One of the conditions for the success of operative therapy is early detection, ideally within three months of injury. The aim of this study was to evaluate associated injuries in patients with severe brachial plexus injury and determine whether there is a characteristic concomitant injury (or injuries), the presence of which, in the polytrauma, could act as a marker for nerve structures involvement and whether there are differences in
