Factors associated with poor sleep health in adults at risk for obstructive sleep apnea.

INTRODUCTION: Sleep quality is essential for health, yet few studies have assessed this issue in adults with suspected obstructive sleep apnea (OSA). This study aimed to evaluate sleep health in these adults using the previously validated Ru-SATED questionnaire during home sleep apnea testing. METHODS: A cross-sectional study was conducted with adults who completed the multidimensional Ru-SATED questionnaire, which evaluates six main dimensions of sleep health. Participants were divided into two groups based on their Ru-SATED scores: good sleep health (≥ 8 points) and poor sleep health (< 8 po

Endotypic Traits Characterizing Obesity and Sleep-related Hypoventilation in Patients with Obstructive Sleep Apnea.

Rationale: Sleep-related hypoventilation disorder (SHD) is common among obese patients with obstructive sleep apnea (OSA), but the pathological endotypes associated with obesity and SHD remain unclear. Objectives: To investigate the relationship between endotypes with body mass index (BMI) among patients with OSA and to explore endotypic traits of patients with comorbid SHD. Methods: We prospectively collected polysomnographic studies of 1,364 patients with OSA and overnight transcutaneous CO2 measurements among 420 obese patients. Endotypic traits were estimated using polysomnographic signals

Increased sympathetic responses induced by chronic obstructive sleep apnea are caused by sleep fragmentation.

Obstructive sleep apnea (OSA) is often associated with sympathetic overactivity and hypertension. These associations are mainly attributed to hypoxia acting on arterial chemoreceptors. However, the contribution of arousal from sleep is unclear. We measured the effect of OSA and sleep fragmentation on cardiovascular and sympathetic function and gene expression in the brain in rats. Male Wistar rats were fitted with a tracheal balloon and EEG and electromyogram electrodes and assigned to control (n = 6), OSA (n = 9), or arousal (n = 8) treatments. The OSA group was subjected to obstructive apnea

Propofol versus dexmedetomidine during drug-induced sleep endoscopy (DISE) for pediatric obstructive sleep apnea.

PURPOSE: To test for differences in DISE findings in children sedated with propofol versus dexmedetomidine. We hypothesized that the frequency of ≥ 50% obstruction would be higher for the propofol than dexmedetomidine group at the dynamic levels of the airway (velum, lateral walls, tongue base, and supraglottis) but not at the more static adenoid level. METHODS: A single-center retrospective review was performed on children age 1-18 years with a diagnosis of sleep disordered breathing or obstructive sleep apnea (OSA) who underwent DISE from July 2014 to Feb 2019 scored by the Chan-Parikh scal

Oxygen desaturation during night sleep affects decision-making in patients with obstructive sleep apnea.

This study assessed decision-making and its associations with executive functions and sleep-related factors in patients with obstructive sleep apnea. Thirty patients with untreated obstructive sleep apnea and 20 healthy age- and education-matched controls performed the Iowa Gambling Task, a decision-making task under initial ambiguity, as well as an extensive neuropsychological test battery. Patients, but not controls, also underwent a detailed polysomnographic assessment. Results of group analyses showed that patients performed at the same level of controls on the Iowa Gambling Task. However,

Effect of sedative-hypnotics, anesthetics and analgesics on sleep architecture in obstructive sleep apnea.

The perioperative care of obstructive sleep apnea (OSA) patients is currently receiving much attention due to an increased risk for complications. It is established that postoperative changes in sleep architecture occur and this may have pathophysiological implications for OSA patients. Upper airway muscle activity decreases during rapid eye movement sleep (REMS). Severe OSA patients exhibit exaggerated chemoreceptor-driven ventilation during non-rapid eye movement sleep (NREMS), which leads to central and obstructive apnea. This article critically reviewed the literature relevant to preoperat

Upper airway collapsibility during REM sleep in children with the obstructive sleep apnea syndrome.

STUDY OBJECTIVES: In children, most obstructive events occur during rapid eye movement (REM) sleep. We hypothesized that children with the obstructive sleep apnea syndrome (OSAS), in contrast to age-matched control subjects, would not maintain airflow in the face of an upper airway inspiratory pressure drop during REM sleep. DESIGN: During slow wave sleep (SWS) and REM sleep, we measured airflow, inspiratory time, inspiratory time/total respiratory cycle time, respiratory rate, tidal volume, and minute ventilation at a holding pressure at which flow limitation occurred and at 5 cm H2O below t

Sleep disturbances in 22q11.2 deletion syndrome: a case with obstructive and central sleep apnea.

The 22q11.2 deletion syndrome is characterized by wide phenotypic variability, frequently involving characteristic craniofacial features, cardiac malformations, and learning difficulties. Skeletal anomalies are also common and include an obtuse angle of the cranial base, retrognathia, and cervical spine abnormalities. Despite these anomalies, sleep-disturbed breathing is not reported frequently in patients with 22q11.2 deletion syndrome. We describe a patient with an obstructive sleep disturbance that was successfully treated with a tonsillectomy followed by mandibular distraction osteogenesis

Respiratory event duration reveals novel phenotypes beyond the apnea-hypopnea index in obstructive sleep apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is a heterogeneous disorder characterized by recurrent upper airway collapse during sleep. Although the apnea-hypopnea index (AHI) remains the cornerstone of OSA diagnosis and severity assessment, it primarily reflects the frequency of respiratory events and may not adequately capture differences in respiratory event duration or the associated physiological burden. We aimed to identify novel OSA phenotypes by incorporating respiratory event duration into polysomnography-based cluster analysis. METHODS: We retrospectively analyzed consecutive adult pat
