Endoscopic endonasal intracanalicular optic nerve decompression: how I do it.

BACKGROUND: The endoscopic endonasal approach for optic nerve decompression is suited for the management of non-traumatic optic neuropathy but remains underreported, presumably due to transcranial approaches still being favoured at individual centres. METHOD: The optic canal is approached endoscopically and transsphenoidally through the contralateral nostril. Its inferomedial wall is opened using an irrigated diamond drill, and neuronavigation is used to confirm anatomical bearings. CONCLUSION: This technique provides rapid and easy access to the inferomedial aspect of the optic canal and ne

Pterional Craniotomy for Resection of Clinoidal Meningioma Associated With an Aerated Anterior Clinoid Process: 2-Dimensional Operative Video.

Extradural removal of the clinoid performed prior to resection of clinoidal meningiomas has been advocated as a way to facilitate devascularization of the tumor and provide early identification and/or decompression of adjacent neurovascular structures. A small number of video publications exist in the literature that provides useful guidance to surgeons preparing for resection of clinoidal region meningiomas.1-3 However, none of these videos portray the variable anatomy associated with an aerated clinoid process. This known anatomical variant can increase the risk profile associated with resec

Triangulating the ledge: radiographic study of the floor of orbit and derivation of a novel template.

In orbital floor reconstruction, the need for the orbital implant to reach the exact position of the posteromedial ledge is essential, but owing to the complex anatomy of the region, visualisation of the ledge may be difficult. Several morphometric studies, both radiographic and cadaveric, have calculated a mean length from the orbital rim to the ledge. However, those linear measurements are unreliable and possess a higher margin of error for intraoperative guidance. This study attempts to triangulate the position of the posterior ledge from three easily accessible and reproducible points on t

Diagnostic usefulness of panoramic imaging of the pterygopalatine fossa: case of a schwannoma causing pterygopalatine fossa expansion.

The pterygopalatine fossa is a small area between the posterior wall of the maxillary sinus and the anterior surface of the pterygoid process of the sphenoid bone. The pterygopalatine fossa can be seen clearly on panoramic imaging. We present the case of a 57-year-old man who exhibited right pterygopalatine fossa expansion on panoramic imaging. Computed tomography (CT), magnetic resonance imaging (MRI), and panoramic imaging all showed a tumor at the right pterygopalatine fossa in this patient. CT indicated that the tumor replaced right retromaxillary fat and displaced the posterior wall of th…

Endoscopic contralateral transmaxillary approach for pterygoid process osteotomy in total maxillectomy: A technical case report.

An approach for total maxillectomy with endoscopic transection of the pterygoid process via the contralateral maxillary sinus is described. In total maxillectomy, the resection of the pterygoid process of the sphenoid is a key step for successful resection. However, a conventional craniofacial approach requires extensive incision in the face, elevation of the lateral cheek flap. Even after elevation of the lateral cheek flap, visualization of this region is not good. An endoscopic approach through the contralateral maxillary sinus improved visualization of the pterygoid process, and osteotomy

Extracapsular dissection technique with the cotton swab for pituitary adenomas through an endoscopic endonasal approach -- how I do it.

BACKGROUND: Pituitary adenomas are often encased in a histological pseudocapsule that separates the tumor from the normal gland. Transsphenoidal adenoma resection may be performed either in an intra- or an extracapsular technique. The extracapsular fashion offers anatomical orientation, removal of a security margin, reduced risk of opening the arachnoid layer with subsequent CSF flow and identification of invasion. METHOD: The sella turcica is approached through the classic endoscopic endonasal route. After opening the dura of the sellar floor, the interface between the compressed tissue and

Absence of a sphenoid wing in neurofibromatosis type 1 disease: imaging with multidetector computed tomography.

Neurofibromatosis type 1 disease is characterized by pigmented cutaneous lesions and generalized tumors of a neural crest origin and it may affect all the systems of the human body. Sphenoid dysplasia is one of the characteristics of this syndrome and it occurs in 5-10% of the cases; further, abnormalities of the sphenoid wings are often considered pathognomonic. However, complete agenesis of a sphenoid wing is very rare. We report here on an unusual case of neurofibromatosis type 1 disease with the associated absence of a sphenoid wing that was diagnosed by using multidetector computed tomogr

Solitary plasmacytoma of the greater sphenoid wing with secondary submandibular soft tissue metastasis.

We report the follow-up of a case of solitary plasmacytoma of bone in the posterolateral orbital wall previously published in this journal. The patient presented three years after initial diagnosis and treatment with a submandibular soft tissue metastasis, probably in a lymph node. Reports of lymph node metastases from solitary plasmacytoma of bone are rare.

Blunt trauma leading to facial nerve paralysis.

A case in a young child of blunt trauma to the facial nerve, distal to the stylomastoid foramen, is reported. Although the patient made an uneventful recovery, an understanding of the possible sites of injury of the facial nerve and further investigations available to aid diagnosis is important. This child was followed up in the Accident and Emergency Department, however, early referral of this rare condition to an otolaryngologist is advisable.

Cushing's disease in childhood: benign intracranial hypertension after trans-sphenoidal adenomectomy. Case report.

A 7-year-old girl presented with the physical and endocrinological stigmata of Cushing's disease. An adrenocorticotropic hormone (ACTH)-producing pituitary microadenoma was excised. Three weeks after trans-sphenoidal adenomectomy, the patient developed benign intracranial hypertension. Although ACTH levels had decreased to normal, the serum cortisol had fallen to subnormal levels. The child responded to exogenous steroid therapy, which was gradually tapered and discontinued after 5 months. Normal pituitary and adrenal functions persist 2 years later.

Transsphenoidal meningocele with hypothalamic insufficiency.

A 16-year-old girl with transsphenoidal meningocele and signs of hypothalamic insufficiency is presented. Hormonal disturbances have been reported in only three similar cases in the literature. The radiological criteria for the diagnosis are discussed with emphasis on the differential diagnosis of a persistent cranipharyngeal canal. Based on the present case and studies of the pertinent literature we propose that all patients exhibiting signs of dysraphism of the base of the skull should be thoroughly investigated for hormonal disturbances. Children with cleft palate should be examined for pos

HDAC inhibitor AR-42 decreases CD44 expression and sensitizes myeloma cells to lenalidomide.

BACKGROUND: Nasal myiasis, an infestation by fly larvae, is a rare condition typically associated with immunocompromised individuals, poor hygiene, and low socioeconomic status. It is commonly seen in tropical regions and is often linked to chronic sinonasal diseases or underlying health conditions. However, cases in healthy individuals without predisposing factors are uncommon, making this case novel and worthy of documentation. The importance of timely diagnosis and effective management in preventing severe complications cannot be overstated. CASE PRESENTATION: A 40-year-old male presented

Indications, Techniques and Complications Associated With Pterygoid Implants: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: Pterygoid implants represent a graftless option for rehabilitation of patients with posterior maxillary atrophy, engaging the dense cortical bone of the pterygomaxillary complex. Despite clinical growth, a comprehensive synthesis of indications, surgical techniques, and complications is lacking. This review evaluated the prevalence of complications, implant survival rates, and marginal bone loss (MBL) associated with pterygoid implant placement. METHODS: The reporting of this review follows PRISMA 2020 guidelines. Six electronic databases (PubMed, Ovid, Scopus, WoS,

Time and pattern of the fusion of the spheno-occipital synchondrosis in patients with skeletal Class I and Class III malocclusion.

OBJECTIVES: To investigate the time and pattern of fusion of the spheno-occipital synchondrosis in patients with skeletal Class I and Class III malocclusion using cone-beam computed tomography (CBCT). MATERIALS AND METHODS: A total of 262 CBCT images were collected: 140 skeletal Class I (0° < ANB < 4°; 71 males, 69 females) and 122 skeletal Class III (ANB ≤ 0°; 61 males and 61 females). The fusion stages were identified using CBCT images of a six-stage system defined by the appearance of osseous cores and ossifying vestige in the synchondrosis. The age distributions of each stage and the patt

Isolated Unilateral Frontosphenoidal Suture Synostosis in Six Patients: Lessons Learned in Diagnosis and Treatment.

INTRODUCTION: Due to the rarity of isolated frontosphenoidal suture synostosis clinical diagnosis can be challenging. This study of 6 patients aims to review the clinical, radiological findings, and operative techniques used to correct the underlying pathology. METHODS: Patients with isolated frontosphenoidal suture craniosynostosis were selected from a retrospective review of 88 patients with unicoronal synostosis treated during a 3-year period. Two-dimensional photography of patients' soft tissue morphology from the vertex view allowed assessment of the following morphology: frontal bossing

Influence of frontosphenoidal suture synostosis on skull dysmorphology in unicoronal suture synostosis.

Severity of the Harlequin deformity seen in unicoronal synostosis may be augmented when frontoparietal suture synostosis has an associated fusion of the frontosphenoidal suture or in cases of isolated frontosphenoidal synostosis. The purpose of the current study is to characterize various suture fusion patterns along the coronal ring using a modified orbital index (MOI), orbital angle (OA), and endocranial base (EB) angle. This study is a retrospective single institution cohort study. Charts were reviewed over the past 12 years; patients with isolated UCS were included. MOI, OA, and EB were use

Duplication of the abducens nerve at the petroclival region: an anatomic study.

OBJECTIVE: During its course between the brainstem and the lateral rectus muscle, the abducens nerve usually travels forward as a single trunk, but it is not uncommon for the nerve to split into two branches. The objective of this study was to establish the incidence and the clinical importance of the duplication of the nerve. METHODS: The study was performed on 100 sides of 50 autopsy materials. In 10 of 11 cases of duplicated abducens nerve, colored latex was injected into the common carotid arteries and the internal jugular veins. The remaining case was used for histological examination.

[An intracranial tumor in an unusual location associated with Cushing's syndrome].

In a 58 year-old woman with peracute Cushing's syndrome high levels of ACTH were proved by RIA method. Autopsy showed normal pituitary gland in addition to a voluminous tumour situated in planum sphenoidale and lamina cribrosa ossis ethmoidalis. Tumour cells with epithelial features produced ACTH which was found by an indirect immunoperoxidase technique. Tumour was classified as ectopic corticotropic pituitary adenoma.
