Analysis of the risk factors associated with endoscopic sphincterotomy techniques: preliminary results of a prospective study, with emphasis on the reduced risk of acute pancreatitis with low-dose anticoagulation treatment.

BACKGROUND AND STUDY AIMS: The aim of the present study was to analyze the risk factors associated with complications of endoscopic sphincterotomy (ES). PATIENTS AND METHODS: In all consecutive endoscopic sphincterotomies carried out between September 1994 and December 1996, the possible risk factors (12 patient-related factors and 12 procedure-related ones), as well as the concomitant medical treatment, indications, techniques, and success of endoscopic sphincterotomy were evaluated prospectively. Risk factors were analyzed on an exploratory basis using univariate methods. "Potential risk fa…

Comparison of 12- and 15-mm endoscopic papillary large balloon dilation combined with a limited endoscopic sphincterotomy for large bile duct stones: A propensity score-matched analysis.

BACKGROUND AND STUDY AIMS: Endoscopic papillary large balloon dilation (EPLBD) combined with endoscopic sphincterotomy (EST) has been increasingly used to remove large common bile duct (CBD) stones. However, the clinical outcomes of stone extraction have rarely been investigated in regard to the size of the balloon. The aim of this study was to assess the short- and long-term outcomes of 12- and 15-mm EPLBD with a limited EST for large CBD stones. PATIENTS AND METHODS: Patients without prior endoscopic retrograde cholangiopancreatography (ERCP) who received 12- or 15-mm EPLBD in combination w…

Lateral Internal sphincterotomy is still crucial to heal hypertonic chronic anal fissure and normalize the internal anal sphincter tone? Is V-en Y plastic a valuable alternative?

INTRODUCTION: The role of augmented internal anal sphincter (IAS) tone in the genesis of chronic anal fissure (CAF) is still unclear. Lateral internal sphincterotomy (LIS) is the most employed surgical procedure, aiming to reduce the IAS tone leaving a permanent anatomical alteration and it is burdened by high risk post-operative anal incontinence (AI). The aim of this work was to evaluate if the pre-operative manometric alterations of CAFs with hypertonic IAS would normalize after sphincter preserving surgical procedure. METHODS: We enrolled 108 consecutive patients affected by idiopathic an…

Does rectal indomethacin given for prevention of post-ERCP pancreatitis increase bleeding after biliary endoscopic sphincterotomy or cardiovascular mortality?: Post hoc analysis using prospective clinical trial data.

Rectal indomethacin has been proven to be effective for prevention of post-ERCP pancreatitis (PEP) but its impact on bleeding after biliary sphincterotomy (BABES) and cardiovascular mortality has not been extensively studied. We aimed to assess the effect of indomethacin on the rate of BABES and short-term cardiovascular mortality, particularly in patients receiving antiplatelet therapy (APT).In this double-blind, randomized, placebo-controlled, single-center study, 100 mg indomethacin or placebo was given within 1 hour before biliary endoscopic sphincterotomy to in-patients including those wh…

Comparison outcomes between laparoscopic choledocholithotripsy and laparoscopic cholecystectomy and preoperative endoscopic retrograde cholangiopancreatography with sphincterotomy and laparoscopic cholecystectomy in patients with choledocholithiasis and cholelithiasis: an up-to-date meta-analysis.

BACKGROUND: Laparoscopic choledocholithotripsy (or laparoscopic common bile duct exploration [LCBDE]) with laparoscopic cholecystectomy (LC) and preoperative endoscopic sphincterotomy (EST) followed by LC are established treatment strategies for concomitant choledocholithiasis and cholelithiasis. However, the comparative efficacy, safety, and cost-effectiveness of these 1-stage vs 2-stage approaches remain uncertain, particularly with recent advances in minimally invasive interventions. METHODS: A systematic search of MEDLINE, Embase, and the Cochrane Central Register of Controlled Trials was…

Comparative Success Rate and Adverse Effects of Endoscopic Sphincterotomy Versus Endoscopic Papillary Large Balloon Dilation in Large Common Bile Duct Stones Removal. A Propensity Scores Inverse Weighting Analysis.

BACKGROUND/AIMS: Endoscopic sphincterotomy (EST) is a standard treatment for common bile duct (CBD) stones. Endoscopic sphincterotomy combined with endoscopic papillary large balloon dilation (EST-EPLBD) is an effective treatment for difficult CBD stones. This study aims to evaluate and compare the effectiveness and adverse effects of EST-EPLBD and EST in treating large CBD stones. METHODS: We retrospectively analyzed the data of 85 patients with large CBD stones who underwent either EST or EST-EPLBD, resulting in successful CBD stone extraction from January 2018 to June 2022. Propensity scor…

Recurrent Symptomatic Biliary Obstruction due to Duodenobiliary Reflux of Food: A Largely Unknown Complication of Endoscopic Biliary Sphincterotomy: A single-center case series including the results of surgical therapy.

Background and Study Aims Biliary obstruction caused by food is a little known and rarely reported long-term complication of endoscopic sphincterotomy (ES). This retrospective study aimed to characterize patients diagnosed with this condition between 2012 and 2025 and to assess the outcomes of surgical therapy. Patients and Methods Patients were selected who were diagnosed with symptomatic duodenobiliary reflux of food (DBR-F) after earlier ES and who all underwent hepaticojejunostomy with partial resection or ligation of the common bile duct (CBD). DBR-F was diagnosed by pathologic examinatio…

[Internal lateral sphincterectomy. Results].

In January 1986 we initiated a prospective study in our center to evaluate the results of surgery in patients with anal fissure. Three groups were considered according to type of surgery: internal lateral sphincterotomy (ELI), lateral sphincterotomy and resection of cutaneous fibroma (ELI + FC), and sphincterotomy with hemorrhoidectomy (E + H). The basic objective was to evaluate postoperative pain, days of hospitalization and out-patient follow-up, complications and number of recurrences. Postoperative pain occurred in 12% of internal lateral sphincterotomies, in 42% of lateral sphincterotomi…

[Endoscopic papillotomy in benign papillary stenosis].

Since 1977 endoscopic sphincterotomy has been indicated in 18 patients because of benign papillary stenosis. In 17 patients cholecystectomy had been performed some time before. The sphincterotomy was successful in 15 patients. In one patient slight bleeding occurred. In 2 of 3 patients in whom sphincterotomy did not succeed, the endoscopic procedure was complicated by cholangitis and pancreatitis respectively. 12 of the 15 patients in whom sphincterotomy was successful were symptom-free over a median period of 21 months. Therefore, endoscopic sphincterotomy appears to be an effective treatment…

A comparative study in anal fissure treatment.

275 patients with chronic anal fissure were treated by anal stretch, posterior internal sphincterotomy or lateral subcutaneous internal sphincterotomy. All operations were performed under general anesthesia. There was a six-month follow-up period where 23% of the patients who underwent anal stretch developed incontinence to flatus or faeces. Minor degrees of incontinence occurred in 8% of the posterior sphincterotomy group and 4% of the lateral subcutaneous sphincterotomy group. There was a 7% recurrence rate in the anal stretch group, 4% in the posterior sphincterotomy group, and 3% in the la…

Lateral internal sphincterotomy is superior to topical nitroglycerin for healing chronic anal fissure and does not compromise long-term fecal continence: six-year follow-up of a multicenter, randomized, controlled trial.

PURPOSE: Although there is enthusiasm for nonoperative management of anal fissures, most trials have been of short duration (6-8 weeks) and long-term outcome is unknown. The purpose of this study was to assess long-term outcome in two cohorts of patients who had participated in a randomized, controlled trial to compare the effectiveness of topical nitroglycerin with internal sphincterotomy in the treatment of chronic anal fissure. METHODS: Between February 1997 and October 1998, 82 patients with chronic anal fissure were accrued and randomized to 0.25 percent nitroglycerin ointment t.i.d. or …

Operative procedures for fissure in ano.

BACKGROUND: Operative techniques commonly used for fissure in ano include: anal stretch, open lateral sphincterotomy, closed lateral sphincterotomy, posterior midline sphincterotomy and to a lesser extent dermal flap coverage of the fissure. Reports of direct comparisons between operative techniques for anal fissure are variable in their results. These reports are either subject to selection bias (in non-randomized studies) or observer bias (in almost all studies) or have inadequate numbers of patients enrolled to answer the question of efficacy. OBJECTIVES: To determine the best technique fo…

Endoscopic treatment of biliary-tract diseases. An international study.

Data on endoscopic sphincterotomy from 15 gastroenterology centres with very wide experience show that 3618 out of 3853 (93.%) attempts at the procedure were successful. The main indication for sphincterotomy was choledocholithiasis (3070, or 84.9% cases). After sphincterotomy the stones passed spontaneously or were removed in 2779 (90.5%) cases. Bleeding, cholangitis, pancreatitis, perforation, and stone impaction occurred in 254 (7.0%) cases; the mortality-rate was 1.4%. 83 (2.3%) cases required emergency surgery. Endoscopic sphincterotomy is increasingly replacing surgery in the treatment o…

Anal stretch plus fissurectomy for chronic anal fissure.

INTRODUCTION: Anal fissure is a common disease. Usually chronic anal fissures are managed medically. When conservative management fails, surgical treatment should be considered. Lateral internal sphincterotomy has been advocated as the first choice invasive treatment but it has a reported rate of major fecal incontinence of 5%. In order to reduce the onset of major fecal incontinence after anal fissure surgery, it has been proposed to use the anal stretching plus fissurectomy. METHODS: From 2008 to 2011, 457 patients have been operated for chronic anal fissure. RESULTS: Twenty-seven patients…

Surgical treatment of chronic anal fissure. A retrospective study of 1753 cases.

This is a retrospective study of 1753 cases of chronic anal fissures treated by five varying methods over a five-year period from January 1976 to December 1980. Results showed that the incidence of recurrent fissures was higher in those treated by anal fissurectomy with sphincterotomy. There was also a significant difference in operative time, length of hospital stay, patient discomfort, and incidence of urinary retention among these operative methods. Generally, lateral anal sphincterotomy and multiple anal sphincterotomies showed a lesser incidence of these factors. A simpler procedure, such…
