Combined Total Ankle Arthroplasty With Posterior Tibial Tendon Transfer for End-Stage Cavovarus Deformity.

Posterior tibial tendon transfer has been described to reduce and balance the cavovarus deformity in those patients who receive a total ankle replacement for end-stage arthritis. In this article, we discuss the indications and provide a detailed description of the technique for this powerful procedure. Case examples that demonstrate the utility of the procedure are provided.

Hereditary neuropathy with liability to pressure palsies with recurrent facial paralysis as main clinical manifestation in a Chinese patient: A case report.

RATIONALE: Hereditary neuropathy with liability to pressure palsies (HNPP) is a rare autosomal dominant genetic disorder characterized by re … years. After hospitalization, we found that the patient had weakened tendon reflex of the limbs and talipes cavus, which highly suggests chronic peripheral neuropathy. DIAGNOSES: The electromyography examination was performed. Motor nerve conduction detection found conduction block of the right common peroneal nerve at the fibular head; sensory nerve con …

When Does Brace Noncompliance Occur in Patients with Clubfoot Deformity?

BACKGROUND: Noncompliance with bracing and its protocol is the most common cause of clubfoot deformity relapse after correction. The literature does not clarify when patient noncompliance with the brace occurs-during which period of the bracing phase, or in which age group. This study aimed to determine when noncompliance is most likely to occur. METHODS: This descriptive observational study included children with idiopathic talipes equinovarus deformity for whom the foot abduction brace was applied to their feet after complete correction via the Ponseti method. Each patient was observed unti…

STAC3 disorder: a common cause of congenital hypotonia in Southern African patients.

STAC3 disorder, or Native American myopathy, is characterised by congenital myopathy, hypotonia, musculoskeletal and palatal anomalies, and … four patients. Of the clinical group, 93% had a palatal abnormality, 52% a spinal anomaly, 59% had talipes equinovarus deformity/deformities, 38% had arthrogryposis multiplex congenita, and 22% had a history suggestive of malignant hyperthermia. The novel finding that STAC3 disorder is a common African myopathy has important clinical implications for the …

Factors Related to Early Recurrence of Idiopathic Clubfoot Post the Ponseti Method.

Introduction: Idiopathic clubfoot or congenital talipes equinovarus (CTEV) is managed by the Ponseti method worldwide; however, the recurrence of the deformity is a challenging problem. The purpose was to review the factors associated with early recurrence of CTEV post the Ponseti method. Materials and Methods: During 2011-2016, 34 infants with 52 CTEV, who underwent the Ponseti method and a minimum follow-up period of six months, were reviewed. Twenty-two infants (65%) were male, and 18 infants (53%) had bilateral CTEV. Recurrence of CTEV was defined as a reappearance of at least one of the f…

What is a good result after clubfoot treatment? A Delphi-based consensus on success by regional clubfoot trainers from across Africa.

BACKGROUND: Congenital talipes equino-varus (CTEV), also known as clubfoot, is one of the most common congenital musculoskeletal malformations. Despite this, considerable variation exists in the measurement of deformity correction and outcome evaluation. This study aims to determine the criteria for successful clubfoot correction using the Ponseti technique in low resource settings through Africa. METHODS: Using the Delphi method, 18 experienced clubfoot practitioners and trainers from ten countries in Africa ranked the importance of 22 criteria to define an 'acceptable or good clubfoot corre…

Flexor digitorum accessorius longus muscle in resistant clubfoot patients: introduction of a new sign predicting its presence.

Clubfoot, talipes equino varus (TEV), is a common congenital foot anomaly. Some cases are resistant to conservative treatment. Many causes of resistance have been reported, among these, the presence of anomalous muscles; however, the effect of the presence of anomalous muscles on the outcome of conservative management is not well studied. These aberrant muscles are discovered during the extensive surgical release as an abnormal finding. The aim of this work is to study the demographic characteristics of patients with resistant TEV that necessitated extensive soft tissue release at Sudan Clubfo…

[Late diagnosis of congenital argininemia during administration of sodium valproate].

Congenital hyperargininaemia is a rare condition transmitted as an autosomal dominant trait. Following a one-year free interval, repeated vomiting, psychomotor regression and spastic paraparesis with talipes equinus progressively develop. The diagnosis, confirmed by arginine assays in blood and urine, is probably often missed. We report a case of homozygous arginase deficiency belatedly diagnosed at the age of 18 years, when treatment with sodium valproate (VPA) was instituted. This female patient presented with psychomotor regression since the age of 15 months and with paraparesis since she w…

Assessment of Gross Motor Skills in Ponseti-treated Children With Idiopathic Clubfoot at 3 Years Old: Insights from the Peabody Developmental Motor Scale 2.

BACKGROUND: Congenital talipes equinovarus, or clubfoot, can lead to lifelong functional impairments, including diminished gross motor skills (GMS), if left untreated. The Ponseti method corrects idiopathic clubfoot through casting and bracing. Given the importance of GMS in childhood development, this technique must be optimized to support childhood and long-term health outcomes. This study examined immediate posttreatment GMS in 3-year-old children treated with Ponseti, hypothesizing that they would perform on par with their nonclubfoot peers. METHODS: Data from 45 children (33 to 46 mo of …

Functional Outcomes of Surgical Treatment for Giant Cell Tumors around the Knee.

INTRODUCTION: Congenital talipes equinovarus (CTEV), or clubfoot, is a common congenital deformity treated effectively using the Ponseti method. However, conventional cast removal techniques, such as oscillating saws or water soaking, can be uncomfortable, time-consuming, and impractical in resource-limited settings. This study evaluated a novel modification involving the interposition of a sterile polythene sheet between layers of plaster of Paris to facilitate easier, safer, and faster cast removal. MATERIALS AND METHODS: In this prospective observational study conducted from December 2024 …

Finite Element Simulation of Clubfoot Correction: A Feasibility Study Toward Patient-Specific Casting.

Background: Congenital talipes equinovarus (clubfoot) affects 1-2 per 1000 newborns worldwide. The Ponseti method, based on staged manipulations and casting, is the gold standard for correction. However, the biomechanical processes underlying these corrections remain poorly understood, as infants rarely undergo imaging. Computational modeling may offer a non-invasive approach to studying correction pathways and exploring novel applications, such as customized casts. Methods: We developed a proof-of-concept framework using iterative finite element analysis (iFEA) to approximate the surface-leve…
