[Present role and future prospect of superselective intra-arterial infusion chemotherapy for head and neck cancer].

Patients with head and neck squamous cell cancers (HNSC) of stage III and IV have a poor outcome, often losing important functions such as swallowing and speech despite combined surgery and radiotherapy. Two-thirds of such patients die of local recurrence and disseminated metastasis in spite of salvage therapy. Conventional adjuvant chemotherapy is often difficult for HNSC patients, since many of them suffer from associated chronic oral, respiratory and gastrointestinal diseases due to tobacco and alcohol in addition to having poor nutritional and oral hygienic conditions. Ninety-seven patient

Sudden termination of G-CSF injection leads to apoptosis of a large proportion of increased granulocytes.

The effectiveness of granulocyte colony-stimulating factor (G-CSF) for granulocytopenia has been widely recognized. However, although granulocyte counts rapidly increase after a few injection of G-CSF, a large proportion of the increased granulocytes disappear from peripheral blood within a few days following G-CSF withdrawal. Where do they go? In this report, the answer can be seen at a glance. Using MitoCapture and a CCD camera-equipped fluorescence microscope, we succeeded in demonstrating that G-CSF withdrawal induced loss of mitochondrial membrane potential, i.e., an early stage of apopto

Assessment of amifostine as protection from chemotherapy-induced toxicities after conventional-dose and high-dose chemotherapy in patients with germ cell tumor.

BACKGROUND: We assessed the efficacy of amifostine for protection from chemotherapy-induced toxicities in patients treated with conventional-dose paclitaxel, ifosfamide, cisplatin (TIP) and high-dose carboplatin, etoposide and thiotepa (CET) followed by peripheral blood progenitor cell (PBPC) rescue. PATIENTS AND METHODS: In a prospective single-center study 40 patients with relapsed or refractory germ-cell tumors (GCT) were treated with 3 cycles of conventional-dose TIP followed by one cycle of high-dose CET. Patients were randomized either to receive one fixed dose of 500 mg amifostine per

Impact of Prior Treatments on Clinical Outcomes in Patients With Metastatic Castration-Resistant Prostate Cancer: A Real-World Analysis in the United States.

INTRODUCTION: Treatment intensification (combining androgen deprivation therapy [ADT] with an androgen receptor pathway inhibitor [ARPi]) and/or chemotherapy has demonstrated benefit over ADT-only treatment in prostate cancer settings before metastatic castration-resistant prostate cancer (mCRPC). This real-world analysis assessed the rates of treatment intensification before the mCRPC setting, first-line (1L) treatment patterns in mCRPC, and clinical outcomes in patients with mCRPC in the United States. PATIENTS AND METHODS: This noninterventional, retrospective cohort study utilized the Fla

[68Ga]Ga-PSMA-11 PET Tumor Volume Predicts Overall Survival of Patients with Metastatic Prostate Cancer Undergoing Taxane-Based Chemotherapy.

Prostate-specific membrane antigen (PSMA) PET has the potential to monitor the response to taxane-based chemotherapy in patients with prostate cancer and shows promise for predicting outcomes and improving response evaluation. This retrospective study aimed to determine the prognostic value of [68Ga]Ga-PSMA-11 PET (PSMA PET)-derived quantitative tumor burden parameters for overall survival (OS). Methods: Databases from 6 institutions were screened for patients with prostate cancer who underwent PSMA PET and whose serum prostate-specific antigen (PSA) measurements were recorded at baseline and

Survival with Olaparib in Metastatic Castration-Resistant Prostate Cancer.

BACKGROUND: We previously reported that olaparib led to significantly longer imaging-based progression-free survival than the physician's choice of enzalutamide or abiraterone among men with metastatic castration-resistant prostate cancer who had qualifying alterations in homologous recombination repair genes and whose disease had progressed during previous treatment with a next-generation hormonal agent. The results of the final analysis of overall survival have not yet been reported. METHODS: In an open-label, phase 3 trial, we randomly assigned patients in a 2:1 ratio to receive olaparib (

A pilot study evaluating chemotherapy tolerability for breast cancer patients who have received prior treatment and chest radiation for Hodgkin Lymphoma.

MICROABSTRACT: Women treated with chest radiation for Hodgkin lymphoma (HL) have significantly higher risk of developing breast cancer, and little is known about how these patients tolerate chemotherapy for breast cancer. This small retrospective study identified 15 patients, noting that these patients tolerate proposed chemotherapy regimens for breast cancer in rates similar to those without prior HL and therapeutic radiation. PURPOSE: Women treated for Hodgkin lymphoma (HL) with chest radiation have significantly higher risk of developing breast cancer, and little is known about how these p

Therapeutic Potential of Docetaxel plus Cisplatin Chemotherapy for Myasthenia Gravis Patients with Metastatic Thymoma.

The prognosis of myasthenia gravis (MG) in association with invasive or metastatic thymoma is usually worse, and therapeutic options are quite limited. Here, we retrospectively reported the therapeutic effect of docetaxel plus cisplatin (docetaxel/cisplatin) chemotherapy in 7 MG patients with metastatic thymoma. Previously, all patients underwent thymectomy at the first onset of thymoma. After the metastasis of thymoma, none of the patients received thymectomy due to unresectable conditions after surgeon's evaluation for great risk of myasthenic crisis (n = 5) or patients' refusal (n = 2). Maximizing the Clinical Benefit of Anthracyclines in Addition to Taxanes in the Adjuvant Treatment of Early Breast Cancer.

The Oncology Grand Rounds series is designed to place original reports published in the Journal into clinical context. A case presentation is followed by a description of diagnostic and management challenges, a review of the relevant literature, and a summary of the authors' suggested management approaches. The goal of this series is to help readers better understand how to apply the results of key studies, including those published in Journal of Clinical Oncology, to patients seen in their own clinical practice. A healthy 56-year-old postmenopausal woman discovered a palpable mass at the one

Phase II trial of utidelone as monotherapy or in combination with capecitabine in heavily pretreated metastatic breast cancer patients.

BACKGROUND: The treatment of metastatic breast cancer (MBC) remains a great clinical challenge as drug resistance frequently develops. Alternative agents that can overcome drug resistance would offer new therapeutic options. The primary aim of this phase II study was to evaluate the efficacy and safety of utidelone as a monotherapy or in combination with capecitabine in metastatic breast cancer patients previously treated with and resistant to anthracyclines and taxanes. METHODS: In two open-label, noncomparative clinical studies, patients with metastatic breast cancer who previously received
