Agnosia for familiar faces and odors in a patient with right temporal lobe dysfunction.

The authors studied a 53-year-old man with progressive prosopagnosia and inability to recognize his favorite foods by smell. He could not identify pictures of familiar faces, but he could match unfamiliar faces and distinguish them from familiar ones. He could not identify familiar odors, but he could detect them and could perceive them as pleasant or familiar. Neuroimaging revealed temporal lobe changes, especially on the right. Right temporal lesions may produce face and odor agnosia by preventing perceptual familiarity units from accessing semantic associations.

Amygdala enlargement in dysthymia--a volumetric study of patients with temporal lobe epilepsy.

BACKGROUND: Previous studies indicated an important role of the amygdala for emotional information processing. We investigated a possible relationship between amygdala volumes, aggressive behavior, and dysthymia, in patients with temporal lobe epilepsy (TLE). METHODS: Patients with TLE with and without aggression or dysthymia and healthy volunteers were assessed using quantitative MRI. Amygdala volumes were measured in a blinded fashion and corrected for total brain volumes. RESULTS: There was a highly significant enlargement of left and right amygdala volumes in patients with dysthymia (rig…

Embolization of arteriovenous malformations of the temporal lobe via the anterior choroidal artery.

The indications, technique, and results of embolization of arteriovenous malformations with liquid adhesive agents delivered through the anterior choroidal artery are reported. Arteriovenous malformations of the temporal lobe were found in four patients with intracerebral bleeding and two with intractable epilepsy. In five of the six, the dominant arterial feeder was the anterior choroidal artery. All patients underwent superselective catheterization of the anterior choroidal artery and embolization of the arteriovenous malformation. Complications related to the anterior choroidal artery embol…

Ventricular diverticula with localized dysgenesis of the temporal lobe in cloverleaf skull anomaly.

An early fetal case of cloverleaf skull anomaly associated with thanatophoric chondrodystrophy is described. Localized ventricular diverticula are a peculiar finding. In addition there are previously described malformations limited to the temporal lobes and comprising plump gyri with abnormal deep sulci, thick periventricular heterotopias, dysgenesis of the parahippocampal gyrus, agenesis of the Ammon's horn, and small micropolygyria. The skull deformity is not dependent on hydrocephalus, demonstrating inconsistencies in previous pathogenetic concepts.

Brain structural differences in temporal lobe and frontal lobe epilepsy patients: A voxel-based morphometry and vertex-based surface analysis.

PURPOSE: Exploration of the effect of chronic recurrent seizures in focal epilepsy on brain volumes has produced many conflicting reports. To determine differences in brain structure in temporal lobe epilepsy (TLE) and extratemporal epilepsy (using frontal lobe epilepsy (FLE) a surrogate) further, we performed a retrospective analysis of a large cohort of patients with seizure-onset zone proven by intracranial monitoring. METHODS: A total of 120 TLE patients, 86 FLE patients, and 54 healthy controls were enrolled in this study. An analysis of variance of voxel-based morphometry (VBM) was used…

Comparison of selective amygdalohippocampectomy and anterior temporal lobectomy for mesial temporal lobe epilepsy: a systematic review and meta-analysis.

BACKGROUND: Anterior temporal lobectomy (ATL) and selective amygdalohippocampectomy (SAH) are the two most widely adopted surgical approaches for drug-resistant mesial temporal lobe epilepsy (MTLE). Although both procedures target seizure control, they differ in extent of resection and potential morbidity. The relative effectiveness of ATL and SAH remains debated, particularly regarding seizure freedom, visual field deficits, and postoperative complications. OBJECTIVE: To systematically compare seizure freedom (Engel I) rates, Engel III-IV outcomes, visual field deficits, and postoperative co…

Resection of the temporal piriform cortex in mesial temporal lobe epilepsy: Association with seizure freedom in a retrospective cohort study.

OBJECTIVE: This study evaluated the association between temporal piriform cortex (tPC) resection and long-term postoperative outcomes in temporal lobe epilepsy (TLE). METHODS: We conducted a retrospective analysis of patients with TLE undergoing surgery between 2012 and 2022, with a minimum follow-up of 2 years. The extent of resection, including the tPC, hippocampal head/body, and amygdala, was assessed using magnetic resonance imaging scans. These measurements, along with demographic data and other presurgical evaluations, were analyzed for their associations with Engel class postoperative …

The decreased connectivity in middle temporal gyrus can be used as a potential neuroimaging biomarker for left temporal lobe epilepsy.

OBJECTIVE: We aimed to explore voxel-mirrored homotopic connectivity (VMHC) abnormalities between the two brain hemispheres in left temporal lobe epilepsy (lTLE) patients and to determine whether these alterations could be leveraged to guide lTLE diagnosis. MATERIALS AND METHODS: Fifty-eight lTLE patients and sixty healthy controls (HCs) matched in age, sex, and education level were recruited to receive resting state functional magnetic resonance imaging (rs-fMRI) scan. Then VHMC analyses of bilateral brain regions were conducted based on the results of these rs-fMRI scans. The resultant imag…

Exploring the role of the posterior middle temporal gyrus in semantic cognition: Integration of anterior temporal lobe with executive processes.

Making sense of the world around us depends upon selectively retrieving information relevant to our current goal or context. However, it is unclear whether selective semantic retrieval relies exclusively on general control mechanisms recruited in demanding non-semantic tasks, or instead on systems specialised for the control of meaning. One hypothesis is that the left posterior middle temporal gyrus (pMTG) is important in the controlled retrieval of semantic (not non-semantic) information; however this view remains controversial since a parallel literature links this site to event and relation

A case of probable korsakoff's syndrome: a syndrome of frontal lobe and diencephalic structural pathogenesis and a comparison with medial temporal lobe dementias.

Korsakoff's Syndrome is an amnestic disorder that involves both anterograde and retrograde amnesia. Traditionally associated with longstanding alcohol misuse, thiamine deficiency has been long posited in its pathogenesis, as has dienecephalic lesions. Yet, through this case report, we highlight similarities (and differences) with medial temporal lobe memory disorders and the role of frontal lobe dysfunction in its retrograde amnesia.

Temporal stability of visually selective responses in intracranial field potentials recorded from human occipital and temporal lobes.

The cerebral cortex needs to maintain information for long time periods while at the same time being capable of learning and adapting to changes. The degree of stability of physiological signals in the human brain in response to external stimuli over temporal scales spanning hours to days remains unclear. Here, we quantitatively assessed the stability across sessions of visually selective intracranial field potentials (IFPs) elicited by brief flashes of visual stimuli presented to 27 subjects. The interval between sessions ranged from hours to multiple days. We considered electrodes that showe

Alzheimer's Disease-associated Region-specific Decrease of Vesicular Glutamate Transporter Immunoreactivity inthe Medial Temporal Lobe and Superior Temporal Gyrus.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder for which there are very limited treatment options. Dysfunction of the excitatory neurotransmitter system is thought to play a major role in the pathogenesis of this condition. Vesicular glutamate transporters (VGLUTs) are key to controlling the quantal release of glutamate. Thus, expressional changes in disease can have implications for aberrant neuronal activity, raising the possibility of a therapeutic target. There is no information regarding the expression of VGLUTs in the human medial temporal lobe in AD, one of the ear…

Pearls & Oy-sters: Mesial Temporal Seizures in the Absence of the Mesial Temporal Lobe. Seizure Onset vs Seizure Network.

Seizure semiology represents the clinical expression of the activation of the several brain regions comprising an epileptic network. In mesial temporal lobe epilepsy (MTLE), this network includes the insular-opercular-neocortical temporal-hippocampal (IONTH) regions. In this study, we present the case of a patient with pharmacoresistant seizures characterized by nausea, lip-smacking, semipurposeful hand movements, and speechlessness, suggesting dominant hemisphere MTLE, with scalp video-EEG findings and left hippocampal sclerosis on brain MRI confirming the diagnosis. She underwent anterior te…

Serotonin receptor expression in hippocampus and temporal cortex of temporal lobe epilepsy patients by postictal generalized electroencephalographic suppression duration.

OBJECTIVE: Prolonged postictal generalized electroencephalographic suppression (PGES) is a potential biomarker for sudden unexpected death in epilepsy (SUDEP), which may be associated with dysfunctional autonomic responses and serotonin signaling. To better understand molecular mechanisms, PGES duration was correlated to 5HT1A and 5HT2A receptor protein expression and RNAseq from resected hippocampus and temporal cortex of temporal lobe epilepsy patients with seizures recorded in preoperative evaluation. METHODS: Analyses included 36 cases (age = 14-64 years, age at epilepsy onset = 0-51 year…

Costs Associated with Laser Interstitial Thermal Therapy Are Lower Than Anterior Temporal Lobectomy for Treatment of Temporal Lobe Epilepsy.

BACKGROUND: Laser interstitial thermal therapy (LITT) is a minimally invasive alternative to anterior temporal lobectomy (ATL) for treatment of temporal lobe epilepsy. It has gained popularity as familiarity with technique increases and outcomes are better characterized. There has been no direct cost comparison between the 2 techniques in literature to date. The current study directly compares hospital costs associated with LITT with those of ATL patients and analyzes the factors potentially responsible for those costs. METHODS: Patients who underwent ATL (27) and LITT (15) were retrospective…

Neuropsychological functioning in children with temporal lobe epilepsy and hippocampal atrophy without mesial temporal sclerosis: a distinct clinical entity?

Unilateral hippocampal atrophy (HA) is considered as a precursor of mesial temporal sclerosis (MTS) in some patients with temporal lobe epilepsy. However, in other cases, it has been suggested that HA without MTS may constitute a distinct epileptic entity. Hippocampal atrophy without MTS was defined as HA without T2-weighted hyperintensity, loss of internal architecture, or associated lesion seen on the MRI data. To date, no study has focused on the cognitive pattern of children with epilepsy with HA without MTS. The objectives of the present study were to characterize the cognitive profile of…
