Calcific Peritendinitis in the Gluteus Maximus Tendon.

BACKGROUND: Calcific tendinitis is classically a painful condition that most commonly affects the rotator cuff, but may infrequently involve other tendons. CASE REPORT: We discuss a 57-year-old man who presented to the emergency department with a 4-day history of right hip pain, described as the "worst pain in (his) life." The pain was first noticed at night and had progressively worsened. History, physical examination, and initial laboratory workup indicated an inflammatory vs. infectious process. Continued investigations with imaging techniques revealed the source of pain as calcific tendin

Defining imaging sub-phenotypes of psoriatic arthritis: integrative analysis of imaging data and gene expression in a PsA patient cohort.

OBJECTIVES: To define imaging sub-phenotypes in patients with PsA; determine their association with whole blood gene expression and identify biological pathways characterizing the sub-phenotypes. METHODS: Fifty-five patients with PsA ready to initiate treatment for active disease were prospectively recruited. We performed musculoskeletal ultrasound assessment of the extent of inflammation in the following domains: synovitis, peritenonitis, tenosynovitis and enthesitis. Peripheral whole blood was profiled with RNAseq, and gene expression data were obtained. First, unsupervised cluster analysis…

MRI in psoriatic arthritis: insights into pathogenesis and treatment response.

Psoriatic arthritis (PsA) is a clinically heterogeneous condition, and not surprisingly, its MRI features are diverse. Synovitis and accompanying synovial effusions are clearly depicted, and enthesitis is characterized by extracapsular inflammation at the insertions of ligaments and tendons plus accompanying bone edema at bony attachments. Other forms of MRI bone edema include subchondral and diaphyseal involvement; the latter seeming relatively specific to PsA. The pathology of dactylitis can also be elucidated by MRI, which frequently reveals tenosynovitis and soft tissue edema in conjunctio…

Anatomical variations in the first extensor compartment of the wrist. A clinical and anatomical study.

We studied 300 wrists from cadavera and the wrists of forty patients with de Quervain disease to determine the variation in the pattern of the tendons and septa in the first extensor compartment. In 75 per cent of the wrists from cadavera, we found that the number of tendons within the compartment differed from what is considered standard; there was complete or partial septation in 40 per cent. In about a third of the specimens from cadavera, the first extensor compartment was divided by a septum and two tendons or more were present within the major subcompartment. These features might readily

Characterization of a Musculoskeletal Syndrome of Enthesitis and Arthritis in Patients With Atopic Dermatitis Treated With Dupilumab, an Interleukin-4/13 Inhibitor.

OBJECTIVE: To characterize the presentation and outcomes of patients with atopic dermatitis (AD) who developed musculoskeletal symptoms afte … (14 male, 12 female) with a musculoskeletal syndrome of inflammatory enthesitis, arthritis, and/or tenosynovitis. Clinical findings were confirmed by US and MRI. All patients had very good response to dupilumab treatment, and no specific predictors of musculoskeletal syndrome were noted. Symptoms were mild in 16 patients, moderate in 6 patients, and severe in …

Arthritis After Cancer Immunotherapy: Symptom Duration and Treatment Response.

OBJECTIVE: Musculoskeletal manifestations of immune-related adverse events (irAEs) after checkpoint inhibitor immunotherapy for cancer remai … nivolumab monotherapy. Four patients developed inflammatory polyarthritis, 4 oligoarthritis, and 2 tenosynovitis. Six were antinuclear antibody positive and 2 had anti-cyclic citrullinated peptide antibodies. Mean ± SD time from the first dose of immunotherapy until joint involvement was 6.3 ± 4.3 months. All 10 patients were treated with systemic corticostero …

Prosthetic Joint Infections Due to Histoplasma capsulatum: A Report of 3 Cases.

Histoplasma capsulatum causes pneumonia and multisystemic disease in humans. Musculoskeletal involvement in histoplasmosis is most often tenosynovitis and rarely septic arthritis. Even more uncommon is the involvement of prosthetic joints. Here, we report a series of 3 cases of prosthetic joint failures caused by infection due to H capsulatum. Together with a review of 4 previously reported cases, we summarize host characteristics, clinical presentation, surgical approaches, antifungal management, and outcomes of this rare orthopedic joint infection.

Mycobacterium haemophilum skin and soft tissue infection in a kidney transplant recipient: A case report and summary of the literature.

Non-tuberculous mycobacteria are ubiquitous pathogens causing infections in immunocompromised patients. Here, we describe a kidney transplant recipient who developed skin and soft tissue infection by Mycobacterium haemophilum, complicated by tenosynovitis and fluid collection, following an injury sustained to her right foot. Her immunosuppressant dose was reduced, and she underwent prolonged antimicrobial therapy followed by surgical debridement with a favorable outcome. Non-tuberculous mycobacteria should be considered as a potential etiology of subacute skin and soft tissue infections.

Lower extremity infections: Essential anatomy and multimodality imaging findings.

In modern practice, imaging plays an integral role in the diagnosis, evaluation of extent, and treatment planning for lower extremity infections. This review will illustrate the relevant compartment anatomy of the lower extremities and highlight the role of plain radiographs, CT, US, MRI, and nuclear medicine in the diagnostic workup. The imaging features of cellulitis, abscess and phlegmon, necrotizing soft tissue infection, pyomyositis, infectious tenosynovitis, septic arthritis, and osteomyelitis are reviewed. Differentiating features from noninfectious causes of swelling and edema are disc…

Painful Os Peroneum Syndrome Secondary to Hydroxyapatite Deposition Disease.

Painful os peroneum syndrome (POPS) is one of the uncommon causes of lateral foot pain in orthopaedic practice. POPS encompasses a spectrum of pathologies such as an acute or a chronic fracture, peroneus longus tenosynovitis, and attrition or partial rupture of the peroneus longus tendon. Herein, we reported the first case of POPS secondary to hydroxyapatite deposition disease in a 32-year-old female patient. The clinical presentation, the role of radiological interventions, particularly ultrasound-guided barbotage in its diagnosis and effective management, is highlighted.

The Retinaculum Flap for Dorsal Fixation of Distal Radius Fractures.

In spite of the more popular volar approach for plating of distal radius fractures (DRFs), there are still indications for dorsal plating and even combined dorsovolar plating in complex DRFs. Dorsal plating carries a concern for complications such as tenosynovitis, extensor tendon adhesions, and delayed extensor tendon ruptures. These risks can be minimized by use of modern low-profile plates and the use of an extensor retinaculum flap to cover the distal plate and screws. We describe the technique of the dorsal retinaculum flap for dorsal plating of DRFs.
