Implementation of an enhanced recovery after thoracic surgery care pathway for thoracotomy patients-achieving better pain control with less (schedule II) opioid utilization.

BACKGROUND: Enhanced recovery after surgery protocols incorporate evidence-based practices of pre-, intra- and post-operative care to achieve the most optimal surgical outcome, safe on-time discharge, and surgical cost efficiency. Such protocols have been adapted for specialty-specific needs and are implemented by a variety of surgical disciplines including general thoracic surgery. This study aims to evaluate the impact of our enhanced recovery after thoracic surgery (ERATS) protocol on postoperative outcomes, pain, and opioid utilization following thoracotomy. METHODS: This is a retrospecti…

Prediction of Fluid Responsiveness Using Pulse Pressure Variation in Infants Undergoing Ventricular Septal Defect Repair with Median Sternotomy or Minimally Invasive Right Thoracotomy.

Fluid management is challenging in infants after cardiopulmonary bypass. Pulse pressure variation (PPV) derived from pressure recording analytical method (PRAM) is based on lung-heart interaction during mechanical ventilation. A prospective observational study conducted in operating room tested PPV to predict fluid responsiveness in ventricular septal defect infants. Infants in open chest conditions with median sternotomy (n = 26) or minimally invasive right thoracotomy (n = 29) undergoing ventricular septal defect repair were enrolled. After cardiopulmonary bypass and modified ultrafiltration…

Impact of preservation of the latissimus dorsi muscle through a left anteroaxillary thoracotomy on spinal cord protection in descending thoracic and thoraco-abdominal aortic operations†.

OBJECTIVES: The presence of collateral network circulation to the spinal cord, which is reflected in the repair of a descending thoracic aortic aneurysm (dAo) and a thoraco-abdominal aortic aneurysm (TAAo), has been demonstrated in clinical and animal experimental data. The latissimus dorsi muscle (LDM) including the thoracodorsal artery might be one of the major sources of this collateral network. The objective of this study was to evaluate the impact on spinal cord safety of a left anteroaxillary thoracotomy with minimal muscle division including preservation of the LDM in surgery for dAo an…

Anatomical tricuspid valve replacement in a patient with corrected transposition of the great arteries and situs inversus with 90° clockwise rotation of the heart through right thoracotomy.

The patient was a 34-year-old man with corrected transposition of the great arteries and situs inversus who was admitted with dyspnea. He had undergone ventricular septal defect closure and pulmonary valve commissurotomy at the age of 15. Preoperative examinations revealed severe tricuspid (systemic atrioventricular valve) insufficiency associated with dysfunction of the systemic (anatomical right) ventricle. The tricuspid valve orifice was shown to open dorsally by computed tomography. Because of 90° clockwise rotation of the heart, surgery was performed through right side thoracotomy. Tricus…

Endovascular repair of a proximal aortic arch aneurysm: a novel approach of supra-aortic debranching with antegrade endograft deployment via an anterior thoracotomy approach.

Although open surgical repair continues to be the standard therapy for thoracic aortic aneurysms, endovascular intervention has evolved into an acceptable strategy for patients who have prohibitive risks for conventional surgical treatments. Aortic arch aneurysm, in particular, is associated with substantial surgery-related morbidities, yet is typically not suitable for endovascular intervention. We describe a combined technique of supra-aortic trunk debranching through an anterior thoracotomy followed by endovascular repair of a large proximal arch aneurysm in an 82 year-old man 8 years after…

Early Outcomes in Severely Obese Patients Undergoing Sternum-Sparing Minimally Invasive Multivessel Artery Bypass Grafting Using Total Coronary Revascularization via Left Anterior Mini-Thoracotomy.

Background/Objectives: Severe obesity significantly increases the risk of complications following full sternotomy in coronary artery bypass grafting (CABG). However, these patients are frequently excluded from less invasive, sternum-sparing surgical alternatives. This study aimed to assess the safety and practicality of a newly developed technique-Total Coronary Revascularization via left Anterior miniThoracotomy (TCRAT)-that avoids sternotomy in patients with severe obesity requiring multivessel CABG. Methods: From November 2019 to May 2024, a total of 502 non-emergency patients with multives…

Ultrasound-guided external oblique intercostal plane block versus thoracic erector spinae block for post-thoracotomy pain: A randomised double-blinded non-inferior clinical study.

BACKGROUND AND AIMS: Regional techniques are preferred for controlling post-thoracotomy pain due to lower complication rates. This study aimed to compare the analgesic efficacy and safety of ultrasound-guided external oblique intercostal block (EOIB) with thoracic erector spinae plane block (ESPB) for post-thoracotomy pain. METHODS: This randomised, double-blind, non-inferior clinical study involved 60 cases scheduled for thoracic surgery. Cases were randomly allocated into two groups: ESPB and EOIB groups. Using 30 mL 0.25% bupivacaine at the level of the thoracic vertebrae 5, both blocks we…

Post-operative analgesia by high thoracic epidural versus intramuscular nicomorphine after thoracotomy. Part III. The effects of per- and post-operative analgesia on morbidity.

One hundred and twenty-nine patients were subjected to three different types of thoracic operations. The patients were randomly allocated to balanced intravenous anaesthesia including i.v. nicomorphine during surgery and epidural nicomorphine post-operatively (epidural group, n = 58) or to balanced intravenous anaesthesia without i.v. opiates but with high thoracic epidural regional block during the operation and with post-operative intramuscular nicomorphine (intramuscular group, n = 71). Post-operative nicomorphine was only given at the request of the patients, and as frequently as needed to

Simultaneous beating-heart mitral valve replacement and aortic repair following Bentall procedure via double right mini-thoracotomies: a case report.

BACKGROUND: Redo mitral valve surgery by re-sternotomy approach has challenges such as bleeding and organ injury due to adhesion dissection, sternal bone infection, and poor field of view of mitral valve. On the other hand, redo mitral valve surgery via a right mini-thoracotomy approach appears to address these challenges. We successfully performed a double right mini-thoracotomies approach for mitral valve replacement and pseudoaneurysm repair under the beating-heart condition. Herein, we report the effectiveness and the safety of this technique and detailed procedure. CASE PRESENTATION: The…
