Pathogenesis of tracheal stenosis following tracheostomy with a cuffed tube. An experimental study in dogs.

A study was undertaken to clarify the pathogenesis of tracheal stenosis complicating the use of cuffed tracheostomy tubes, a complication of increasing clinical significance. Tracheostomy with a cuffed inflated tube was established in dogs and was maintained for two weeks. The effects of tracheostomy were assessed by bronchoscopy, tracheography, and histology at the time of killing, eight weeks after extubation. Animals were divided into four groups, each mimicking a common clinical situation. A control group had uncuffed tubes. A method was designed to determine the pressure developed between

Anesthetic Management of a Patient With Emanuel Syndrome.

Emanuel syndrome is associated with supernumerary chromosome, which consists of the extra genetic material from chromosome 11 and 22. The frequency of this syndrome has been reported as 1 in 110,000. It is a rare anomaly associated with multiple systemic malformations such as micrognathia and congenital heart disease. In addition, patients with Emanuel syndrome may have seizure disorders. We experienced anesthetic management of a patient with Emanuel syndrome who underwent palatoplasty. This patient had received tracheotomy due to micrognathia. In addition, he had atrial septal defect, mild pu…

Four had a documented laryngeal injury, one of which was a major injury requiring a tracheotomy. CONCLUSION: Laryngeal examination should be standard of care for any child presenting after a near-hanging event.

Transmylohyoid/Submental intubation: review, analysis, and refinements.

Successful outcome in any surgery is dependent on unobstructed access. Management of patients with polytrauma of the face or those undergoing multiple/complex facial osteotomies has always been a challenge, not only to maxillofacial surgeons but also to the anesthetists, as both specialists fight for the same anatomic territory. Hernandez in 1986 published the first article on the submental route for endotracheal intubation. He developed this technique to avoid tracheotomy particularly in maxillofacial trauma where short-term maxillomandibular fixation was required.Since our first report in 19…

Life-threatening systemic toxicity and airway compromise from a common European adder bite to the tongue.

A 24-year-old man was bit on the tongue by a European common adder. Within 15 min following envenomation, he experienced tongue swelling, hypotension and impaired consciousness. Antihistamine, corticosteroid and crystalloids were administered. Within 105 min of envenomation, increasing oral, pharyngeal and facial oedema compromised the airway, leading to respiratory failure, concomitant with circulatory failure related to hypoxaemia and systemic toxic effects. Acute tracheotomy secured the airway, and two doses of antivenom successfully treated the systemic, toxic effects. The reaction was sev…

Idiopathic vocal fold paralysis: clinical course and outcomes.

This investigation details the results of a retrospective chart review of 35 patients with idiopathic vocal fold paralysis. Clinical findings at presentation, as well as patient outcomes in both the short and long-term duration are comprehensively reported. Spontaneous improvement of vocal fold function was observed in 25% of patients with long-term follow up. Of those with unilateral paralysis, 27% elected to undergo surgical intervention for improvement of symptoms. Four out of the five (80%) patients with bilateral paralysis required surgical intervention, including tracheotomy to relieve s…

Sleep disorders caused by brainstem tumor: case report.

Few studies concerning sleep disorders in brainstem lesions or tumors have been published. We report the case of a girl who was operated on for a brainstem tumor at the age of 4 years. In postsurgery, she had hemiparesis of the left side, swallowing difficulties, and severe apneas requiring a tracheotomy with nocturnal ventilation. The child's health improved progressively. Two sleep recordings were performed at 7 and 9 years without nocturnal ventilation. These recordings showed sleep disorders with a decrease in total sleep time and rapid eye movement (REM) sleep. Several central apneas were…

Recurrent Wegener's granulomatosis with subglottic granulation.

While following the course of patients with Wegener's granulomatosis (WG) in which it had been possible to achieve a remission, we encountered a patient in whom we observed granuloma formation in the subglottis. A tracheotomy was performed because of the risk of respiratory distress, and after that there was a recrudescence of the patient's WG. During the course of the above, the patient remained cANCA (anti-neutrophil cytoplasmis antibody with cytoplasmic pattern)-negative. When these facts were considered, it appeared that when the lesions are localized and progressing gradually, cANCA posit…

Neonatal laryngoceles. A cause for airway obstruction.

The neonatal laryngocele is a congenital pathologic entity originating in the saccule of the laryngeal ventricle. This entity may present as … case. Surgical intervention required airway endoscopy and an external approach to excise the mass. Tracheotomy was avoided in both cases; however, one infant required further surgery for laryngomalacia. Both children have done well in follow-up.

[Arytenoidopexy in bilateral abductor paralysis of the glottis].

The arytenoidopexy operation has been performed in 15 cases of bilateral abductor paralysis of different causes. A tube on n. 6 for women and a tube of n. 6.5 for men has been used for the intratracheal general anesthesia without a tracheotomy. The microdissection of the arytenoid has been realised under the operation microscope. The fixation suture of the arytenoid has been passed with a special needle set prepared by the experimental studies on the laryngectomised pieces. The functional respiration result has been controlled by the pre and postoperative spirometry. The laryngeal spasm disapp…

Reduction of salivary flow with transdermal scopolamine: a four-year experience.

Scopoderm transdermal therapeutic system (TTS) is applied to prevent nausea and vomiting associated with motion sickness. Dry mouth is the most common side effect, appearing in up to two thirds of the patients treated. We have used this side effect to the benefit of patients with sialorrhea or with difficulties swallowing normally secreted amounts of saliva. More than 100 patients with tumors of the aerodigestive tract before and after surgery, and patients after parotidectomies, after tracheotomies, with peritonsillar abscesses, tonsillitis and pharyngitis, and neurologic disorders were thus

Vocal cord paralysis in children is an uncommon but often disabling problem. It may be congenital or acquired either in the immediate period … o address the problem, as is most appropriate to the individual case. The usefulness of intubation, tracheotomy, surgical lateralization of the vocal folds, and reinnervation in the management of these problems is discussed, and 30 cases of vocal cord paralysis in children under the age of 5 years are presented.

One particular form of congenital stenosis of larynx that should be considered as a separate entity is the combination of a glottic stenosis … ated glottic palmation: all attempts at endoscopic treatment resulted in failure that intubation or tracheotomy was necessary in 7 cases. One of the 3 fatal cases was directly related to lack of detection of the subglottic stenosis. A successful laryngoplasty was performed in 10 children.

Stomal recurrences: a clinicopathological analysis and protocol for future management.

From 1965 to 1975, 452 total laryngectomies were performed at the Washington University Medical Center, St. Louis, Mo. During this period, 33 stomal recurrences were diagnosed. These are clinicopathologically analyzed with regard to tumor size, site, histopathology and original surgical procedure. An association with transglottic tumors, emergency tracheotomy and, surprisingly, hemilaryngectomy is noted. Three theories of etiology are discussed. Since treatment to date is disappointing, several proposed methods of prophylaxis are presented. These include emergency laryngectomy, recurrent laryn…
