The surgical treatment of severe scoliosis and kyphoscoliosis in the pediatric population is complicated and has high morbidity and mortalit … sonable or possible. Acute correction of a deformity may also cause neurologic injury. Halo gravity traction (HGT) allows for progressive, gradual, and sustained correction of the spinal deformity in the coronal, sagittal, and axial planes, leading to a decrease in the amount of correction needed at definitive posterior fusion. This relates to decreased po …

BACKGROUND AND OBJECTIVE: This study evaluated the use of a dual-delivery methotrexate (MTX) strategy for diabetic tractional retinal detachment (TRD). Diabetic retinopathy (DR) is a leading cause of blindness. Inflammation plays a key role in TRD, and MTX has anti-inflammatory properties.
PATIENTS AND METHODS: This is a pilot study that included 60 patients with TRD. They were block randomized to either MTX with pars plana vitrectomy (PPV) (n = 30) or PPV alone (control, n = 30). MTX was added to the irrigation fluid during surgery and an intra-silicone injection at the end. One month after …

Intertrochanteric fractures are a common occurrence in the general population; however, in patients with above-knee amputations, they are relatively rare. In this patient population, positioning on a fracture table presents a particularly difficult problem prior to the fixation of an intertrochanteric fracture. Here, we describe a 57-year-old man with extensive vasculopathy and reduced bone density who presents with an intertrochanteric fracture after a fall from standing. Adequate traction of the amputated leg was achieved via the modification of a standard fracture table and the utilization …

PURPOSE: Intraoperative traction can improve deformity correction during posterior spinal fusion (PSF). This is commonly done with invasive distal femoral or pelvic pins, or traction boots. The novel technique of intraoperative skin traction (ISkinT) avoids risks associated with intraoperative skeletal traction (ISkelT) or hyperlordosis with extended hip position. We aimed to describe ISkinT and assess its safety and efficacy in PSF in non-ambulatory scoliosis.
METHODS: Retrospective review of patients aged 10-21yo who underwent T2-pelvis PSF with ISkinT from 2017 to 2023. Demographics and ra…

A 23-year-old man developed bilateral rhegmatogenous retinal detachments secondary to high-titer ocular syphilis. The patient's titer increased four-fold after completing a 14-day course of intravenous penicillin (IVP). He underwent bilateral pars plana vitrectomy with silicone oil fill in both eyes. In this article, the authors propose an updated treatment method for patients with advanced ocular syphilis that includes oral doxycycline for 30 days following 14 days of IVP to optimally minimize the patient's infectious burden. Following surgery and this new treatment regime, this patient's bes

PURPOSE: To describe a surgical technique using the structural advantages of beveled tip cutters.
METHODS: The introduction of beveled tips has been one of the few modifications that have been performed to vitrectomy probes since first described by Machemer in 1972. Shovel and cut technique uses this incredible modification to access tighter planes and remove broad diabetic membranes.
DESCRIPTION OF TECHNIQUE: The shovel and cut technique can be used with any gauge probe to which the bevel tip is applied. The beveled tip of the cutter is used in a shovel manner to create a tissue plane betwe

How to prescribe traction on boundary surface is still an open question in peridynamics. This problem is investigated in this paper. Through introducing the induced body force defined by boundary traction, the Silling's peridynamic motion equation is extended to a new formulation called the traction-associated peridynamic motion equation, which is verified to be compatible with the conservation laws of linear momentum and angular momentum. The energy conservation equation derived from the traction-associated peridynamic motion equation has the same form as that in the original peridynamics adv…
