Neonatal Point-of-Care Lung Ultrasonography: Techniques and Normal Findings.

Point-of-care lung ultrasonography (POC-LUS) has become a routine bedside tool for evaluating respiratory status in many neonatal intensive care units. This article walks neonatologists through the fundamentals of its application-probe selection, system setup, and image acquisition. Lung aeration can be assessed using a 6-zone scanning approach. We review the normal sonographic appearance of the neonatal lung, including lung sliding, A-lines, and the B-lines commonly seen during transitional physiology, and describe how M-mode confirms pleural motion when sliding is equivocal. Common pitfalls

A hemodynamic-driven approach to chronic pulmonary hypertension in congenital diaphragmatic hernia.

Despite ample research in acute pulmonary hypertension in neonates with congenital diaphragmatic hernia (CDH) leading to improved survival, there is a paucity of literature on the physiology of chronic pulmonary hypertension in this population. Serial hemodynamic assessments and physiology-centric care have been shown to be safe and effective in the acute transitional and peri-operative care of these neonates and will likely be informative in the discussion of chronic pulmonary hypertension as well. Herein, we discuss the epidemiology, contributors, and proposed physiology-based phenotypes of

Longitudinal associations between perceived father and grandparent support of mothers in early childhood, and maternal mental health.

BACKGROUND: Maternal mental health issues have emerged as a public health concern garnering increasing attention in recent years, especially in low and middle-income countries (LMICs). The involvement of fathers and grandparents in infant care has been recognized as a key factor affecting maternal mental health. However, few studies have focused on the relationship between infant care assistance patterns and maternal mental health issues. This study aims to examine how infant care assistance patterns and its transitions over time affect maternal mental health in rural China. METHOD: This long

Water-soluble contrast in the management of adhesive small-bowel obstruction: a Canadian centre's experience with guideline development and implementation.

BACKGROUND: Orally administered water-soluble contrast (WSC) can track resolution of small-bowel obstruction (SBO), but no universal pathway for its use exists. We developed and implemented an evidence-based guideline for the use of WSC in the management of adhesive SBO, to be implemented across hospitals affiliated with the University of Toronto. METHODS: We performed a systematic review and created a clinical practice guideline for WSC use in the management of adhesive SBO. The guideline was approved through consensus by an expert panel and implemented in 2018. We performed a prospective co

Adapting to Adulthood: A Review of Transition Strategies for Osteogenesis Imperfecta.

Osteogenesis Imperfecta (OI), known as "brittle bone disease," presents a rare genetic disorder characterized by bone fragility, often accompanied by skeletal deformities and extraskeletal complications. OI is primarily associated with collagen type I defects, responsible for the syndromic nature of the disease affecting a broad range of tissues. As such, its multisystemic complexity necessitates multidisciplinary care approaches in all patient life stages. OI treatment remains largely supportive, commonly including bisphosphonates and orthopedic surgeries, which show promise in children. Alth

From Hospital to Home: Evidence-Based Care for Worsening Heart Failure.

Heart failure (HF) is a leading cause of hospitalization in older adults. Patients are at high risk of readmission and death following hospitalization for HF. There is no standard approach of health care delivery during the hospital-to-home transition period, leaving missed opportunities in care optimization. In this review, we discuss contemporary randomized clinical trials that tested decongestion strategies, disease-modifying therapies, and health care services that inform the care of patients with worsening HF. We provide evidence-informed recommendations for optimizing therapies and impro

How do we organize smooth, effective transfer from paediatric to adult diabetes care?

Transition from paediatric to adult care is a major milestone in the life of a young person with diabetes, yet it is commonly poorly done. This finding is supported by the dearth of publications in the literature that particularly relate to transition in diabetes care. Other transitions occur at this time of life, for example: from parental supervision to growing independence and from secondary school to post-school options. Health professionals know, from anecdotal evidence, that this is the period when young people with diabetes 'drop out' of the system. They often have no specialist follow-

Adult endocrinologists' perspectives on transitioning adolescent patients with congenital adrenal hyperplasia.

OBJECTIVES: The study aimed to evaluate adult endocrinologists' perspectives on caring for patients with congenital adrenal hyperplasia (CAH) and views on their transition from pediatric to adult care. METHODS: An online survey was conducted among adult clinical endocrinologists at Harvard Medical School-affiliated hospitals from March to October 2022. RESULTS: Most participants (25/34, 73.5 %) treat patients with CAH and expressed moderate to high confidence (23/32, 71.9 %) in their care. Those that did not treat or accept referrals cited insufficient expertise, knowledge, and resources as

Large social disparities in spontaneous preterm birth rates in transitional Russia.

OBJECTIVE: This study estimated the effect of maternal sociodemographic, obstetric and lifestyle factors on the risk of spontaneous preterm birth in a Russian town. METHODS: All women with singleton pregnancies registered at prenatal care centres in Severodvinsk in 1999 comprised the cohort for this study (n=1559). Analysis was based on spontaneous live singleton births at the maternity home (n=1103). Multivariable logistic regression was applied to quantify the effect of the studied factors on the risk of preterm birth. Differences in gestation duration were studied using multiple linear reg

"It's On Your Shoulders Now" Transitioning from Child-to-Adult UK Cleft Lip/Palate Services: An Exploration of Young Adults' Narratives.

ObjectivesTreatment for cleft lip and/or palate (CL/P) in the United Kingdom is administered on a standardised pathway from diagnosis to early adulthood, with options to be re-referred in later life. At age 16, patients become responsible for their treatment decisions. Evidence from the wider health literature indicates this transition can be challenging and that this population may require additional support. The present study explored young people's experiences of transition to adult care in the context of CL/P services, with the aim of identifying support needs and informing future service

ENDOCRINOLOGY AND ADOLESCENCE: Dealing with transition in young patients with pituitary disorders.

INTRODUCTION: The transition age is the period between childhood to adulthood; it refers to a broad set of physical, cognitive and sociocultural modifications, arbitrarily defined as starting in late puberty and ending with full adult maturation. Pituitary disorders in adolescence represent a challenge that requires careful management during the transition to adult care. METHODS: Given the complexity of care of pituitary disorders in the transition age, we have reviewed the relevant medical literature focusing on aetiology, clinical manifestations, treatment strategies of GH deficiency (GHD),

Proceedings of the University of Alabama at Birmingham Continuous Renal Replacement Therapy Academy (2023-2024): Managing De-Escalation of Acute Renal Replacement Therapy and Optimizing Drug Dosing during Renal Replacement Therapy Transitions.

In this second installment of the proceedings of the University of Alabama at Birmingham Continuous Renal Replacement Therapy (CRRT) Academy, we focus on the topic of transitions of care in acute renal replacement therapy (RRT). Although we have accumulated data from thousands of critically ill patients with AKI randomized to different strategies for RRT initiation, no trial data exist to guide de-escalation of RRT in the intensive care unit. However, for survivors of severe AKI whose kidney function does not recovery rapidly enough to allow for liberation directly from CRRT, successful de-esc

Transitional Urology for Male Adolescents: What Adult Urologists Should Know.

In recent years, there has been increasing interest in transitional urology, or how to best prepare patients with major congenital urologic diseases, such as bladder exstrophy and neuropathic bladder, to manage their own health care with adult urologists. However, common pediatric urologic conditions may be encountered by the adult urologist with more regularity. This review focuses on three relatively common conditions which may be identified in childhood, the consequences from which a patient may seek help from an adult urologist: cryptorchidism, varicocele, and Klinefelter syndrome.

Safe homecoming intervention after hospital discharge: a retrospective cohort study of mortality and readmission among patients 75 years or older in Sweden.

BACKGROUND: After a hospital stay, older patients are at increased risk of health complications that may lead to readmission to hospital or premature death. Globally, numerous interventions have been applied to enhance transitional care for older patients, but the evidence base remains inconclusive. One form of a transitional care intervention is the Swedish primary health care Safe homecoming intervention, a time-limited, interprofessional and tailored home health care intervention designed to prevent avoidable complications and readmissions following hospital discharge. Transitions in Glomerular Disease.

BACKGROUND: The transition from pediatric to adult care is a complex and challenging issue for increasing numbers of adolescents and young adults (AYA). Despite increasing attention to the transition of AYA with chronic medical conditions, there remain difficulties which vary by local resources, disease processes, and individual factors. There are also poorly understood factors regarding transition involving rare diseases, including glomerular disease. Specialties and organizations are working to evaluate and enhance the transition process; however, the variability in resources limits formatio
