After discussion of the antigen structure of Treponema pallidum it is shown that only reactions with the pallidum antigen can establish a serological diagnosis of syphilis. Reactions with Reiter antigen are superfluous, reactions with lipoid antigen (cardiolipin) are non-specific and not very sensitive and consequently only conditionally useful as a test reaction. BACKGROUND: Neurosyphilis refers to infection of the central nervous system by Treponema pallidum. The clinical presentation is variable and nonspecific. Neuroimaging findings are complex and that the diagnosis is based on clinical presentation, cerebrospinal fluid (CSF) parameters, and serologic and CSF evidence of syphilis. Enzyme immunoassay (EIA) using recombinant antigens for the detection of Treponema pallidum-specific antibodies in sera of syphilis patients was developed. OBJECTIVES: Syphilis is a sexually transmitted infection (STI) caused by treponema pallidum. Its rash usually affects the trunk and limbs extensively, including the palms and soles of the feet. Secondary syphilis confined to the face is extremely rare.
METHODS: We report a case of annular rupioid secondary syphilis, which was misdiagnosed as verruca vulgaris.
