Admission Forced Vital Capacity Adds a Predictive Physiologic Tool to Triage Patients Suffering Rib Fractures: A Prospective Observation Trial.

BACKGROUND: Patients with rib fractures are at high risk for morbidity and mortality. This study prospectively examines bedside percent predicted forced vital capacity (% pFVC) in predicting complications for patients suffering multiple rib fractures. The authors hypothesize that increased % pFVC is associated with reduced pulmonary complications. METHODS: Adult patients with =3 rib fractures admitted to a level I trauma center, without cervical spinal cord injury or severe traumatic brain injury, were consecutively enrolled. FVC was measured at admission and % pFVC values were calculated for

Descriptive Analysis of Casualties Rapidly Returned to the Fight after Injury: Reverse Triage Implications for Large Scale Combat Operations.

BACKGROUND: During large scale combat operations, rising numbers of casualties will likely outstrip in-theater US military medical hospitalization assets. This highlights the importance of identifying those casualties who can return to the fight in order to minimize further medical resource depletion. We describe specific characteristics of casualties returned to duty without requiring evacuation from theater during recent major combat operations. MATERIALS AND METHODS: We conducted a secondary analysis of previously published data from the Department of Defense Trauma Registry during 01 Janu

Reliability of Field Assessment Stroke Triage for Emergency Destination Scale Use by Paramedics: Mobile Stroke Unit First-Year Experience.

BACKGROUND AND PURPOSE: Field Assessment Stroke Triage for Emergency Destination (FAST-ED) scale is a helpful tool to triage patients with stroke in the field. However, data on its reliability in the prehospital setting are lacking. We aim to test the reliability of FAST-ED scale when used by paramedics in a mobile stroke unit covering a metropolitan area. METHODS: As part of standard operating mobile stroke unit procedures, paramedics initially evaluated patients. If the event characterized a stroke alert, the FAST-ED score was determined by the paramedic upon patient contact (in-person) and…

Remote triage incorporating symptom-based risk stratification for suspected head and neck cancer referrals: A prospective population-based study.

BACKGROUND: Remote triage for suspected head and neck cancer (HNC) referrals was adopted by many institutions during the initial peak of the coronavirus disease 2019 pandemic. Its safety in this population has not been established. METHODS: A 16-week, prospective, multicenter national service evaluation was started on March 23, 2020. Suspected HNC referrals undergoing remote triage in UK secondary care centers were identified and followed up for a minimum of 6 months to record the cancer status. Triage was supported by risk stratification using a validated calculator. RESULTS: Data for 4568 …

System Level Informatics to Improve Triage Practices for Sickle Cell Disease Vaso-Occlusive Crisis: A Cluster Randomized Controlled Trial.

BACKGROUND: National Heart Lung and Blood Institute guidelines for the treatment of vaso-occlusive crisis among people with sickle cell disease in the emergency department recommend assigning an emergency severity index of 2 at triage. However, patients with sickle cell disease often do not receive guideline-concordant care at triage. To address this gap, a decision support tool was developed, in the form of a text banner on the triage page in the electronic health record system, visible to triage nurses. METHODS: A prospective quality improvement initiative was designed where the emergency s…

Genome-wide microRNA analysis of HPV-positive self-samples yields novel triage markers for early detection of cervical cancer.

Offering self-sampling for HPV testing improves the effectiveness of current cervical screening programs by increasing population coverage. Molecular markers directly applicable on self-samples are needed to stratify HPV-positive women at risk of cervical cancer (so-called triage) and to avoid over-referral and overtreatment. Deregulated microRNAs (miRNAs) have been implicated in the development of cervical cancer, and represent potential triage markers. However, it is unknown whether deregulated miRNA expression is reflected in self-samples. Our study is the first to establish genome-wide miR…

Automating the triage of rheumatology outpatient referrals: a comparative evaluation of 23 large language models under simple and advanced prompting.

Triage of rheumatology outpatient referrals is a high-volume administrative task that consumes senior specialist time without advancing patient care. The human triage system is only moderately accurate and reproducible. We assessed whether contemporary large language models (LLMs) are able to perform well enough to support automating this task in practice. In addition, the effects of different prompting techniques on triage accuracy and cost was assessed to help identify the optimal approach. Twenty referral scenarios spanning the urgency spectrum were created by a certified Australian rheumat…

Accelerating care for knee injuries: Impact of a novel acute knee injury virtual triage clinic on timely diagnosis and treatment.

INTRODUCTION: Knee injuries are a common presentation to Emergency Departments (ED). Initial assessment prioritizes the exclusion of fractures, dislocations and other urgent pathologies. However, subsequent soft-tissue knee injuries referral pathways to orthopedic services remains inconsistent. Variability in orthopedic referral pathways may lead to delays in definitive diagnosis and intervention, potentially compromising recovery and long-term functional outcomes. METHODS: This study reviews the first 250 patients referred from ED to an acute knee injury virtual triage clinic. Virtual clinic…

Artificial intelligence-assisted triage of cervical biopsies using the low-cost, 3D-printed OpenFlexure Microscope: a paired comparative feasibility study.

OBJECTIVE: Artificial intelligence has been used in pathology to analyze whole-slide images for the triage of biopsies for pathologist review. However, this has been performed with prohibitively costly slide scanners that may be unavailable in low- and middle-income countries, where the number of trained pathologists is limited. We evaluated whether a 3D-printed slide scanner (OpenFlexure Microscope) could create images of sufficient quality that could be used for artificial intelligence triage of cervical biopsies and compared them to images created by a commercial slide scanner. METHODS: Ce…
