Effect of PPD inoculation on serum cytokine profiles in latent tuberculosis infection populations.

This study aims to investigate the impact of PPD skin test administration on serum cytokine levels in individuals with latent tuberculosis infection (LTBI) compared to healthy controls. Through quantitative analysis of multiple cytokines in serum samples before and after testing, significant differences were observed in cytokine level changes among LTBI individuals following PPD stimulation. The results indicate that prior to PPD administration, there were no significant differences in cytokine levels between the LTBI group and healthy controls. However, following PPD testing, individuals with

Frequency of abnormal findings on chest radiograph after positive PPD in children and adolescents in an urban setting in the United States.

BACKGROUND: Chest radiographs (CXR) for tuberculosis (TB) screening in children are valuable in high-burden settings. However, less certain in low prevalence contexts. In the United States, positive PPD is sufficient to treat for "latent" TB, or TB infection in asymptomatic patients. OBJECTIVE: We sought to determine frequency of abnormal CXR findings after a positive purified protein derivative (PPD) test at a tertiary pediatric center in the United States. METHOD: A retrospective evaluation was conducted of patients (0-18 years) with a CXR after a positive PPD (e.g., known exposure, employ

Zidovudine-induced restoration of cell-mediated immunity to mycobacteria in immunodeficient HIV-infected patients.

OBJECTIVE: To describe a localized form of Mycobacterium avium intracellulare (MAI) infection occurring concurrently with the restoration of cutaneous delayed-type hypersensitivity (DTH) responses to mycobacterial antigens after commencement of zidovudine therapy in immunodeficient HIV-infected patients. PATIENTS: The first 108 Western Australian patients with a CD4+ T-cell count of < 200 x 10(6)/l and/or symptomatic disease to be given zidovudine (ZDV), of whom 72 had adequate DTH data. METHODS: DTH responses to seven antigens were measured by the 'Multitest' method before and on at least t

Delayed hypersensitivity: bone marrow as the source of cells in delayed skin reactions.

Adult Lewis rats were thymectomized, irradiated, and restored with bone marrow from allogeneic (or F(1)) donors. They were passively sensitized to tuberculin by a subsequent transfer of Lewis lymph node cells and were given intradermal skin tests with tuberculoprotein. In 24-hour skin reactions the majority of cells, in successive experiments, were shown to be allogeneic (or F(1)) with the use of isoantibody against the antigens of the transplanted marrow cells and by the indirect fluorescent antibody technique. Our results demonstrate that the non-specific cells making up a large proportion o

[Preliminary evaluation of the prevalence of tuberculosis infection in an occupational risk group].

BACKGROUND: The aim of this study was to gain knowledge on the prevalence of tuberculosis infection in occupational groups whose professional activity could implicate a high risk of contact with persons infected with Mycobacterium tuberculosis. Additionally, we sought to identify risk factors in order to establish effective measures for control and prevention. With this aim we carried out a pilot study among members of our police force. MATERIAL AND METHODS: Reactivity against tuberculin was tested in 150 individuals belonging to the Spanish National Police Force and an epidemiological survey

Field evaluation of specific mycobacterial protein-based skin test for the differentiation of Mycobacterium bovis-infected and Bacillus Calmette Guerin-vaccinated crossbred cattle in Ethiopia.

Bovine tuberculosis (bTB) challenges intensive dairy production in Ethiopia and implementation of the test and slaughter control strategy is not economically acceptable in the country. Vaccination of cattle with Bacillus Calmette-Guerin (BCG) could be an important adjunct to control, which would require a diagnostic test to differentiate Mycobacterium bovis (M. bovis)-infected and BCG-vaccinated animals (DIVA role). This study describes an evaluation of a DIVA skin test (DST) that is based on a cocktail (DSTc) or fusion (DSTf) of specific (ESAT-6, CFP-10 and Rv3615c) M. bovis proteins in Zebu-

A Defined Antigen Skin Test That Enables Implementation of BCG Vaccination for Control of Bovine Tuberculosis: Proof of Concept.

In most low- and middle-income countries (LMICs), bovine tuberculosis (bTB) remains endemic due to the absence of control programs. This is because successful bTB control and eradication programs have relied on test-and-slaughter strategies that are socioeconomically unfeasible in LMICs. While Bacillus Calmette-Guérin (BCG) vaccine-induced protection for cattle has long been documented in experimental and field trials, its use in control programs has been precluded by the inability to differentiate BCG-vaccinated from naturally infected animals using the OIE-prescribed purified protein derivat

Tuberculosis in wild olive baboons, Papio cynocephalus anubis (Lesson), in Kenya.

Several wild olive baboons from a single troop in the Masai Mara Game Reserve, Kenya were observed to be lethargic and emaciated. Five were trapped and tuberculin tested by intradermal inoculation of 0.1 cc (100 IU) mammalian old tuberculin in the upper eyelid. Two of the five showed positive reaction at 72 hr and were examined at necropsy. Gross lesions in both animals consisted of multiple nodules with caseation in the lung, spleen and tracheobronchial lymph nodes. There were multiple granulomas throughout the lung, spleen and the lymph nodes. Tissues were cultured on Lowenstein-Jensen media…

Primary tuberculosis of the glans penis.

A 60-year-old man presented with multiple superficial ulcers on the glans penis. Histopathology, a positive tuberculin test result, and therapeutic response to antituberculous therapy confirmed the diagnosis of penile tuberculosis. Examination was otherwise normal except for a solitary enlarged reactive lymph node on the right side. There was no evidence of coexistent tuberculous infection elsewhere.

Dinitrochlorbenzene (DNCB) treatment of viral warts. A 5-year follow-up study.

A series of 43 patients with therapy-resistant viral warts of long-duration were treated with dinitrochlorbenzene (DNCB). Of the 37 patients, who completed the treatment, 19 were cured, including five of seven patients with hyperkeratotic tumor-like warts. The majority of the patients showed a depressed cell-mediated immune response. 25 patients were reexamined 5 years later and 8 additional patients answered on inquiry. Six (18%) of these patients had warts. Eight of the 21 primarily tuberculin-negative patients had developed immunity to tuberculin, and none of them had warts. Five of the 6 p

Delayed-type hypersensitivity, mycobacterial vaccines and protective immunity.

There is a longstanding debate over the implications of natural and vaccine-induced delayed type hypertensivity for protective immunity to mycobacterial infections. The identification of correlates of vaccine-induced protective immunity should help explain the inconsistent behaviour of BCG vaccines in different populations and assist in efforts to devise improved vaccines. More than 70,000 subjects in Karonga District, northern Malawi were skin tested with soluble antigens of the tubercle and leprosy bacilli, and then followed up for five years for tuberculosis and leprosy incidence. Incidence

The detection of airborne transmission of tuberculosis from HIV-infected patients, using an in vivo air sampling model.

BACKGROUND: Nosocomial transmission of tuberculosis remains an important public health problem. We created an in vivo air sampling model to … ses of tuberculosis. There was a wide variation in monthly rates of guinea pigs developing positive tuberculin test results (0%-53%). Of 292 animals exposed to ward air, 159 developed positive tuberculin skin test results, of which 129 had laboratory confirmation of tuberculosis. The HIV-positive patients with pulmonary tuberculosis produced a mean of 8.2 infecti …

[Erythema induratum--atypical manifestation of infection Mycobacterium tuberculosis--case report].

The term erythema induratum refers to nodules usually located on the flexion surface of the lower legs, characterized by chronic and recurrent course--usually in women. The lesions are directly related to Mycobacterium tuberculosis infection which may be confirmed by tuberculin skin test, Quantiferon test or PCR. Diagnosis of erythema induratum is a direct indication for antytuberculous therapy extended with vascular drugs. In the presented case all above mentioned skin symptoms were diagnosed and their ethiological connection with Mycobacterium tuberculosis was documented.

Interferon gamma release assay (QuantiFERON-TB Gold In Tube) in patients of sarcoidosis from a population with high prevalence of tuberculosis infection.

BACKGROUND AND OBJECTIVE: Detecting latent tubercular infection (LTBI) in sarcoidosis has important treatment implications. Traditionally tuberculin skin test (TST) is relied upon for this purpose. However, sarcoidosis is known to produce tuberculin anergy, which is not affected by high prevalence of tuberculosis (TB) infection. Interferon gamma release assays (IGRAs) have a higher sensitivity and specificity for detecting Mycobacterium tuberculosis (MTB) infection than the conventional TST as they utilize antigens specific for MTB complex. However, there is limited data regarding the performa

Interferon-gamma release assays.

Diagnosis of latent tuberculosis infection (LTBI) should be targeted toward individuals and groups with high risk of progression to active tuberculosis (TB). Low-risk populations should not be screened. Interferon-gamma release assays (IGRAs) perform as well or better than the tuberculin skin test in most targeted populations. IGRAs are preferred for bacille Calmette-Guérin (BCG)-vaccinated populations. A positive IGRA in a person at low risk for TB exposure should be confirmed with a repeat test or another method before recommending LTBI treatment. The choice of which IGRA to use is generally

Cell-mediated immunity in an ageing population.

Eight hundred and eighty patients hospitalized in a geriatric hospital were routinely tested with 2, 10, 30 and 100 i.u. tuberculin. Among these, fifty-four patients were selected on the basis of negative skin tests and absence of evident diseases interfering with the function of the immune apparatus. A battery of tests analysing cell-mediated immunity was applied to those fifty-four patients. It appears that elderly patients having a negative test to 100 i.u. tuberculin show very infrequent sensitization to three other thymus-dependent antigens. The capacity of this selected population to bec
