Advancing Digital Access to Physical Therapy via Virtual and Extended Reality Technology: Prototype Development and Usability Evaluation.

BACKGROUND: The United States faces significant challenges in physical therapy (PT) access due to high demand, a shortage of professionals, and patient-related obstacles, which can adversely affect recovery and function. Limited access to PT may lead to increased dependence on medications for pain management, highlighting the need for nonpharmacologic options to reduce opioid overprescribing. Low back pain, a leading cause of disability and high medical costs, is a common reason for requiring PT following surgery. Studies have shown that virtual reality (VR)-guided movements can improve motor

A Computer Interface Controlled by Upper Limb Muscles: Effects of a Two Weeks Training on Younger and Older Adults.

As the population worldwide ages, there is a growing need for assistive technology and effective human-machine interfaces to address the wider range of motor disabilities that older adults may experience. Motor disabilities can make it difficult for individuals to perform basic daily tasks, such as getting dressed, preparing meals, or using a computer. The goal of this study was to investigate the effect of two weeks of training with a myoelectric computer interface (MCI) on motor functions in younger and older adults. Twenty people were recruited in the study: thirteen younger (range: 22-35 y

Closed-loop control of trunk posture improves locomotion through the regulation of leg proprioceptive feedback after spinal cord injury.

After spinal cord injury (SCI), sensory feedback circuits critically contribute to leg motor execution. Compelled by the importance to engage these circuits during gait rehabilitation, assistive robotics and training protocols have primarily focused on guiding leg movements to reinforce sensory feedback. Despite the importance of trunk postural dynamics on gait and balance, trunk assistance has comparatively received little attention. Typically, trunk movements are either constrained within bodyweight support systems, or manually adjusted by therapists. Here, we show that real-time control of

Wronger than wrong: Graded mapping of the errors of an avatar in the performance monitoring system of the onlooker.

EEG studies show that observing errors in one's own or others' actions triggers specific electro-cortical signatures in the onlooker's brain, but whether the brain error-monitoring system operates according to graded or discrete rules is still largely unknown. To explore this issue, we combined immersive virtual reality with EEG recording in participants who observed an avatar reaching-to-grasp a glass from a first-person perspective. The avatar could perform correct or erroneous actions. Erroneous grasps were defined as small or large depending on the magnitude of the trajectory deviation fro

Come together: human-avatar on-line interactions boost joint-action performance in apraxic patients.

Limb apraxia (LA) is a high-order motor disorder linked to left-hemisphere damage. It is characterized by defective execution of purposeful actions upon delayed imitation, or verbal command when the actions are performed in isolated, non-naturalistic, conditions. Whether interpersonal interactions provide social affordances that activate neural resources different from those requested by individual action execution, which may improve LA performance, is unknown. To fill this gap, we measured interaction performance, behavioral and kinematic indexes of left-brain damaged patients with/without LA

Three-Dimensional Skin Deformation as Force Substitution: Wearable Device Design and Performance During Haptic Exploration of Virtual Environments.

Virtual reality systems would benefit from a compelling force sensory substitute when workspace or stability limitations prevent the use of kinesthetic force feedback systems. We present a wearable fingertip haptic device with the ability to make and break contact in addition to rendering both shear and normal skin deformation to the fingerpad. A delta mechanism with novel bias spring and tether actuator relocation method enables the use of high-end motors and encoders, allowing precise device control: 10 Hz bandwidth and 0.255 mm RMS tracking error were achieved during testing. Development of a Virtual Approach-Avoidance Task to Assess Alcohol Cravings.

The aim of the present study was to develop a new approach-avoidance task in a virtual environment that could be used to assess the response to virtual alcohol-related situations by heavy social drinkers (HSDs) and light social drinkers (LSDs). Thirty-six male undergraduates (18 HSDs, 18 LSDs) responded to signals when they pulled or pushed a joystick after watching scenes of alcohol- or nonalcohol-related situations in a virtual environment. The HSD group spent more time on moving away from alcohol-related situations than nonalcohol-related situations. We found that the HSD group had difficul

A Platform to Collect Structured Data from Multiple EMRs.

Adoption and use of Electronic Medical Records (EMRs) is continuing to rise across Canada, leading to more data being generated. These data, however, are not being captured in a standardized manner, they are not available for research, surveillance or health system management, and they are not having a real-time impact on healthcare providers at the point of care. Multiple stakeholders, including researchers and system evaluators, require easy access to high quality, structured data. As current EMRs are not able to effectively meet their needs, we engaged multiple stakeholders to assist in des

Automatic stress-relieving music recommendation system based on photoplethysmography-derived heart rate variability analysis.

This paper presents an automatic stress-relieving music recommendation system (ASMRS) for individual music listeners. The ASMRS uses a portable, wireless photoplethysmography module with a finger-type sensor, and a program that translates heartbeat signals from the sensor to the stress index. The sympathovagal balance index (SVI) was calculated from heart rate variability to assess the user's stress levels while listening to music. Twenty-two healthy volunteers participated in the experiment. The results have shown that the participants' SVI values are highly correlated with their prespecified

Noninvasive ventilation: practical advice.

PURPOSE OF REVIEW: This critical review discusses the key points that would be of practical help for the clinician who applies noninvasive ventilation (NIV) for treatment of patients with acute respiratory failure (ARF). RECENT FINDINGS: In recent years, the growing role of NIV in the acute care setting has led to the development of technical innovations to overcome the problems related to gas leakage and dead space. A considerable amount of research has been conducted to improve the quality of the devices as well as optimize ventilation modes used to administer NIV. As a result, also mechani

Progresses in the pursuit of aldose reductase inhibitors: the structure-based lead optimization step.

Aldose reductase (ALR2) is a crucial enzyme in the development of the major complications of diabetes mellitus. Very recently it has been demonstrated that the ARL2 inhibitor, fidarestat, significantly prevents inflammatory signals (TNF-α, LPS) that cause cancer (colon, breast, prostate and lung), metastasis, asthma, and other inflammatory diseases. Currently, fidarestat is in phase III clinical trial for diabetic neuropathy and was found to be safe. Thus the finding of novel, potent ARL2 inhibitors is today more than in the past in great demand as they can pave the way for a novel therapeutic
