Uvulopalatopharyngoglossoplasty (UPPGP) in the treatment of the obstructive sleep apnea syndrome.

Out-patient polysomnography was conducted prior to surgery in 26 male, obstructive sleep apnea patients. Resection of lateral aspects of the tongue base and conservation of the proximal part of the uvula muscle were carried out in addition to conventional palatopharyngoplasty. At a minimum of 6 months after surgery, the patients were given a questionnaire for assessment of treatment. At the same time, polysomnography was repeated for objective evaluation of the results of surgery. More than 90% of the patients were satisfied with the outcome of treatment. The objective results by means of apne…

[The laryngeal mask].

The laryngeal mask (LM) is a new concept developed by Brain. Easily inserted, the LM allows appropriate ventilation without the disadvantages of either the facial mask or the endotracheal intubation with its own complications. The limits of its use concern all the cases of reduction of thoracopulmonary compliance and full stomach. The LM offers no protection against regurgitation and aspiration. Airway obstruction may occur following laryngospasm (light anaesthesia) or down folding of the epiglottis. Trauma to the uvula and the posterior pharyngeal wall have been reported. The LM may be useful…

[Wallenberg's syndrome caused by a brain tumor--a case report and literature review].

we report a patient with Wallenberg's syndrome caused by glioma of the lateral medulla oblongata, and review the literature for Wallenberg's syndrome associated with neoplastic disease. A 46-year-old man was admitted because of progressive dysphagia and vertigo. Neurological examination revealed atypical symptoms and signs of Wallenberg's syndrome on the right side, hypalgesia on the second and third divisions of the left trigeminal nerve, paresis of the right palate and uvula, and ataxia of the right extremities. Although CT showed no abnormality in the posterior fossa, MRI demonstrated a mas…

Airway area by acoustic reflections measured at the mouth.

We tested the hypothesis that features of upper airway and tracheal geometry can be inferred from acoustic reflection data recorded at the mouth. In six subjects we computed inferences of airway cross-sectional area vs. distance and compared them with measurements obtained from orthogonal radiographic projections of the trachea. The acoustic data show local area maxima at the uvula and hypopharynx and local minima at the oropharynx and the glottis. With subjects breathing air the inferred tracheal areas markedly exceeded the radiographic measurements. With subjects breathing 80% He-20% O2 ther…

Visual suppression of vestibular nystagmus after cerebellar lesions.

Visual suppression of calorically induced vestibular nystagmus was observed following discrete lesions of various structures in the cerebell … he dentate nucleus resulted in a very short term loss of suppression. Extirpation or lesions of the uvula, vermis, para-flocculus, cerebellar cortex, or the fastigial or interpositus nuclei had no observed effect on the visual suppression of vestibular nystagmus. The results of this study suggest that the flocculus and nodulus function as intermediators …

[2000 year history of tonsillectomy. Images from the history of otorhinolaryngology, highlighted by instruments from the collection of the German Medical History Museum in Ingolstadt].

BACKGROUND: The etymology of the anatomical terms and their use in history are elucidated: "Tonsil" (from Latin tonsa = the oar) in use sinc … S: Precursors of special instruments for tonsillectomy were instruments designed for shortening the uvula: uvulotomy. Paré (1564) and Scultetus (1655) devised instruments that permitted placing a thread shaped like a snare around the uvula and cutting it off by strangulation. Hildanus (1646), Scultetus (1655) and Heister (1763) presented an instrument o …

Predictors of uvulopalatopharyngoplasty outcomes in patients with supine predominant positional obstructive sleep apnea: a prospective observational study.

BACKGROUND: It is uncertain which factors substantially influence outcomes after uvulopalatopharyngoplasty (UPPP) in patients with supine predominant obstructive sleep apnea (POSA). OBJECTIVE: To explore the predictors of UPPP outcomes in POSA patients. METHODS: A total of 108 patient(52 positional patients (POSA) and 56 nonpositional patients(NPP)), who underwent the revised uvulopalatopharyngoplasty (H-UPPP), were retrospectively studied. The pre-operative information of these patients, including polysomnography (PSG), and upper airway CT, were collected for analysis. RESULTS: No differen

Palatopharyngoplasty and Palatal Anatomy and Phenotypes for Treatment of Sleep Apnea in the Twenty-first Century.

Successful palatopharyngoplasty is critical for successful sleep apnea surgery. Traditional uvulopalatopharyngoplasty was primarily excisional, whereas newer techniques, such as expansion sphincterpharyngoplasty, are more reconstructive. Studies of flow dynamics even demonstrate that the effectiveness of maxillofacial surgery is significantly mediated through stiffening and enlargement of the lateral retropalatal and pharyngeal airway. The current modified technique of uvulopalatopharyngoplasty/expansion sphincteroplasty aims to maximize relocation and tension on the palatopharyngeus muscle, o

Modified sling snoreplasty: double triangle shaped suture uvulopalatopexy.

Following the uvulopalatopharyngoplasty and laser assisted uvulopalatoplasty as treatments for mild obstructive sleep apnea syndrome (OSAS) originating from the soft palate, palatal stiffening operations began to be described as causing less morbidity, interfering less with velopharyngeal and mucosal functions, and being applicable in outpatient settings. We present modified uvulopalatopexy procedure which can be used in mild OSAS patients is a simple, minimally invasive and effective new method. In this article, we present a modified form of sling snoreplasty method and two cases in whom this

Triangular uvulopalatal flap.

The aim of this technique is shortening and tightening the soft palate to increase the retropalatal upper airway patency. Postoperative scarring may also stabilize the soft palate and thus prevent vibration and snoring sound generation at this site.

Mid-line swelling of the palate.

Sarcoidosis is a multi-system, non-caseating granulomatous disease of unknown aetiology that may affect any organ. The oral involvement of sarcoidosis is rare and usually an initial manifestation of the disease. In this case report the authors present a 25-year-old African-American woman with palatal sarcoidosis treated successfully with intra-lesional corticosteroid injections. The oral manifestations of sarcoidosis are relatively uncommon and may be the only manifestation of the disease. Suspected cases of oral sarcoidosis should be biopsied and subsequently referred to a physician to rule o

[Surgical therapy of obstructive sleep apnea syndromes: results of the Ulm treatment program].

The article reports on the results obtained by uvulopalatopharyngoplastic surgery (= UPPPS) in 31 patients suffering from obstructive sleep apnoea syndrome (= OSAS) and in 9 patients with habitual or obstructive snoring. All patients were subjected to thorough preoperative examination including rhinometry, nasopharyngeal video-endoscopy, radiocephalometry and polysomnography. In 8 out of 9 (89%) of the habitually snoring patients and in 17 of 31 (55%) of OSAS patients, surgery was successful. In these patients, a postoperative respiratory disturbance index (RDI) of less than 10 was recorded, w

Simple treatment for snoring also a means of prediction of uvulopalatopharyngoplasty success?

The old Indian literature describes a technique known as Sutra-Neti. That is, passing a catheter or similar material through the nose and out of the mouth as a means of clearing the airway. One of our patients adapted this method, tying the catheter end to end, in order to control his severe snoring and obstructive sleep apnoea. This was effective for several months. He subsequently responded well to a uvulopalatopharyngoplasty.

Upper-airway surgery for treating obstructive sleep apnea. Measuring and predicting success.

The effectiveness of upper-airway surgery in treating obstructive sleep apnea was examined in 37 patients. All patients received a uvulopalatopharyngoplasty. Some also received tonsillectomies, septoplasties, and turbinectomies. The success of the operations was evaluated along three dimensions: a decrease in the number of apneas per sleep hour, a reduction in maximum nocturnal oxygen desaturation, and a decrease in daytime somnolence. Over 80% of the patients improved on at least one of these factors following surgery. However, postoperative intercorrelations among these three variables were
