Is surgery the gold standard in the treatment of obstructive hypertrophic cardiomyopathy?

OBJECTIVES: Hypertrophic cardiomyopathy is a complex and relatively common genetic cardiac disease and has been the subject of intensive scrutiny and investigation for over 40 years. The aim of this non-randomized cohort study was to compare subjective and objective outcomes in hypertrophic cardiomyopathy patients undergoing drug therapy, surgical myotomy-myectomy, dual-chamber pacing and alcohol septal ablation. METHODS: We examined 194 patients: 103 with non-obstructive hypertrophic cardiomyopathy and 91 with obstructive hypertrophic cardiomyopathy. All the patients with a non-obstructive f

Staged Surgical Management of Anatomically Corrected Malposition of Great Arteries.

A nine-month-old girl diagnosed with anatomically corrected malposition (atrioventricular discordance, ventriculoarterial concordance, and malposed great arteries) complicated by multiple ventricular septal defects (VSD) and multifactorial left ventricular outflow tract obstruction (LVOTO) presented for management after pulmonary artery banding. She underwent interim palliation in the form of bilateral cavopulmonary connections, a modified Damus-Kaye-Stansel-type anastomosis, and subsequent staged one-and-a-half ventricle repair (1.5 repair) at the age of three years in the form of VSD closure

In Thickness and in Health: Delayed-Onset Pompe Disease Resembling Hypertrophic Cardiomyopathy.

Delayed-onset Pompe disease is an extremely rare genetic disease in adults and should be considered in patients with severe left ventricular hypertrophy with left ventricular outflow tract (LVOT) obstruction due to its resemblance to hypertrophic cardiomyopathy (HCM). A case of a 58-year-old woman with no known comorbidities with excellent functional capacity presented because of the gradual onset of exertional dyspnea associated with fatigue, diminished exercise tolerance, occasional exertional chest tightness, and lightheadedness. The electrocardiogram showed left ventricular hypertrophy wit

Congenital mitral-aortic discontinuity.

Mitral-aortic (M-A) discontinuity is an uncommonly observed congenital malformation. From the embryologic point of view, it is a failure of the superior endocardial cushion of the A-V canal reach and insert into the aortic root (the mitral-aortic fibrous trigone). It should be differentiated from aorto-left ventricular discontinuity, a complication of endocarditis with excavating abscesses without boundaries. In congenital mitral-aortic discontinuity, the anterior leaflet of the mitral valve fails to insert into the aortic root. We report our experience with 2 patients. In patient 1, the sever

Yasui procedure for an interrupted aortic arch type C with an aberrant right subclavian artery from the pulmonary artery: right subclavian artery-free graft technique.

A male neonate (2.5 kg) who presented with an interrupted aortic arch type C, hypoplasia of the aortic valve and left ventricular outflow tract obstruction received bilateral pulmonary artery bands as a first step of a hybrid interim palliation. Due to an intimal tissue flap at the origin of the left common carotid artery and a high-risk situation for PDA stenting, a complete early correction was undertaken. For full correction, the large curvature of the aortic arch was reconstructed using the aberrant right subclavian artery as a free graft by implanting it between the right and left common

High maternal selenium levels are associated with increased risk of congenital heart defects in the offspring.

OBJECTIVE: To examine whether maternal hair or cord blood selenium levels are associated with infant congenital heart defects (CHDs). METHOD: A hospital-based case-control study was performed with samples collected from 2010 to 2014. Selenium levels in maternal hair and fetal cord serum were measured using Inductively Coupled Plasma-Mass Spectrometry (ICP-MS). Data from medical records of infants with CHDs were collected. Logistic regression analysis was employed to examine the independent association between maternal selenium exposure and fetal CHD. RESULTS: In total, 888 pregnant women wer

Clinical results of arterial switch operation for double-outlet right ventricle with subpulmonary VSD.

OBJECTIVE: An arterial switch operation is considered a good alternative for the repair of double-outlet right ventricle (DORV) with atrioventricular concordance connection and subpulmonary ventricular septal defect (VSD) when intraventricular rerouting is not feasible. The clinical results of an arterial switch operation with ventricular septal defect closure for this anomaly were studied. METHODS: Between 1986 and 1997, 27 patients ranging from 10 days to 5 years of age (mean 0.4 years) underwent an arterial switch operation with ventricular septal defect closure for the correction of doubl

Mavacamten Versus Alcohol Septal Ablation in Obstructive Hypertrophic Cardiomyopathy: An Echocardiography-Derived Pressure-Volume Analysis.

BACKGROUND: Obstructive hypertrophic cardiomyopathy (oHCM) is characterized by left ventricular (LV) outflow tract obstruction, which increases afterload and chronically activates the Anrep response, a compensatory (afterload-driven) state of hyperdynamic systole, prolonged systolic ejection time, and increased myocardial workload. We investigated whether the myosin inhibitor mavacamten reverses this state, comparing its effects to the anatomic relief achieved by alcohol septal ablation. METHODS: Thirty-six patients with symptomatic oHCM were treated with mavacamten. Of these, 29 who achieved

Left ventricular systolic dysfunction during exercise and dobutamine stress in patients with hypertrophic cardiomyopathy.

OBJECTIVES: We sought to characterize stress-induced left ventricular systolic dysfunction in patients with hypertrophic cardiomyopathy (HCM). BACKGROUND: Myocardial ischemia and diastolic dysfunction occur in patients with HCM. We hypothesized that, in the setting of transient myocardial ischemia, left ventricular systolic dysfunction occurs during exercise and dobutamine stress. METHODS: We studied 39 patients with HCM but without obstructive symptoms at rest or coronary artery disease. A continuous ventricular function monitor equipped with cadmium telluride detectors (VEST) was used to e

Subaortic stenosis following anatomic correction for transposition of the great arteries.

The authors present a case report of a 12-year-old boy with diagnosis of transposition of the great arteries and ventricular septal defect, who underwent an arterial switch operation plus closure of the septal defect. On follow-up, left ventricular outflow tract obstruction was detected, initially mild. Last year, he started complaining of fatigue on exercise. Severe subaortic stenosis was diagnosed and surgical repair was performed with good results. Subaortic stenosis is a rare complication on follow-up of patients who have undergone an arterial switch operation, particularly those with tran

Lipomatous hypertrophy of the interatrial septum: A benign cardiac mass.

Lipomatous hypertrophy of the interatrial septum (LHIS) is a rare but benign cardiac tumor that can be found on cardiac imaging such as echocardiogram, or during surgery or an autopsy. Cardiac MRI is the best imaging modality to determine the borders of the tumor and its extension into the intraventricular septum and ventricular free wall. Patients require close monitoring because LHIS may cause right or left outflow tract obstruction or superior vena cava obstruction, requiring cardiac surgical intervention. This article describes a patient with LHIS who underwent cardiac surgery because of h

Takotsubo cardiomyopathy with LVOT obstruction in a case of STEMI: a rare cause of peri-PCI hypotension.

Takotsubo cardiomyopathy (TCM) associated with left ventricular outflow tract (LVOT) obstruction in the event of an ST-elevation myocardial infarction (STEMI) is a rare cause of hypotension during percutaneous coronary intervention (PCI). Herein, we describe a 57-year-old woman who presented with STEMI and underwent PCI. She developed hypotension which worsened during inotropic therapy. Echocardiography revealed evidence of LVOT obstruction in the setting of TCM. Therefore, inotropic support was promptly discontinued. Beta blockers and phenylephrine were rapidly administrated, resulting in imp

BATMAN-Savior or vigilante?

BATMAN is a new technique to prevent left ventricular outflow tract (LVOT) obstruction from transcatheter mitral valve replacement (TMVR) by deploying the transcatheter heart valve from the apex through a perforation of the anterior mitral valve leaflet. The risks of uncontrolled balloon dilatation of the anterior mitral valve leaflet include extension of the tear superiorly into the aorto-mitral curtain or laterally to avulse the trigone from the annulus. Percutaneous laceration of the anterior mitral leaflet, pre-emptive alcohol septal ablation, and transatrial leaflet resection are alternat

Rastelli operation for transposition of the great arteries with ventricular septal defect and pulmonary stenosis.

BACKGROUND: The optimal surgical treatment of patients with transposition of the great arteries, ventricular septal defect, and pulmonary stenosis is controversial. Although the Rastelli operation has been standard surgical management of this lesion, aortic root translocation with right ventricular outflow tract (RVOT) reconstruction (Nikaidoh) and the pulmonary artery translocation (Lecompte) or REV (réparation a l'étage ventriculaire) are surgical alternatives more recently introduced to treat this complex lesion. This report reviews our 20-year experience with the Rastelli procedure and att

Favorable cardiac remodeling in response to treatment in obstructive hypertrophic cardiomyopathy: a current appraisal.

Hypertrophic cardiomyopathy is a heterogenous genetic condition caused by myocyte disarray with varying degrees of interstitial fibrosis. Traditional management strategies focused on symptom mitigation and sudden cardiac death prevention among high-risk patients. The only disease-modifying treatments have historically been septal reduction therapy or heart transplantation. However, cardiac myosin inhibitors have emerged as promising novel pharmacotherapies. Emerging evidence indicates that cardiac myosin inhibitors as well as surgical myectomy result in not only symptomatic benefit and reducti

Comprehensive evaluation of hypertrophic cardiomyopathy: European Journal of Heart Failure expert consensus document.

Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiac disease and a leading cause of heart failure and sudden cardiac death (SCD) in young adults. Given its complex pathophysiology, phenotypic diversity, and rapidly evolving therapeutic landscape, a structured and multidisciplinary approach to care is essential. This manuscript outlines a six-pillar framework to standardize and optimize evaluation and management of HCM. The proposed model organizes HCM care into six key domains. (i) Establishing the correct diagnosis, which requires differentiation between sarcomeric HCM and p

Models of congenital heart disease in fetal lambs.

Intracardiac flow patterns were chronically altered by partially obstructing left ventricular (LV) inflow or outflow in midgestational fetal lambs. Physiological measurements of the fetal circulation were made serially through indwelling catheters and the use of radioactive microspheres. With LV inflow obstruction, mean LV output (LVO) decreased to 30% of control (P less than 0.01). Within seven days, the LV/right ventricular (RV) weight ratio decreased to 70% of control (P less than 0.01), and the mean LV/RV chamber volume decreased to less than one-half of control (P less than 0.001), simula

Advantages of the caudocranial left anterior oblique left ventriculogram in adult heart disease.

Biplane axial left cineventriculography represents the most accurate diagnostic technique for evaluating acquired and congenital heart disease. However, data have accumulated to indicate that without angled views of the left ventricle, the diagnosis will be incomplete and inaccurate in a significant number of patients. Left ventriculography is the acknowledged standard for left ventricular performance. However, comparison of the conventional or nonangled left anterior oblique left ventriculogram with the angled views of the left ventricle obtained with either two dimensional ultrasound or radi

Subaortic obstruction in double outlet right ventricles. Surgical considerations for anatomic repair.

BACKGROUND: Subaortic obstruction is one of the risk factors for anatomic repair of double outlet right ventricles (DORV). A comprehensive approach to such lesions has been developed in our institution since 1981. This retrospective work analyzes the results of this approach. METHODS AND RESULTS: Between January 1981 and September 1992, 30 patients aged 15 days to 15 years (mean, 44.8 months) underwent repair of a DORV associated with subaortic obstruction. Eighteen patients had a palliative procedure before complete repair. The ventricular septal defect (VSD) was subaortic in 15 patients, do
