[Diagnosis and treatment of horizontal canal benign paroxysmal positional vertigo].

OBJECTIVE: To explore effective methods for the diagnosis and treatment of horizontal-canal benign paroxysmal positional vertigo (HC-BPPV). METHODS: Medical records from nine patients with HC-BPPV, treated between July 1996 and March 2000, were retrospectively analyzed. Data of complete history, audiograms, positional tests and neuro-otological examinations were collected. All patients were treated with a particle repositioning maneuver called the "barbecue rotation" which starts with the patient in the supine position and consists of three 90-degree head rotations towards the unaffected ear.…

Epley's manoeuvre: treatment of choice for benign paroxysmal positional vertigo.

Benign paroxysmal positional vertigo (BPPV) is one of the commonest causes of peripheral vestibular disorders. In this prospective study 21 patients with BPPV were treated by Epley's manoeuvre. All patients had an immediate improvement in their symptoms. Recurrence was noted in three patients who required further treatment sessions with resultant improvement in all. However, one patient who originally had suffered from Ménière's disease involving the same ear for more than 14 years developed another recurrence which was treated successfully by further application of Epley's manoeuvre. This stu…

A relationship between autoimmune thyroiditis and benign paroxysmal positional vertigo?

Benign paroxysmal positional vertigo (BPPV) is a labyrinthine disorder with a typical behavior: intense crises of rotational vertigo induced by postural changes of the head, with short duration and usually good responsiveness to rehabilitative maneuvers. This phenomenon is thought to be subsequent to the movement of floating particles in the labyrinthine fluids, which can provoke gravitational stimulations. In order to conduct a metabolic and autoimmune examination, 70 patients affected by BPPV were examined. In 34 cases (48.5%) autoimmune alterations were found: in 19 cases (27.1%) the level …

Benign paroxysmal vertigo in childhood: a long-term follow-up.

Benign paroxysmal vertigo in children is characterized by sudden attacks of vertigo lasting seconds or minutes. During the attack, the child has nystagmus and is unable to stand without support. Initially, the attacks are frequent, later slowly disappearing. Nineteen children who were diagnosed in 1975-1981 participated in a follow-up study. Sixteen of them were examined with audiometry and electronystagmography. Age at onset was from 5 months to 8 years, and the symptoms disappeared after 3 months to 8 years. The follow-up was performed 13 to 20 years after diagnosis. Twenty-one percent devel…

[Repositioning maneuver in benign paroxysmal vertigo of horizontal semicircular canal].

The introduction of the Lempert or Baloh "barbecue" maneuver in clinical practice has resolved many cases of paroxysmal positional vertigo of the horizontal semicircular canal. Unfortunately, this maneuver is rather difficult to perform in elderly or overweight patients and in those with reduced mobility. In such cases the Vannucchi "forced position" is the maneuver of choice. Diagnostic confirmation is, however, still a problem. The authors feel diagnosis is only confirmed when the positional nystagmus has been converted, inverted or disappears after the physical therapy maneuver. For this re…

Eighth cranial nerve lipoma manifesting as intractable vertigo--Case report.

A 49-year-old female presented with an extremely rare lipoma of the cerebellopontine angle (CPA) manifesting as intractable vertigo and tinnitus. Subtotal removal of the lipoma was performed to debulk the tumor and decompress the cranial nerve VIII. Her complaints were resolved without new neurological deficit. We recommended surgery in patients with symptomatic CPA lipomas for diagnosis, debulking, and decompression of cranial nerves to relieve the symptoms.

Benign paroxysmal vertigo of childhood: a long-term follow-up.

We examined clinical aspects of Benign Paroxysmal Vertigo (BPV) in infancy and its most frequent differential diagnosis, in particular analogies and differences with forms of "migrainous vertigo" (MV) of later onset. During a long-term follow-up of 7 cases of BPV, diagnosed according to the Basser criteria, 5 of 7 BPV cases spontaneously resolved and 6 of 7 patients later developed migraine and other migraine-related symptoms. This course differs from that described for MV only in the age of onset of headache and in the chronological relationship with vertigo. The authors suggest that BPV can …

[Surgical and physical therapy treatment of benign paroxysmal positional vertigo].

Two treatments for the benign paroxysmal positional vertigo are presented and evaluated in this article. The first type of treatment is the surgical section of the posterior ampullary nerve according to Gacek and the second is physiotherapeutic by the manoeuvre of Semont. The section of the posterior ampullary nerve has been realised between 1980 and 1986 on 5 patients suffering from persistent and disabling positional vertigo. All 5 patients were relieved from their vertigo immediately after the operation. One patient developed a post-operative hearing loss. The physiotherapeutic manoeuvre of…

[Benign paroxysmal positional vertigo: clinical aspects and medico-legal considerations].

Benign paroxysmal positional vertigo (BPPV) is quite frequently found as an otological consequence of cranial traumas; thus leading to medico-legal implications. An attempt has herewith been made to evaluate these implications on the basis of data available. Therefore, 158 patients suffering from BPPV were examined between 1984-1986. Of these 23% of the total were post-traumatic forms (of medico-legal interest). Among patients suffering from post-traumatic vestibular disturbances in the broad sense, the incidence of BPPV proved to be greater in the ages ranging from 50 to 70. In those under 30…

Benign positional vertigo: clinical and oculographic features in 240 cases.

We report the clinical and oculographic features in 240 patients with benign positional vertigo (BPV). In each case, after a rapid position change from the sitting to head-hanging position, a stereotyped torsional paroxysmal positional nystagmus was visually observed and recorded with electronystagmography (ENG). The mean age of onset was 54 years, with a range of 11 to 84 years. In slightly more than one-half of the cases (122/240) a likely diagnosis was determined. The most common identifiable causes were head trauma (17%) and viral neurolabyrinthitis (15%). Females outnumbered males approxi…

Neuro-otological study of positional vertigo caused by head injury.

A neuro-otological examination was given to 22 patients with paroxysmal positional vertigo within 1 week of head injury. In all cases, reverse rotatory positional nystagmus accompanied by a vertiginous sensation was observed in the positional nystagmus test. No relationship between the affected side and the direction of eye movement in the head-hanging position was found in the positional nystagmus test. From the prognosis and the nystagmus findings, the cause of the vertigo was thought to be partial damage to the inner ear. The clinical course was generally favorable, but recovery was slower …

Chronic vertigo and dizziness signal unmet needs in stroke recovery.

INTRODUCTION: Health-related quality of life (HRQoL) in stroke survivors often remains impaired. Chronic vertigo and dizziness are common but frequently overlooked symptoms. We investigated their impact on HRQoL and characterized symptom patterns. PATIENTS AND METHODS: Patients from a prospective stroke cohort study were assessed during hospitalization and followed up after three and twelve months. We evaluated functional outcomes using the modified Rankin Scale (mRS) and National Institute of Health Stroke Scale (NIHSS), and measured HRQoL with the Stroke Impact Scale (SIS). The relationship…

Causal Relationship Between Hypertension And Vertigo: A Mendelian Randomization Study.

INTRODUCTION: Current genetic research on the relationship between hypertension and vertigo is limited, and traditional observational studies cannot establish a causal relationship due to design limitations, particularly regarding whether hypertension acts as a causal risk factor for specific vertigo subtypes, such as benign paroxysmal positional vertigo (BPPV). METHODS: This study employed a two-sample MR approach to infer causal relationships via genome- wide association study (GWAS) data, thereby addressing the limitations of traditional observational studies. In addition to analyzing the …

Beyond Vertigo: Vestibular, Aural, and Perceptual Symptoms in Vestibular Migraine.

PURPOSE: To review the vestibular, aural, and perceptual symptoms of vestibular migraine (VM) that may present alongside vertigo. RECENT FINDINGS: Increased research attention to the wide spectrum of symptoms presenting in VM patients has improved understanding of this disorder, with recent identification of five different VM phenotypes. Research into the clinical overlap between VM and other chronic vestibular syndromes such as persistent postural-perceptual dizziness and mal-de-debarquement syndrome reveals a range of vestibular symptoms and hints at pathophysiological connections between m…

IL-6 Serum Levels in COVID-19 Patients With Vertigo.

Introduction Dizziness and vertigo represent well-established symptoms of COVID-19. An overexpression of cytokines, a condition often described with the term "cytokine storm" or "hypercytokinemia", is a key characteristic of SARS-Cov-2 infection and plays a pivotal role in disease progression and prognosis. Among them, IL-6 is of major importance.  Purpose The purpose of this study is to investigate any probable IL-6 serum titer difference in COVID-19 patients with vertigo (V+) or without vertigo (V-) admitted to the COVID-19 internal medicine departments of Attikon University Hospital, Athens…
