Surgical approaches for stage I and II thymoma-associated myasthenia gravis: feasibility of complete video-assisted thoracoscopic surgery (VATS) thymectomy in comparison with trans-sternal resection.

Complete resection could be achieved in virtually all myasthenic patients with Masaoka stage I and II thymoma using the trans-sternal technique. Whether this is appropriate for minimally invasive approach is not yet clear. We evaluated the feasibility of complete video-assisted thoracoscopic surgery (VATS) thymectomy for the treatment of Masaoka stage I and II thymoma-associated myasthenia gravis, compared to conventional trans-sternal thymectomy. We summarized 33 patients with Masaoka stage I and II thymoma-associated myasthenia gravis between April 2006 and September 2011. Of these, 15 patie

The Impact of the Number of Ports on Perioperative Outcomes Following Video-Assisted Thoracoscopic Surgery for Non-Small-Cell Lung Cancer: A Systematic Review and Meta-Analysis.

Video-assisted thoracoscopic surgery (VATS) has emerged as a less invasive technique for treating resected lung cancer compared with open surgery. In recent years, the uniportal VATS technique has gained popularity for lung resection in small nodules and ground glass lesions. However, it remains unclear whether single-port VATS offers more favorable perioperative outcomes than multi-port approaches. This study aims to evaluate the perioperative outcomes of single-port versus 2-port and 3-port VATS in patients with early-stage non-small cell lung cancer. A literature search was conducted across

Are the Efficacy and Safety of Chest Tubes in Uniportal Video-Assisted Thoracic Surgery Related to the Level of Intercostal Space Insertion or to the Drain Type? A Prospective Multicenter Study.

OBJECTIVES: The aim of this study is to evaluate if the efficacy and safety of chest tube placement are influenced by the level of intercostal space insertion (uniportal VATS vs. biportal VATS) or by the type of drain employed (standard vs. smart coaxial drain). METHODS: Data on patients who underwent either uniportal or biportal VATS upper lobectomies with lymphadenectomy were prospectively collected in three European centers. The uniportal VATS group with a 28 Fr standard chest tube (U-VATS standard) was compared with the uniportal VATS group with a 28 Fr smart drain (U-VATS smart), and U-V

Comparison of analgesic efficacy of the conventional approach and mid-transverse process to pleura approach of the paravertebral block in video-assisted thoracoscopy surgeries: A randomised controlled trial.

BACKGROUND AND AIMS: The paravertebral block (PVB) is an effective alternative to thoracic epidural analgesia for post-operative analgesia in thoracic surgeries. Despite the use of ultrasound in PVB, the search for a safer approach continues. This study was conducted to compare the analgesic efficacy of conventional and mid-transverse process to the pleura (MTP) approach of the PVB. METHODS: Forty patients aged between 18-60 years, posted for video-assisted thoracoscopic surgery, were enroled for this study. Patients were randomised into two groups using a random number table, and group alloc

Efficacy of Nonpharmacological Interventions in Acute Postoperative Pain of Patients with Lung Cancer Undergoing Video-Assisted Thoracoscopic Surgery: A Protocol for Systematic Review and Network Meta-Analysis.

BACKGROUND: Acute postoperative pain is one of the most common and painful complications of lung cancer patients undergoing video-assisted thoracoscopic surgery, which seriously affects the health and quality of life of patients. Although using medication for analgesia is the standard, their risk of adverse events still cannot be ignored. Non-pharmacological interventions are increasingly studied, but their relative efficacy in a given population remains unclear. The purpose of this study protocol is to compare the efficacy of non-pharmacological interventions in acute postoperative pain of pa

A 5-Item Prediction Model Incorporating Pain With Neuropathic Characteristics at 2 Weeks for Chronic Postsurgical Pain After Video-Assisted Thoracoscopic Lung Surgery: A Prospective Cohort Study.

BACKGROUND: Chronic postsurgical pain (CPSP) after video-assisted thoracoscopic surgery (VATS) is common, yet clinicians lack tools to identify high-risk patients at routine follow-up. We hypothesized that pain with neuropathic characteristics at 2 weeks, signifying central sensitization, could stratify risk and guide precision analgesia. METHODS: This prospective cohort study enrolled 500 adults undergoing VATS lung resection. Two logistic models were developed: one using variables on postoperative day 1 (POD1), and another adding neuropathic pain features on POD14. The primary outcome was C

Liposomal Bupivacaine vs. Plain Bupivacaine with Dexamethasone for Rhomboid Intercostal Block in the Management of Postoperative Pain After Video-Assisted Thoracoscopic Surgery: A Randomized Non-inferiority Trial.

INTRODUCTION: Effective and sustained postoperative analgesia is essential to enhance recovery after video-assisted thoracoscopic surgery (VATS). Liposomal bupivacaine, a multivesicular formulation enabling extended local anesthetic release, offers a mechanistic advantage over conventional agents. However, prior comparisons with adjuvant-enhanced regimens such as plain bupivacaine plus dexamethasone were confounded by pharmacological and dose inequivalence. This equivalence-dose randomized controlled trial evaluated whether liposomal bupivacaine provides non-inferior analgesia to the standard

Effects of opioid-free anaesthesia on postoperative nausea and vomiting in patients undergoing video-assisted thoracoscopic surgery (OFA-PONV trial): study protocol for a randomised controlled trial.

BACKGROUND: Postoperative nausea and vomiting (PONV) is a common complication after general anaesthesia and is associated with morbidity and prolonged length of stay. Growing evidence suggest that opioid-free general anaesthesia (OFA) may reduce PONV in various surgical settings. We aim to evaluate the efficacy of OFA on the incidence of PONV compared with opioid-based anaesthesia among adults undergoing thoracoscopic surgery. METHODS: This is a prospective, single-centre, randomised controlled trial comparing OFA and opioid-based anaesthesia for thoracoscopic surgery. A total of 168 adults w
