A fast and sensitive method by high performance liquid chromatography for the quantification of the methylxanthine metabolites of caffeine in human plasma is described. Plasma is extracted with 20% isopropanol in chloroform and the evaporated residue is redissolved and chromatographed on silica gel using dichloromethane containing 2.2% of a solution of methanol-ammonium formateformic acid (100:0.02:0.017). A gas chromatographic method is presented for the measurement of theophylline in serum. Complications of tattooing occur in 2% of cases and include foreign body granulomas, for which treatment options are limited and often ineffective. Allopurinol, a xanthine oxidase inhibitor, is used to treat granulomatous reactions, but often leads to systemic side effects. Two recent case reports have described topical application as better tolerated. Herein, we report a patient who developed a granulomatous reaction to permanent eyebrow make-up, for which topical allopurinol achieved a satisfactory result.

Gout is a common form of inflammatory arthritis characterized by the deposition of MSU in the joints and surrounding tissues, which results in inflammation and recurrent painful attacks. Currently, xanthine oxidase inhibitors, i.e., Allopurinol and Febuxostat, are used in the therapy. Recently, nanoparticles (NPs) containing metal oxides and non-metal oxides have also been developed to better manage gout. This comprehensive review summarizes the pathophysiology of gout, currently used drugs in the treatment, followed by emerging therapies for gout.

In Bufo arenarum, as in other amphibian species, nuclear maturation--characterized by germinal vesicle breakdown (GVBD) and meiosis resumption--is induced by progesterone. Seasonal variations in the sensitivity to progesterone were observed in Bufo arenarum oocytes. The gonadotropin FSH and drugs such as dbcAMP and xanthine did not affect nuclear maturation per se, although they acted as negative modulators of progesterone-induced nuclear maturation. Similar results to FSH in nuclear maturation were obtained with 17 beta-estradiol.

Mo(VI) and Mo(V) salts both react selectively with Hantzsch esters to produce substitute pyridines in good-to-excellent yield (75-99%). The remarkable reactivity and selectivity of MoOCl(4) under reflux of acetonitrile and MoCl(5) in dichloromethane at room temperature encouraged us to propose that molybdenum-containing enzymes (such as xanthine or aldehyde oxidase) also participate to some degree in the metabolism of 1,4-dihydropyridine drugs in the liver analogous to NADH in the respiratory chain.

Morphological and biochemical (NAD, NADF, xanthine oxidase, lipid peroxidation products) disorders at liver tissue samplings were studied at 60 patients with hepatic echinococcosis. It is demonstrated that hydatid cysts of both lobe of liver are associated with high level of lipid peroxidation products and low level of respiratory coenzymes at liver tissue. It is concluded that morphological and biochemical examinations of liver tissue samplings are the main methods for early diagnosis of hepatic failure at patients with hepatic echinococcosis.

Self-injurious behavior is a common clinical problem in children with Lesch-Nyhan syndrome, an X-linked disorder of purine metabolism. This behavior is not observed in other conditions associated with increased serum concentrations of uric acid, hypoxanthine, and xanthine. Various neurotransmitters appear to play a pivotal role in self-injurious behavior. The authors present a patient with Lesch-Nyhan syndrome, whose self-injurious behavior was effectively treated with gabapentin, and discuss possible mechanisms of action.

In vitro deformability of red blood cells from SS-genotype patients in vasoocclusive crisis of sickle cell disease was significantly improved by the in vitro addition of the xanthine derivative pentoxifylline (Trental). Monkeys transfused with discarded sickle cell crisis blood showed aggregation of red cells and platelets on metal screens inserted into arterio-venous anastomoses. This phenomenon could be prevented by pretreatment of the animals with 24 mg/kg of pentoxifylline. The possible mechanisms of action are discussed.

A computational approach to simulate the formation of possible imprinted polymers in acetonitrile solution for theophylline (THO) is proposed, using combined molecular dynamics (MD), molecular mechanics (MM), docking and site mapping computational techniques. Methacrylic acid (MAA) and methylmethacrylate (MMA) monomers are used to simulate possible homo and copolymer structures. The model is able predict binding affinity and selectivity when considering THO analogues, such as caffeine, theobromine, xanthine and 3-methylxanthine. Comparison with available experimental data is proposed.
