Carcinomatous meningitis appearing as acoustic neuromas: two cases.

BACKGROUND: For acoustic neuromas, stereotactic radiotherapy (radiosurgery or stereotactic fractionated radiotherapy) has been established as an important alternative to microsurgery. In most cases initial symptoms are slow progression of unilateral hearing loss, tinnitus or vertigo or acute hearing loss with vertigo. MRI scan shows a contrast-enhancing tumor within the inner auditory channel. If the patient undergoes primary radiotherapy, diagnosis is usually not verified histologically. Therefore, careful evaluation of the medical history is mandatory despite a typical appearance on the MRI

Safety and efficacy of fractionated stereotactic radiotherapy for acoustic neuromas.

BACKGROUND: The treatment of acoustic neuromas (AN) has historically involved surgical excision or stereotactic radiosurgery, with a relatively limited number of reports available describing the use of fractionated stereotactic radiotherapy (FSRT). To enhance the existing knowledge regarding the safety and efficacy of this treatment modality, we describe our initial experience with FSRT for AN. METHODS: From 1999-2005, 20 patients (12F, 8M) with AN underwent FSRT. All patients were treated using the Radionics X-Knife 4.0 3D planning system, receiving 54 Gy in 1.8 Gy daily fractions with a pre

The prevalence and clinical course of patients with 'incidental' acoustic neuromas.

CONCLUSIONS: This study suggests that among patients diagnosed with 'incidental' acoustic neuromas (ANs), a substantial portion are discovered incidentally. Small and medium-sized ANs that are found incidentally may have a more benign nature, and may be less likely to require interventions. OBJECTIVE: To estimate the prevalence of ANs, and to compare the prognosis and progression of the disease between those diagnosed incidentally verse symptomatically with an AN. PATIENTS AND METHODS: This was a retrospective evidence-based case series of patients with AN treated at a tertiary medical cente

Spatial hearing disability after acoustic neuroma removal.

OBJECTIVES/HYPOTHESIS: Previous studies on hearing loss (HL) after acoustic neuroma removal concentrate mainly on pure-tone hearing results rather than hearing disability. Our objectives were to use the Speech, Spatial and Qualities of Hearing scale (SSQ), a comprehensively validated questionnaire, to characterize and quantify the auditory disabilities that patients experience with a profound unilateral HL after acoustic neuroma removal. STUDY DESIGN: Forty-four patients with profound unilateral HL after acoustic neuroma surgery completed the SSQ. Their findings were compared with those of a

Acoustic field of a ballistic shock wave therapy device.

Shock wave therapy (SWT) refers to the use of focused shock waves for treatment of musculoskeletal indications including plantar fascitis and dystrophic mineralization of tendons and joint capsules. Measurements were made of a SWT device that uses a ballistic source. The ballistic source consists of a handpiece within which compressed air (1-4 bar) is used to fire a projectile that strikes a metal applicator placed on the skin. The projectile generates stress waves in the applicator that transmit as pressure waves into tissue. The acoustic fields from two applicators were measured: one applica

Acoustic roles of the laryngeal cavity in vocal tract resonance.

The acoustic effects of the laryngeal cavity on the vocal tract resonance were investigated by using vocal tract area functions for the five Japanese vowels obtained from an adult male speaker. Transfer functions were examined with the laryngeal cavity eliminated from the whole vocal tract, volume velocity distribution patterns were calculated, and susceptance matching analysis was performed between the laryngeal cavity and the vocal tract excluding the laryngeal cavity (vocal tract proper). It was revealed that the laryngeal cavity generates one of the formants of the vocal tract, which is th

Occupational magnetic field exposure and the risk of acoustic neuroma.

BACKGROUND: Acoustic neuroma is the intracranial tumor subtype showing the highest relative risk in relation to ionizing radiation but other environmental risk factors are largely unknown. This study was performed to investigate the effect of power frequency magnetic fields. METHOD: A total of 793 cases between 1987 and 1999 were identified through the Swedish cancer registry and 101,762 controls were randomly selected from the total population. Information about occupation was obtained from censuses and linked to gender specific job-exposure matrices based on actual measurements of 50 Hz mag

The translabyrinthine approach for acoustic neuroma and its common complications.

A retrospective analysis of 15 cases intracanalicular acoustic neuroma that undergone tumour excision by translabyrinthine approach spanning from August 1996 until December 2002 is presented. The main presenting complaints are unilateral hearing loss (100%) and tinnitus (86.7%). The mean age of presentation was 48.5 years old. Magnetic resonance imaging is the most important investigation tool to diagnose acoustic neuroma. At six months post operatively, the facial nerve was normal or near normal (grade I and II) in 46.6%, grade III to IV in 46.6% and grade V to VI in 6.7% of the cases respect

Mobile phone use and acoustic neuroma risk in Japan.

OBJECTIVES: The rapid increase of mobile phone use has increased public concern about its possible health effects in Japan, where the mobile phone system is unique in the characteristics of its signal transmission. To examine the relation between mobile phone use and acoustic neuroma, a case-control study was initiated. METHODS: The study followed the common, core protocol of the international collaborative study, INTERPHONE. A prospective case recruitment was done in Japan for 2000-04. One hundred and one acoustic neuroma cases, who were 30-69 years of age and resided in the Tokyo area, and

Setting the standard--UK neurosurgical acoustic neuroma practice.

INTRODUCTION: The aim of this study was to determine how current practice in the UK and Ireland complies with the Clinical Effectiveness Guidelines for the Management of Acoustic Neuromas. MATERIALS AND METHODS: A survey of units and consultants using a standardised questionnaire was carried out. RESULTS: Fifty-six neurosurgeons treat acoustic neuromas in 33 out of 34 units. In 27 units, one or two surgeons specialise in this area. Caseload per annum per surgeon ranged from 2 to 30, median 15. Forty-one neurosurgeons (75%) work with an ENT surgeon. All surgeons use facial nerve monitoring du

Glucocorticoid receptors modulate auditory sensitivity to acoustic trauma.

Glucocorticoids are widely used to treat different hearing disorders yet the exact mechanisms of glucocorticoid action on the inner ear are not known. The inner ear of both humans and experimental animals demonstrate an abundance of glucocorticoid receptors (GRs) in both neuronal and non-neuronal tissues. In this review, we discuss how activation of the hypothalamic-pituitary-adrenal axis can directly modulate hearing sensitivity. Recent findings indicate that several factors define the responsiveness of the peripheral auditory system to glucocorticoids including the concentration of agonist,

[Impact of sound signals of high intensity on the acoustic analyzer].

Guinea-pig experiments were made for objective assessment of the effects of high-intensity noise on the acoustic analyzer. Noise with intensity above 100 dB for 11 hours is accompanied with thresholds rise by 20 dB. In some animals hearing loss was observed for several weeks after discontinuation of a sound impact. In the presence of initially high conductive hypoacusis intensive sound load may cause short-term sound threshold lowering with further damage to neurosensory cochlear elements.

Cervical dystonia responsive to acoustic and galvanic vestibular stimulation.

We examined the effects of acoustic and galvanic vestibular stimulation in a patient with cervical dystonia. Acoustic stimulation consisted of three conditions: "baseline" (no stimulation), "vestibular" (500 Hz bone-conducted tone bursts), and "control" (5,000 Hz tone bursts). Rectified electromyographic activity in the sternocleidomastoid was measured. Galvanic stimulation (1.5-2.5 mA current steps) was delivered to the mastoids, and head acceleration was measured. Vestibular acoustic stimulation reduced neck muscle activity between 16% and 44% (P < 0.001), and galvanic stimulation reduced he

Acquired port-wine stain related to acoustic neuroma.

Port-wine stains are frequently seen congenital vascular malformations consisting of ectatic dermal capillaries. Acquired port-wine stain that develops later in life is an uncommon vascular lesion that is morphologically identical to a congenital port-wine stain. In the majority of acquired port-wine stains, the aetiology is unknown, but trauma is an important causative factor. Other proposed aetiologies include chronic sun exposure, hormonal changes, frostbite injury, obstruction of the peritoneovenous shunt, herpes zoster infection, and cerebral arteriovenous malformation. Here we report the
