Influence of smoking and race on immunoglobulin G subclass concentrations in early-onset periodontitis patients.

Recent data indicate that smoking is an important risk factor for the development of periodontitis. Smoking is also known to reduce serum immunoglobulin G (IgG) levels. Interestingly, patients with the localized form of early-onset periodontitis (LJP) have elevated levels of serum IgG2, and those who smoke are not clinically different from nonsmoking LJ subjects. In contrast, patients with the generalized form of early-onset periodontitis (G-EOP) who smoke have more extensive destruction than their nonsmoking counterparts. Given the effects of smoking on EOP and the association of IgG2 with le

[The indications for the surgical treatment of chronic periodontitis of the permanent teeth in children and adolescents of different health groups].

Chronic granulating and granulomatous periodontitis of 250 permanent teeth was treated in children and adolescents aged 7 to 17, surgery had to be resorted to in 44 (18%) cases. Three groups of indications for radical treatment of chronic periodontitis in children were distinguished: difficulties in or impossibility of conservative treatment, inefficacy of conservative treatment, and somatic indications. The conservative and surgical methods together are most often indicated for patients in whom the adverse conditions of treatment (the first group of indications) are associated with an aggrava

Periodontitis associated with plasminogen deficiency: a case report.

BACKGROUND: Plasminogen deficiency is a rare autosomal recessive disease, which is associated with aggressive periodontitis and gingival enlargement. Previously described treatments of plasminogen deficiency associated periodontitis have shown limited success. This is the first case report indicating a successful therapy approach consisting of a non-surgical supra- and subgingival debridement in combination with an adjunctive systemic antibiotic therapy and a strict supportive periodontal regimen over an observation period of 4 years. CASE PRESENTATION: The intraoral examination of a 17-year-…

Prevalence of clinical attachment loss in adolescents in Santo Domingo, Dominican Republic.

BACKGROUND: Data on periodontal conditions in adolescents in the Dominican Republic are scarce. The aim of the present cross-sectional study was to estimate the prevalence of periodontal attachment loss among Dominican adolescents. This study did not attempt to classify the disease into aggressive and chronic periodontitis. METHODS: A random sample of 2,007 Dominican adolescents was obtained. A probability, weighted sample was selected using a complex, multi-stage probability sampling design. The study was clustered in 26 schools and 106 classes. The study subjects were clinically examined un

[Subjective, functional and microbiological parameters of the oral cavity in gastroesophageal reflux patients with acidic and subacidic refluctant].

The aim of the study was to compare dental and periodontal status, oral hygiene, salivation rate, functional parameters of saliva and oral microbiota composition in patients with gastroesophageal reflux disease (GERD) with acidic and subacidic refluctant. The study comprised 69 participants divided in 3 groups: 22 healthy volunteers (controls) and 2 main groups: 25 GERD patients with acidic (group 2) and 22 patients with subacidic refluctant (group 3). Poor dental and periodontal condition was revealed in group 3 patients probably because of aggressive intestinal content reflux in the oral cav

8-year follow-up of central giant cell lesion mimicking apical periodontitis.

INTRODUCTION: Lesions of nonendodontic origin may mimic apical periodontitis. Central giant cell lesions (CGCLs) are aggressive or nonaggressive benign idiopathic intraosseous lesions of the jaw. This report describes a case of a CGCL in the periapical region of teeth #21-#26 of a 17-year-old female who sought orthodontic care because of a change in the position of tooth #23. METHODS: Clinical examination revealed mild facial asymmetry caused by increased volume in the mental region and cortical bone expansion but no cortical disruption. A panoramic radiograph showed a well-defined radiolucen

Argeloside I inhibits the pathogenicity of Porphyromonas gingivalis TDC60.

Porphyromonas gingivalis, a major pathogen associated with chronic periodontitis, produces several virulence agents in the outer cell membrane, including gingipains and hemagglutinins. These virulence factors enable the bacteria to adhere to periodontal tissue and degrade host proteins to obtain the nutrients needed for dental plaque formation. P. gingivalis TDC60 was recently identified as the most aggressive P. gingivalis strain to dates. In this study, we isolated a known pregnane glycoside, argeloside I, from the aqueous extract of Solenostemma argel leaves. Argeloside I completely hindere

Salivary Microbiome Shifts in Response to Periodontal Treatment Outcome.

PURPOSE: Periodontitis is linked to a localized dysbiotic microbial shift. This trending may often not be evident due to deep taxonomic changes of low abundance organisms and lack of consideration of variations in the treatment response. By using next generation sequencing this study aims to evaluate the salivary microbiome dynamics of periodontal treatment and the implication of treatment outcome EXPERIMENTAL DESIGN: Patients with generalized aggressive periodontitis are treated non-surgically and followed up for 6 months. Saliva is collected for microbiome profiling by next generation sequen…

Lung Abscess Caused by Tannerella forsythia Infection: A Case Report.

BACKGROUND: Tannerella forsythia is a gram-negative anaerobic bacterium commonly found in the oral cavity. It is among the common pathogenic bacteria associated with gingivitis, chronic periodontitis, and aggressive periodontitis. However, there is currently no literature discussing lung abscesses primarily caused by T. forsythia infection. PRESENTATION: This article presents the case of a 55-year-old male with a massive lung abscess. The patient underwent ultrasound-guided percutaneous drainage, and the sample was sent for pathogen metagenomic next-generation sequencing (mNGS) testing. The t…

Cyclosporin A-induced gingival hyperplasia in psoriasis: review of the literature and case reports.

Cyslosporin A (CyA) treatment of psoriasis is warranted in severe cases where other, conventional antipsoriatic approaches have failed. Gingival hyperplasia is a rare side effect of cyclosporin A treatment in psoriasis patients. Previous studies in cyclosporin A-treated patients (mostly transplant recipients) have demonstrated correlation between cyclosporin A serum levels and oral hygiene status on the one hand, and the prevalence and severity of this overgrowth on the other hand. Severe cases of gingival overgrowth may call for radical treatments such as periodontal surgery. Our aim was to p

Metallothioneins in Failure of Dental Implants and Periodontitis Down Syndrome Patients.

BACKGROUND: Sometimes dental implants seem to be the only therapeutic alternative for the oral rehabilitation of patients with Down syndrome, given that they usually lose all their teeth early due to suffering aggressive periodontitis and they do not usually have the skills required to wear removable prostheses. However, the evolution of dental implants in these patients shows very adverse results. It is possible that basal genetic alterations, or at least some characteristics of these, may underlie these clinical results. The metabolic pathway of metallothioneins, molecules with an important …

Periodontitis as a risk factor of atherosclerosis.

Over the last two decades, the amount of evidence corroborating an association between dental plaque bacteria and coronary diseases that develop as a result of atherosclerosis has increased. These findings have brought a new aspect to the etiology of the disease. There are several mechanisms by which dental plaque bacteria may initiate or worsen atherosclerotic processes: activation of innate immunity, bacteremia related to dental treatment, and direct involvement of mediators activated by dental plaque and involvement of cytokines and heat shock proteins from dental plaque bacteria. There are

The bleeding site: a multi-level analysis of associated factors.

AIM: To evaluate the association between the probability of a sulcus/pocket to bleed on probing (BoP) and patient related as well as site-specific characteristics. METHODS: Data from 88960 sites were retrospectively derived from the clinical record charts of 601 adult patients. BoP (positive/negative) had been recorded at the initial periodontal visit after probing pocket depth (PPD) assessment. To analyse the influence of patient-, tooth- and site-related factors on the probability for a site to be BoP+, a logistic, 3-level model was built with BoP as the binary outcome variable. RESULTS: (
