Pediatric IOL implantation: the KKESH experience.

BACKGROUND: Intraocular lens (IOL) implantation is increasingly accepted as a means of correcting pediatric aphakia. However, few large seri … ercentage of patients with penetrating trauma (24%) and a significant level of poor compliance with amblyopia treatment (33%), one third of patients saw 20/40 or better, and 60% had at least 20/80 acuity. Improved visual outcome correlated with later onset and shorter duration of opacity and compliance with occlusion therapy. The posterior capsule was left …

[Plexiform neurofibroma of the upper eyelid and orbit in a patient with von Recklinghausen's disease--case report].

Neurofibromatosis type 1 is an autosomal dominant genetic disease, which belongs to the neuro-oculo-cutaneous syndromes (phakomatoses). The authors present a case of a girl with familial neurofibromatosis. In addition to typical skin lesions and Lisch nodules on the iris, the plexiform neurofibroma of the upper eyelid and orbit as well as hamartoma of the central nervous system were observed. Due to the massive infiltration of the left upper eyelid causing its complete ptosis and, in turn, the amblyopia of the left eye, the tumour was partially rexcised. This lesion is not totally removable in…

Unilateral Oil Droplet Cataract in a Child: Importance of Early Detection and Intervention.

Oil droplet cataract is a rare pediatric lens opacity that can cause significant amblyopia if not promptly diagnosed and treated. A 5-year-old male presented with intermittent exotropia and reduced vision in the left eye. Visual acuity was 6/6 in the right eye and 5/60 in the left eye, improving to 6/36 with correction. Retinoscopy revealed an oil droplet reflex in the left eye. Systemic evaluation ruled out galactosemia. Phacoemulsification with intraocular lens implantation was performed, followed by refractive correction and occlusion therapy. At 3 months, left eye vision improved to 6/24 u…

Aberrant regeneration of accommodation in pediatric inferior division oculomotor palsy.

Aberrant regeneration occurs in forms of oculomotor motor nerve palsy and frequently involves the pupil, but the incidence and functional impact of ciliary muscle involvement in pediatric patients is sparsely reported in the literature. A 4-year-old girl presented with inflammatory oculomotor motor nerve paresis affecting the inferior division. Initial treatment focused on her inability to accommodate through her physiologic +2.5 D hyperopia and the prevention and treatment of amblyopia. She subsequently developed aberrant regeneration of the pupil, with miosis on adduction. Following eye musc…

Temporal keratoconus in a pediatric patient.

PURPOSE: To report a pediatric patient with bilateral temporal keratoconus. OBSERVATIONS: A 14-year-old male presented with a two-year history of progressively worsening visual acuity in both eyes and suspicion for undiagnosed amblyopia in the right eye. Retinoscopy revealed a scissoring reflex in both eyes and corneal topography demonstrated high keratometry values (Kmax 57.9 D and 46.1 D in the right and left eyes, respectively), with relative temporal steepening approximately coinciding with the thinnest pachymetry in both eyes. Corneal cross-linking was recommended. CONCLUSIONS AND IMPOR…

[Analysis of factors causing visual disability and its preventive measures in China].

Analysis of sampling data from Chinese people in 1987 showed prevalence of visual disability was 10.08/1000. Major factors causing visual disability were congenital or hereditary defects, ametropia, amblyopia and corneal disease in children aged 0-14 years; senile cataract, trachoma and corneal disease in the elderly people aged 60 and over; and ametropia, amblyopia, cataract and corneal disease in the other people. Glaucoma and eye trauma were two major causes leading to severe visual disability. Suggestion of three-level prevention for visual disability in the country was put forward by the …

Management of monocular cataract with long-term dilation in children.

We describe the use of long-term dilation and patching therapy in the management of three cases of pediatric monocular cataract. The cataracts were felt to be visually significant in the undilated state but consistent with good formed vision in the dilated state. One patient had a congenital cataract managed from 4 weeks of life, and two patients had late diagnosed cataracts of indeterminate onset presenting with dense deprivation amblyopia. The congenital cataract patient ultimately demonstrated recognition acuity of 20/40, and both the older children ultimately demonstrated an acuity of 20/3…

Results of a randomised trial of treating abnormal hypermetropia from the age of 6 months.

Children who were abnormally hypermetropic at the age of 6 months were randomly allocated treatment with spectacles or no treatment. The eventual incidence of squint was the same in both groups (approximately 24%). The last known visual acuity of the two groups was not significantly different either. Therefore there is no indication to screen infants with a view to preventing squint/amblyopia by optical correction of hypermetropia. If, however, the children allocated treatment are divided into two subgroups--those who wore glasses consistently and those who probably or certainly did not do so-…

The fitting of contact lenses in aphakic infants.

Infants born with a unilateral congenital cataract have always been considered a particularly difficult group of patients to treat. Our experience in fitting contact lenses to such children, after cataract removal, is described. Contact lenses must be specially designed, to account for the significant differences in size and shape of the infant eye from that of an adult eye. A group of 15 children born with a unilateral cataract, were fit with contact lenses, and have been treated for amblyopia for an average of less than 2 years. Eight patients have obtained acuities measured by preferential …

Evaluating visual loss with multi-flash campimetry.

Multi-flash campimetry is a computer-implemented psychophysical technique that allows temporal resolution to be assessed in 120 points per eye in a half hour. It has previously been reported that the results can serve to distinguish between multiple sclerosis patients and control subjects. We have now used this technique to measure temporal resolution in patients with amblyopia, cataracts, optic neuritis or macular degeneration. Our results indicate that multi-flash campimetry is at least as sensitive to visual loss in patients with amblyopia or cataracts as visual acuity testing with the Snel…

Choice of surgical technique in the management of congenital cataract.

A group of patients with congenital cataracts which were operated by an aspiration technique was compared with a similarly aged group who were operated by a lensectomy technique. Of the 28 eyes in which the lens was aspirated, leaving the posterior capsule intact, nineteen required re-operation a total of 32 times to keep the pupillary area clear for refraction and accurate contact lens prescription. The re-operations were carried out during the first 18 months of life which are critical for visual development, and probably represent a substantial force in the genesis of the amblyopia which pr…

Refractive errors associated with hemangiomas of the eyelids and orbit in infancy.

Asymmetrical refractive errors, both astigmatic and myopic, were associated with infantile hemangiomas of the eyelids and orbit in 46% of 37 patients who had large lesions and upper eyelid involvement predisposing to the ammetropia. The axis of the astigmatic error related to the location of the eyelid hemangioma and correlated closely with keratometric measurements of corneal astigmatism. The refractive errors tended to be stable despite eventual resolution of the hemangiomas. Efforts to combat strabismic and refractive amblyopia were rewarding in many patients. A history of complete eyelid o…

Retinal Dysfunction in Patients with Congenital Fibrosis of the Extraocular Muscles Type 2.

INTRODUCTION: Congenital fibrosis of the extraocular muscles type 2 (CFEOM2) is a distinct non-syndromic form of congenital incomitant strabismus secondary to orbital dysinnervation from recessive mutations in the gene PHOX2A. The phenotype includes bilateral ptosis, very large angle exotropia, ophthalmoplegia, and poorly-reactive pupils. Other than amblyopia, afferent visual dysfunction has not been considered part of CFEOM2; however, we have repeatedly observed non-amblyopic subnormal vision in affected patients. The purpose of this study was to document this recurrent feature of the phenoty…

Visual outcome of three generations in a Turkish family with Marfan syndrome.

PURPOSE: The long-term outcome and postoperative visual acuity in a single family with ectopia lentis secondary to Marfan syndrome is presented. METHODS: Thirteen family members, ages ranging from 3 to 47 years, were evaluated from 2006 to 2008. Ten members were found to have primary or secondary visual problems secondary to ectopia lentis. RESULTS: Evaluation of 10 members with subluxated lenses in three generations showed that the leading cause of loss of vision (< 20/200) was amblyopia (n = 5), followed by surgical complications (n = 2) and retinal detachment (n = 1). Moderate amblyopia w…

[Corneal disease in childhood-Hereditary, degenerative or infectious?].

BACKGROUND: Adequate visual acuity significantly contributes to the age-appropriate development of children's neurobehavior. Infantile corneal opacities are rare but implicate a high potential for amblyopia. OBJECTIVE: This review aims to provide an overview of the most common causes of infantile corneal opacities and highlights ophthalmopathological correlations. METHODS: The following review is based on an extensive literature search. RESULTS: If metabolic diseases, traumatic or infectious events can be excluded as a cause for an infantile corneal opacity, it is important to focus on the …

[Organic causes of bad visual development in the child].

The eye trouble of organic origin must be diagnosed and treated before the installation of an irreversible amblyopia. This early diagnosis rests on the observation of the child by his parents but especially on the knowledge of the various clinical signs by the general practitioner or the pediatrist. Thus, the blindness and the behaviors which is referred to it (leucocorie, strabism, nystagmus, or sometimes exophtalmy) must alert the doctor who will then address the child towards the ophthalmologist. This one will be able to highlight a malformation of the anterior segment, a cataract or glauco…

Pars plana lensectomy for the management of ectopia lentis in children.

BACKGROUND: Nontraumatic ectopia lentis arises from a variety of inherited disorders. It may also occur as an isolated abnormality in an otherwise healthy individual. Surgical intervention is necessary when lens subluxation causes a significant refractive error resulting in amblyopia. PATIENTS AND METHODS: We report the results of pars plana lensectomy for nontraumatic ectopia lentis in 24 eyes of 13 patients and provide data on the merits of this surgical approach. better in 17 of 22 eyes and 6/12 or better in 19 of 22 eyes.
