Treatment of erosive reflux oesophagitis: a double-blind multicentre trial with nizatidine 300 mg b.i.d. versus placebo.

Histamine H2-receptor antagonists at conventional doses have proved only partly efficacious in the treatment of reflux oesophagitis. The aim of this study, therefore, was to compare the efficacy of high doses of nizatidine (300 mg b.i.d.) versus placebo in 117 patients with grade I and II oesophagitis. After 6 weeks' treatment, 70.6% of nizatidine-treated patients showed complete endoscopic healing against 25.4% of placebo-treated subjects (p less than 0.001). After 12 weeks, the respective healing rates were 77.5% and 47.4% (p less than 0.01). Nizatidine also proved significantly more effecti

A comparison of roxatidine acetate and ranitidine in gastric ulcer healing.

A randomised multicentre double-blind study was conducted to compare the efficacy and safety of roxatidine acetate 75 mg twice daily and ranitidine 150 mg twice daily in 295 patients with endoscopically confirmed gastric ulcers. Substantial reductions in ulcer diameters and healing rates of 85.6 and 88.2% for roxatidine acetate and ranitidine, respectively, were obtained after 8 weeks of treatment. There was no difference in healing rates between smokers and non-smokers in either group. The relief of day and night-time epigastric pain was comparable for both treatment groups, as was antacid ta

[Food-induced ileus associated with intraperitoneal adhesions].

Six patients were treated for food ileus within a three-year period. In three patients an obstructing phytobezoar (made up of bran, sauerkraut and pumpkin in one case each) was removed at surgery from the small intestine. In three other patients ingestion of a grapefruit, bran and turnip led to an ileus, which lasted for 3-9 days but responded to conservative treatment after 3-9 days. The common finding in all six patients was severe intraperitoneal adhesions of the small intestine (after repeated laparotomies in five, due to Crohn's disease in one). A high content of indigestible roughage in

Triletide in a cimetidine-controlled clinical evaluation in duodenal ulcer patients.

A controlled study was carried out in 20 out-patients with endoscopically confirmed active duodenal ulcer to compare the effectiveness and tolerance of a new cytoprotective agent, triletide, with an established antisecretory agent, cimetidine. Patients were allocated at random to receive 8-weeks' treatment with either 1.5 g triletide per day or 1.2 g cimetidine per day. The results showed that all patients experienced improvement of the ulcer condition by the end of treatment, the vast majority being found endoscopically healed. There was no statistically significant difference between the gro

Cytoprotective approach to peptic ulcer therapy: a preliminary dose-finding clinical investigation with triletide.

Thirty out-patients with gastric (8) or duodenal (22) ulcers were randomly assigned to receive a daily dose of 1.0, 1.5 or 2.0 g triletide over a period of 8 weeks. Endoscopic findings and overall clinical rating indicated that triletide effectively accelerated ulcer healing in a proportion of patients (73% of responders), with a trend for better response at doses higher than 1 g/day, regardless of the ulcer location. Heartburn, epigastric pain and concomitant antacid consumption improved significantly in each dose group, but to a significantly greater extent in the highest dose group than in

Pyrogastrone treatment of peptic oesophagitis: analysis of 104 patients treated during a 3 1/2-year period.

A retrospective analysis was carried out in 104 patients with peptic oesophagitis treated with Pyrogastrone (carbenoxolone with alginate-antacid) during a 3 1/2-year period. There wer 3 treatment groups: one had Pyrogastrone alone, another pyrogastrone with metoclopramide and in the third, both agents were given together with cimetidine. Highly significant symptomatic improvement was achieved in 85% of 96 patients treated between 4 and 8 weeks and equally significant endoscopic healing was noted in 76% of 55 patients for the same period. Best results were seen in those given Pyrogastrone alone

Overlap of functional dyspepsia and GERD--diagnostic and treatment implications.

GERD and functional dyspepsia are the two most prevalent upper gastrointestinal disorders. Gastro-oesophageal reflux is most commonly diagnosed using the cardinal symptoms of heartburn and regurgitation. Patients might also be diagnosed using a questionnaire, after empiric treatment with an acid suppressant, after upper endoscopy or by pH testing. Functional dyspepsia is best diagnosed using symptoms outlined by the Rome committee in conjunction with a normal upper endoscopy. Theoretically, distinguishing these two populations should be easy for all health-care providers. In reality, however,

Helicobacter pylori treatment instead of maintenance therapy for peptic ulcer disease: the effectiveness of case-finding in general practice.

BACKGROUND: Maintenance therapy with acid-inhibiting medication is common in general practice. Since the eradication of Helicobacter pylori has become the treatment of choice for peptic ulcer disease, H. pylori treatment could replace maintenance therapy in patients with an ulcer history. AIM: To determine the effectiveness of a full peptic ulcer disease history case-finding strategy, together with subsequent H. pylori testing and treatment, in discontinuing maintenance therapy. METHOD: Patients were included from seven general practices, who had been using acid-inhibiting medication for mor

Halitosis and Helicobacter pylori. A possible link?

The exact pathophysiological mechanism of halitosis is not clear, and in many patients the etiology is an enigma. We followed three couples in which one member or both had halitosis. All the subjects had evidence of Helicobacter pylori infection. All received a treatment course of colloidal bismuth subcitrate four times a day and 250 mg metronidazole three times a day. There was impressive improvement in their symptoms, the halitosis disappearing along with eradication of the organism. We call the attention of physicians to the possible connection between halitosis, H. pylori infection, and fa

Xerostomia. A neglected symptom.

Xerostomia, the subjective feeling of dry mouth caused by a severe reduction in the flow of saliva, is a common problem that is particularly prevalent among the aged. It has become increasingly evident that dry mouth is associated with a number of serious systemic conditions and diseases. Among these are the intake of commonly prescribed medications, autoimmune diseases, and irradiation to the head and neck. The diminution in the flow of saliva may profoundly affect oral health, disturb digestion and speech, and seriously impair the patient's quality of life. Food avoidance, nonabsorption of s

[Therapy of acute pancreatitis].

The unpredictable course of acute pancreatitis needs a careful surveyance of the patient in the first days of the acute attack in order to apply therapeutic measures adequate to the severity of the symptoms. The avoidance of food or drink and gastric suction appears to be sufficient to prevent endogenous stimulation of the gland while there is probably no benefit of anticholinergic drugs or carboanhydrase inhibitors. Early adequate substitution of fluids using watery solutions, plasmaexpanders or blood is of decisive importance. For treatment of pains spasmoanalgetics, synthetic opium derivati
